Two patients were admitted to our hospital with a diagnosis of miliary tuberculosis (TB). Gallium-67 citrate (Ga) scans and chest X-rays were used to localize the TB lesions. One case showed normal chest X-ray with significant gallium uptake in the lung while the other showed diffuse interstitial infiltrations of both lungs in chest X-ray with negative result in gallium scan. We suggest that the combination of Ga scans and chest X-rays might be indicated to avoid the discrepancy between the two images and missing early diagnosis of miliary TB.

A middle aged woman with a pyrexia of unknown origin was shown to have borderline lepromatous leprosy. Early gallium-67 scintigraphy demonstrated increased uptake in the subcutaneous tissues of the face and thighs. As a result of these findings skin biopsy was obtained from the right thigh which gave a diagnosis of borderline lepromatous leprosy. The authors have been unable to find other reports of gallium-67 scintigraphy in leprosy but the pattern of gallium-67 distribution should suggest the diagnosis.

A 19-year-old woman had a residual gallium-sequestering mediastinal mass after treatment for Hodgkin disease. Coincidentally, she also had hyperthyroidism. The initial concern was that the mass was residual Hodgkin disease. Thymic hyperplasia has been described in association with both these conditions. The mass disappeared after treatment of her hyperthyroidism.

A case of neoplastic angioendotheliosis of the lung is reported. A 56-year-old man presented with a 2-month history of fever, increased serum lactic dehydrogenase, and arterial hypoxemia. Chest radiography revealed diffuse reticulonodular opacity, whereas CT scanning showed diffuse small nodular areas of increased attenuation with indistinct borders. Diffuse pulmonary uptake of Ga-67 was noted in scintigraphy. Combination chemotherapy designed for lymphoma induced a remission of the disease.

Scanographic examinations with the use of citrate 67Ga in patients with respiratory sarcoidosis made it possible to discover valid incidence, localization and evidence of a pathological process. An unusual case is presented of bilateral breast uptake of [67Ga]citrate in a patient with a hypothalamic granuloma in the absence of galactorrhea is presented. A possible mechanism for this incidental finding is elevated prolactin levels, as other causes of gallium breast uptake such as drug therapy, and intrinsic breast disease, were not present.

A case of a 12-year-old boy with sacroiliitis documented by positive Tc-99m MDP and Ga-67 scans is described. Isolation of brucella melitensis from the blood and bone marrow established the diagnosis. He responded promptly to docycycline therapy. Throughout the course of his disease this boy had neither fever nor other signs of brucellosis, and x-ray was normal.

In 37 patients with seemingly localized non-Hodgkin's lymphoma of the Waldeyer's ring (WR-NHL), lymphangiography (LAG) and/or gallium-67 scans (Ga-67 scans) were done. Before these procedures, 20 patients were diagnosed as Stage I, and 17 as Stage II. LAG was done for 30, and Ga-67 scans for 32, 25 of whom had both. Five patients (16%) were upstaged to Stage III or IV by Ga-67 scans. Only one (3%) had abnormal LAG findings, in whom Ga-67 scans also showed abnormal accumulation in the para-aortic region. Because of this low positive rate, LAG is not recommended for staging of WR-NHL.

Sixty-six patients with lung cancer underwent mediastinal staging with gallium scanning, CT scanning and mediastinal exploration at mediastinoscopy and/or thoracotomy. Histological findings at time of mediastinal exploration were correlated with the results of the non-invasive staging scans. Leiomyosarcoma of the large intestine, excluding the rectum, is a rare lesion and only 58 cases have been previously reported. Significant uptake of radioactivity of gallium-67 to the tumor has not been reported and this report may be the first case. Although preoperative diagnosis is considered to be difficult, barium enema study and colonofiberscopy disclosed the findings suggesting extracolic growth of the tumor in our case, which are thought to be characteristics of nonepithelial tumors. Gallium-67 citrate scanning may be useful for confirming the diagnosis.

The association between increased serum angiotensin converting enzyme (ACE) activity and active sarcoidosis is well documented. During a recent outbreak of acute histoplasmosis, a disease that shares many of the clinical and roentgenographic features of sarcoidosis, we examined serum ACE activity. The value of Ga-67 scanning to detect acute infectious lung disease has been described. We present a patient who apparently improved both clinically and radiographically after acute Legionnaires' pneumonia. Five months later a relapse developed. During his relapse the pulmonary uptake of Ga-67 and the appearance of chest x-rays were disparate. We suggest that pulmonary Ga-67 uptake may be a more sensitive indicator of the resolution of pneumonia than is chest radiography. Therapeutic success may be assumed when pulmonary Ga-67 uptake is absent.

FDG PET CT has limitations in breast cancer due to its low tracer uptake in Grade 1 and 2 tumors, estrogen receptor positivity, lobular histology, and osteoblastic skeletal metastases. Trivehexin PET targets αvβ6 integrin, which is relatively specific for epithelial cancer cells. We present a case of a 67-year-old woman with recently diagnosed metastatic breast cancer. Trivehexin PET not only showed lesions with a higher target-to-background ratio compared with FDG PET but also depicted additional metastatic sites.

We present a case of metastatic castration-resistant prostate cancer (mCRPC) with prior hepatic involvement who ultimately received 177 Lu-PSMA therapy. During the posttreatment scan following the third cycle, atypical uptake was observed in the left ankle, which was also evident as FAPI uptake on the 68 Ga-FAPI-46 PET/CT performed for restaging, and was subsequently confirmed as avascular necrosis through MRI correlation. This case illustrates the false-positive uptake of 177 Lu-PSMA-617 and 68 Ga-FAPI-46 in avascular necrosis of the talus.
