Anastomotic-Transverse Colon Fistula Following Gastrectomy: A Case Report of Delayed Diagnosis 14 Years After Surgery.

BACKGROUND Gastrocolic fistula is a rare complication of partial gastrectomy that can present with weight loss, diarrhea, and feculent vomiting. Most cases documented in the literature have occurred following non-physiological anastomoses, such as Billroth II procedures. This report describes the case of a 60-year-old man with a gastrocolic fistula from the gastrectomy anastomosis to the transverse colon 14 years after a Billroth I (gastroduodenal) anastomosis. CASE REPORT A 60-year-old male patient presented with a 6-year history of recurrent diarrhea, which had worsened over the past 6 month

Distal Gastrectomy with Billroth II Reconstruction is Associated with Oralization of Gut Microbiome and Intestinal Inflammation: A Proof-of-Concept Study.

BACKGROUND: Subtotal gastrectomy with Billroth II reconstruction (SGB2) results in increased gastric pH and diminished gastric barrier. Increased gastric pH following PPI therapy has an impact on the gut microbiome, intestinal inflammation, and possibly patient health. If similar changes are present after SGB2, these can be relevant for patient health and long-term outcomes after surgery. The aim of the study is to investigate whether SGB2 is associated with specific changes in gut microbiome composition and intestinal inflammation. PATIENTS AND METHODS: This cross-sectional proof-of-concept

Pancreatic somatostatinoma diagnosed preoperatively: report of a case.

CONTEXT: Somatostatinoma is a rare neoplasm of the pancreas. Preoperative diagnosis is often difficult. CASE REPORT: We report a 72-year-old woman with a pancreatic head tumor measuring 37 mm in diameter, and enlargement of the lymph nodes on the anterior surface of the pancreatic head and the posterior surface of the horizontal part of the duodenum. Laboratory data showed an elevated plasma somatostatin concentration. Examination of a biopsy specimen of the pancreatic head mass obtained by endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) showed histopathological features of a ne

Braun enteroenterostomy reduces delayed gastric emptying after pylorus-preserving pancreatoduodenectomy: a retrospective review.

BACKGROUND: Several recent studies have suggested that Braun enteroenterostomy (BEE) during conventional pancreatoduodenectomy might decrease delayed gastric emptying (DGE). However, the advantages and disadvantages of performing BEE during pylorus-preserving pancreatoduodenectomy (PPPD) remain controversial. METHODS: The medical records of 185 patients who underwent PPPD either with or without BEE between January 2008 and June 2013 were retrospectively reviewed, and the postoperative course of the 2 groups was compared. RESULTS: Ninety-eight patients underwent PPPD with BEE and 87 without B

[Surgical treatment of gastric outlet obstruction].

Today surgery for peptic ulcer disease is largely restricted to the treatment of complications. About two-thirds of operations for complicated peptic ulcer disease are due to perforations, about one-third of operations are necessary to stop peptic ulcer bleeding, despite endoscopic treatment. In rare cases, peptic ulcer penetration requires surgery. Approximately 1-2% of patients with peptic ulcer disease develop gastric outlet obstructions (GOO), and about 80% of GOO due to peptic ulcer disease are caused by duodenal ulcers. Through retrospective analysis authors make it one's aim to investig

Stent placement of gastroenteric anastomoses formed by magnetic compression.

PURPOSE: To evaluate the use of stents for prolonging the patency of gastroenteric anastomoses (GEA) induced by magnet compression. MATERIALS AND METHODS: Rare earth magnets were inserted perorally and serially in 15 dogs so as to mate across the gastric and jejunal walls. After magnet excretion, the resulting GEA was identified endoscopically, dilated (n = 1), and stented with bare (n = 2) or partially covered (n = 6) flared 10-mm or 12-mm Z stents. The GEA was followed at 2-4-week intervals for patency; malfunctioning shunts were irrigated, or dilated with angioplasty balloons. Gross and hi

Gastric mycosis following gastric resection and vagotomy.

In a prospective five-year follow-up study of 289 consecutive patients subjected to antrectomy and gastroduodenostomy with or without vagotomy, 130 patients underwent gastroscopy. Gastric mycosis was present almost exclusively in patients subjected to combined antrectomy and vagotomy (36%). Gastric acidity seemed to be of only minor or no importance in the development of the mycosis. The residual volume in the gastric remnant was significantly higher in patients with gastric mycosis. The impaired emptying of the gastric remnant is most likely a vagotomy effect and may be the main reason for th

Symposium on peptic ulcer disease: 3. Practical management of recurrent peptic ulcer.

Recurrent peptic ulcer usually develops as the result of an ill-advised or poorly executed operation. The commonest surgical error is an incomplete vagotomy. Diagnosis is made best by endoscopy. Mandatory investigation includes determination of serum gastrin and calcium, and measurement of basal and maximal acid output. Management is surgical and depends on the initial ulcer operation. Decision-making is aided by the Hollander insulin test, the secretin infusion test and occasionally by a technetium scan. There is no place for procedures that do not reduce acid output. Emergency treatment of a

The early postprandial dumping syndrome: prevention and treatment.

The early postprandial dumping syndrome can be prevented or minimized by the appropriate selection of the operative procedure to fit the patient and the peptic ulcer problem he presents, and by proper attention to diet in the early postoperative period. When it does occur, the syndrome usually responds favorably to dietary management and tends to spontaneously regress in severity with time. For these reasons further surgery is seldom required for the early postprandial dumping syndrome. In the patient who fails to improve with diet therapy and time and has disabling symptoms often accompanied

Comparison Between Billroth II and Billroth II + Braun Anastomosis in Gastrectomy for Gastric Cancer.

INTRODUCTION: Distal gastrectomy remains the predominant therapeutic approach for gastric cancer, with digestive tract reconstruction as an integral procedure. The implementation of Braun anastomosis following Billroth II anastomosis is common in distal gastrectomy. This retrospective cohort study evaluated the clinical utility of Braun anastomosis by comparing the outcomes and quality of life between Billroth II (B-II) and Billroth II with Braun (B-IIB) anastomosis in the treatment of gastric cancer. METHODS: A retrospective cohort study examined clinical and pathological data from 377 patie

Gastric remnant necrosis secondary to cholesterol crystal embolization after distal gastrectomy in a gastric cancer patient: a case report.

BACKGROUND: Distal gastrectomy with lymph node dissection, a standard operative technique for gastric cancer treatment, is safely performed because the stomach has a rich vascular supply. Gastric remnant necrosis caused by cholesterol crystal embolization following distal gastrectomy has not been described previously. We report a case of gastric remnant necrosis in a patient with cholesterol crystal embolization. CASE PRESENTATION: A 70-year-old man with a history of cholesterol crystal embolization presented to our surgery department with complaints of anorexia and dysphasia. He was diagnose

[A case of anastomotic recurrence of gastric cancer who underwent esophagojejunostomy, treated with S-1 monotherapy leading to complete response (CR)].

A 67-year-old male with an anastomotic recurrence of gastric cancer who underwent esophagojejunostomy, was treated with S-1 monotherapy leading to a complete response (CR). The patient was diagnosed with gastric cardia cancer and underwent a total gastrectomy with lymph node dissection. Two years and 2 months after surgery, the patient was administered S-1 100 mg/day (4-week administration and 2-week rest) because anastomotic recurrence was confirmed. The patient was unable to take anything by mouth, but this chemotherapy could be orally administered after one course. After six courses, tumor
