Ruptures of the posterior cruciate ligament (PCL) and especially proximal bony avulsion fractures in children are very rare. This in combination with a rupture of the popliteal artery is extremely rare. Thus, an exact incidence is not available from the literature. Overall, these injuries are severe and often lead to chronic knee instability. We report a case of a 9-year-old boy who suffered a traumatic displacement of the left knee with a rupture of the popliteal artery. Prior to transfer to our department, he was treated by a saphenous vein bypass graft and by a transfixation of the knee usi

BACKGROUND: The optimal surgical treatment of Ogden Type IV tibial tubercle avulsion fractures (TTAFs) remains controversial, given this injury pattern's rarity and its multiplanar involvement of the proximal tibial physis. Small, single-center studies suggest type IV fractures have higher complication rates and more variable fixation constructs than other TTAF types. The purpose of this study is to delineate fixation constructs among surgically managed type IV TTAFs and identify factors associated with postoperative complications.
METHODS: A retrospective, multicenter cohort study was conduc

Posterior cruciate ligament (PCL) avulsion fractures at the tibial attachment site are managed using various techniques. Some surgeries involve internal fixation with an adjustable double-loop plate, anterior-to-posterior suture suspension fixation, hollow lag screw fixation, and steel wire fixation. In this case, an X-shaped knot and double posteromedial portals are used to retain the posterior septum for fixation. In this technique, we describe double posteromedial portals are used in this method. The internal joint is fixed with an X-shaped knot, and the external joint is fixed with SwiveLo

BACKGROUND: Intramedullary screw (IMS) fixation was wildly used in fifth metatarsal base fractures (FMBFs) and the results were satisfactory. However, in the comminuted osteoporosis or small displaced avulsion FMBFs, anatomical reduction and stable fixation could not be achieved with IMS. The Locking Compression Plate (LCP) distal ulna hook plate fixation was a novel alternative fixation method. The aim of this retrospective cohort study was to determine if LCP distal ulna hook plate fixation resulted in improved outcomes compared to the traditional IMS fixation in displaced avulsion FMBFs.

AIMS: The purpose of this study was to compare symptomatic treatment of a fracture of the base of the fifth metatarsal with immobilisation in a cast. Our null hypothesis was that immobilisation gave better patient reported outcome measures (PROMs). The alternative hypothesis was that symptomatic treatment was not inferior.
PATIENTS AND METHODS: A total of 60 patients were randomised to receive four weeks of treatment, 36 in a double elasticated bandage (symptomatic treatment group) and 24 in a below-knee walking cast (immobilisation group). The primary outcome measure used was the validated V

Arthroscopic techniques have gradually replaced open surgery as a routine method for treating posterior cruciate ligament tibial avulsion fractures. However, traditional arthroscopic surgery is associated with problems such as limited visualization, unstable fixation, and unsatisfactory reduction. By comparing various methods reported in the literature and integrating clinical experience, our team has developed an optimized technique to address current surgical challenges. Using dual posteromedial portals, we adopt a "single-suture" method, in which the same epidural needle and a single suture

BACKGROUND: Posterior cruciate ligament (PCL) avulsion fractures at the tibial insertion can cause posterior knee instability and accelerate osteoarthritis. Meyers-McKeever type II/III fractures require surgery. Traditional open approaches are traumatic, while arthroscopic techniques are limited by equipment, learning curves, and fixation efficacy.
OBJECTIVE: To evaluate the short-term clinical outcomes and feasibility of a combined vertical medial posterior incision approach using a small T-plate and high-strength suture fixation for isolated PCL avulsion fractures.
METHODS: We retrospectiv

This is the first report of acute avulsion fracture of the ischial tuberosity (AFIT) treated by suture anchor fixation using the suture bridge technique. A 13-year-old boy developed sudden, severe right hip pain while running a short distance. Pelvic images revealed the avulsion fracture of the right ischial tuberosity with displacement of the avulsed fragment by 35 mm. We performed open reduction and reconstruction fixation 5 days after the injury using the subgluteal approach with longitudinal skin incision. Four suture anchors were set at the ischial tuberosity, and the avulsed fragment was

