Dominant strictures in primary sclerosing cholangitis: A multicenter survey of clinical definitions and practices.

Dominant strictures (DSs) of the biliary tree occur in approximately 50% of patients with primary sclerosing cholangitis (PSC) and may cause … total of 54 faculty (41.2%) completed the survey, of whom 24 (44.4%) were hepatologists, 21 (38.9%) gastroenterologists, and 9 (16.7%) advanced endoscopists. One of the major study findings was that there was heterogeneity among participants' definition, evaluation, management, and follow-up of DSs in PSC. The majority of participant responses were in accordance with …

Endoscopic assessment of the oesophageal features of eosinophilic oesophagitis: validation of a novel classification and grading system.

OBJECTIVE: Abnormalities are commonly identified during endoscopy in eosinophilic oesophagitis (EoE). There is no standardised classificatio … repe paper oesophagus. Endoscopic videos from 25 patients with EoE and controls were reviewed by 21 gastroenterologists. Interobserver agreement was assessed by estimating multi-rater κ and the proportion of pairwise agreement. RESULTS: Using the original grading system, agreement for rings, furrows and exudates was moderate (κ=0.38-0.46, 56-65% agreement) but poor …

Colonoscopy-related colonic ischemia.

Colonoscopy is a risk factor for colon ischemia. The colon is susceptible to ischemia due to its minor blood flow compared to other abdominal organs; the etiology of colon ischemia after colonoscopy is multifactorial. The causative mechanisms include splanchnic circulation impairment, bowel preparation, drugs used for sedation, bowel wall ischemia due to insufflation/barotrauma, and introduction of the endoscope. Gastroenterologists must be aware of this condition and its risk factors for risk minimization, early diagnosis, and proper treatment.

[A probable case of alveolar echinococcosis in Normandy, France].

Echinococcus multilocularis induced liver infection was diagnosed in a Moroccan patient. Diagnosis was based on CT scan results and Western Blot test. Contamination probably occurred in France, in the Cherbourg area where the patient travelled frequently and ate wild berries. This case and other recently reported cases outside the usual endemic areas (Besançon and the Massif Central) suggest that the Echinococcus multilocularis epidemic has moved towards the west of France. French gastroenterologists should be aware of this parasitic disease.

[The characteristics of treating gastric and duodenal peptic ulcer in workers in an oil-refining enterprise].

The article covers a topical problem--study of transitory disablement in oil-processing workers suffering from peptic ulcer. The authors demonstrate data on increased occurrence of peptic ulcer among those workers engaged mostly into the main industrial process and stress the great economic detriment caused by the disease. Application of endoscopy treatment and therapy within the local prophylactic department appeared to dramatically decrease the transitory disablement. The article could be interesting for gastroenterologists, industrial medicine officers.

Management of perianal fistulas in Crohn's disease: an up-to-date review.

Perianal disease is one of the most disabling manifestations of Crohn's disease. A multidisciplinary approach of gastroenterologist, colorectal surgeon and radiologist is necessary for its management. A correct diagnosis, based on endoscopy, magnetic resonance imaging, endoanal ultrasound and examination under anesthesia, is crucial for perianal fistula treatment. Available medical and surgical therapies are discussed in this review, including new local treatment modalities that are under investigation.

[The clinical picture, diagnosis and treatment of thoracalgias].

The clinical picture of 260 patients with thoracalgia was characterized by chest pain, thoracic spine pain, subcostal and epigastric pain. In several patients treatment (traditional) failed. Different diseases of the digestive organs were found: chronic cholecystitis, biliary dyskinesia, gastric and duodenal ulcers which significantly contributed to the painful syndrome. Cooperation of the neurologist, gastroenterologist and surgeon favoured adequate rehabilitation of this category of patients.

The Real Relevance of Nonacid Reflux in Pediatric Patients.