Tibial-sided posterior cruciate ligament avulsion fractures are challenging injuries that often occur concomitantly in the setting of multiligament knee and other soft-tissue injuries. There is no consensus on the optimal surgical approach or timing of treatment for these injuries. This Technical Note describes the fixation of a displaced posterior cruciate ligament avulsion fracture with concomitant grade 3 medial collateral ligament injuries and bucket-handle lateral meniscus tears using open and arthroscopic techniques. This method allows the surgeon to address multiple pathologies in a sin

BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries.
INDICATIONS: The complex triad injury presente

Gustilo type IIIB open forearm fractures associated with avulsion injuries of multiple extensor tendons are difficult to reconstruct. Not only are bones, nerves, blood vessels, and soft tissues injured, but also tendons directly related to hand function. A 74-year-old man sustained an injury to his dominant right hand after being hit by a heavy pulley. The patient was diagnosed with a Gustilo type IIIB open forearm fracture, and multiple extensor tendons were avulsed from the musculotendinous junction. On the day of injury, the radius was fixed using a volar locking plate, and the ulnar head w

BACKGROUND: Degloving of the sole of the foot is a rare and serious injury because the heel pad cannot be replaced by similar tissue. The management is challenging and only a few cases have been reported with different treatment regimens.
METHODS: Here, we report on a 46-year-old female patient with complex foot trauma consisting of complete avulsion of the heel pad at the hindfoot and a soft tissue defect at the posterior aspect of the heel accompanied by rupture of the anterior tibial tendon and fractures of the talus, calcaneus and midfoot. The sole of the foot was fixed to the calcaneus w

Extra-articular hip impingement from prior traumatic injury to the anterior inferior iliac spine (AIIS) is an uncommon cause of groin pain in young athletes. Currently, the most common treatment for this injury is arthroscopic decompression. However, hip arthroscopy is not universally available and requires advanced skills. We report 2 cases of the development of extra-articular hip impingement from unusual bony exostoses off the AIIS after traumatic injury in 2 young athletes who underwent open surgical resection. The multidisciplinary sports medicine team should be aware of the development o

Proximal humerus fractures are common in the older population. These fractures can be associated with dislocation of the shoulder. Four-part fracture-dislocations and head-splitting 3-part fractures are generally indicated for shoulder hemiarthroplasty in the older population. When such surgery is warranted, inspection of the glenoid during the procedure is indicated, as loss of evaluation may lead to postoperative shoulder instability. There is value in alerting orthopedic surgeons to the possibility of anterior glenoid rim fracture in association with proximal shoulder fracture-dislocation.

BACKGROUND: A greater trochanteric (GT) fracture after primary total hip arthroplasty (THA) is a clinically relevant complication that can lead to gait abnormalities and chronic lateral hip pain. Early postoperative GT avulsion fractures are rare, possibly resulting from poor bone quality or the use of two transosseous drill holes for posterior capsular repair. Computer- and robotic-assisted surgery may increase fracture risk due to cortical defects from temporary pins. This study aimed to assess the rate of early GT fractures and identify contributing factors and prevention strategies.

A 77-year-old, right-hand-dominant woman presented after a low-energy fall with dorsal displacement involving the proximal and distal interphalangeal joints of the right small finger, along with a volar plate avulsion fracture at the base of the middle phalanx. Closed reduction was performed without anesthesia, followed by static aluminum splinting and occupational therapy. At her two-month follow-up, she reported minimal pain, improved function, and maintained joint alignment. This case highlights successful conservative management of a rare double interphalangeal dislocation with associated

OBJECTIVE: Displaced tibial avulsion fractures of the posterior cruciate ligament (PCL) significantly compromise knee stability; however, existing clinical data regarding treatment and prognosis are limited. There exists a paucity of biomechanical research concerning various surgical methods for tibial avulsion fractures of the PCL, and optimal management remains controversial. Therefore, the objective of this study was to investigate the biomechanical stability of displaced tibial avulsion fracture using suture bridge fixation, screw fixation, and TightRope fixation at varying flexion angles.