Gastroesophageal reflux (GER) is one of the most common presenting complaints to pediatric gastroenterologists, and a wide variety of symptoms in children are potentially attributable to typical or atypical GER. While reflux diagnosis and treatment paradigms classically have focused on targeting acid, there is a growing recognition of the prevalence and significance of nonacid GER in both children and adults. This review explores the role of nonacid reflux in pediatric patients, including definitions, associations with symptoms, pathophysiologic mechanisms, and treatment implications.

Acalculous Cholecystitis Due to Histoplasma capsulatum in a Patient With HIV Infection.

Although acalculous cholecystitis typically occurs in critically ill individuals, it has been described in patients with HIV infection. Recognition of AIDS-related acalculous cholecystitis is important for gastroenterologists to provide optimal therapy, since many different opportunistic pathogens may be involved. Histoplasma capsulatum causes a wide spectrum of infectious sequelae, including disseminated histoplasmosis in patients with HIV infection, but biliary tract involvement has been described infrequently. We present a case of acalculous cholecystitis caused by H. capsulatum in a patien…

Management of patients following detection of unsuspected colon lesions by PET imaging.

Positron emission tomography (PET) is a well-established and integral component of multimodality imaging in oncology. However, the expanded use of PET in oncological and also non-oncological imaging (such as in assessing inflammatory conditions) has identified more lesions or tumors at unsuspected locations, such as in the large bowel during examination of patients not known to have colorectal disease. We review the clinical significance of colon lesions that were discovered incidentally by PET imaging and management strategies for gastroenterologists.

Neoplastic precursors of the gallbladder and extrahepatic biliary system.

Premalignant lesions of the gallbladder and extrahepatic bile ducts have been poorly studied compared with their counterparts in the remainder of the gastrointestinal tract. Some of the problematic issues, such as the subjectivity of grading and determination of the relative risk of cancer, are challenging. Although the understanding of the biology of these lesions has improved in the past several years, there are still many issues that can only be addressed by close collaboration between gastroenterologists, radiologists, hepatobiliary surgeons, oncologists, and pathologists.

[Chronic inflammatory bowel disease and pulmonary involvement].

Pulmonary manifestations associated with chronic inflammatory bowel diseases (IBDs) are a factor of morbidity and mortality. This respiratory involvement can manifest in various ways (airways, interstitial, etc.) and remains rare, but is probably underestimated. Subclinical respiratory involvement is frequently found in IBD patients. Airway involvement with bronchiectasis is the most frequent respiratory manifestation. Management is based on a multidisciplinary approach (pulmonologist and gastroenterologist) and depends on the etiology.

Adult Celiac Disease: Recognition and Diagnosis for the Primary Care Provider.

Celiac disease is an immune-mediated enteropathy in response to dietary gluten causing malabsorption and a multitude of systemic changes. Although referral to a gastroenterologist is recommended, a primary care physician plays a crucial role in the recognition, diagnosis, and management of this condition. Systemic manifestations outside of the traditionally recognized gastrointestinal signs should prompt a primary care physician to begin the initial work up of this common gastrointestinal disease.

Anti-reflux procedures: complications, radiologic findings, and surgical and gastroenterologic perspectives.

This article provides an overview of the current surgical anti-reflux procedures and their imaging findings, as well as the surgical complications. Accurate and timely clinical assessment requires an engaged radiologist fluoroscopist who understands the perspectives of their interdisciplinary colleagues, including the surgeon and gastroenterologist. The complex pathophysiology calls for an interdisciplinary approach, and the radiologist needs to tailor their evaluation to answer the specific questions posed by their clinical colleagues and by the presenting symptomatology.

Transitioning Adolescents and Young Adults With Neurogastroenterology and Motility Disorders to Adult Care. Current Practices and Challenges in North America.

BACKGROUND: Pediatric neurogastroenterology and motility (NGM) disorders frequently persist into adulthood, yet there are no standardized transition of care (TOC) practices for this patient population. We characterized current TOC practices for adolescents and young adults with NGM disorders across North America. METHODS: A survey was distributed through listservs for North American pediatric gastroenterologists, registered dietitians, and advanced practice providers. The survey assessed institutional characteristics, TOC practices, team structure, handoff practices, barriers, and unmet needs…
