Doctors are optimistic in their perception of how acceptable endoscopy is for patients. We analyzed elements that contribute to a poor experience for the patient and the agreement between the perceptions of endoscopists and patients. A case of hematemesis caused by jejunal gastric intussusception diagnosed by gastroscopy is presented. The acute form of gastric intussusception is a rare condition after gastric surgery, but it is important to be aware of the remote possibility because of a tremendously high mortality, if treatment is delayed more than 48 hours. The gastroscopic evaluation of the acute type may be extremely difficult, especially after gastric resection, the survey being very poor.

Syphilis is a chronic systemic infectious disease. Gastric involvement is uncommon. We report of a 55-year-old female patient with a history of epigastric pain and a diagnosis of Syphilis confirmed by main clinical symptoms, tolulized red unheated serum test(TRUST) and treponema pallidum hemagglutination assay(TPHA). Gastric metastases from solid tumours are very infrequent and in most cases appear simultaneously with other metastases. The most frequent primary tumors are lung and breast. Clinical data and symptoms are non-specific, and can range from abdominal discomfort to massive gastrointestinal bleeding. The diagnoses must be established by gastroscopy and biopsy. We present an unusual case of digestive haemorrhage secondary to gastric metastases from oropharyngeal carcinoma.

The precipitating factor in bleeding oesophageal varices is unknown. To investigate this problem 10 patients who had bled from oesophageal varices were examined by manometry, pH monitoring and gastroscopy to determine whether gastro-oesophageal reflux occurred. No evidence for reflux was found. These results, together with previously published reports, cast doubt on peptic oesophagitis or reflux as an aetiological factor in the initiation of oesophageal variceal bleeding.

A 43-year-old male with a diagnosis of severe acute pancreatitis (SAP) was admitted to our hospital. After receiving the step-up approach for three weeks, the patient had a sudden hematemesis (nearly 400 ml). Intrathoracic gastric herniation after laparoscopic antireflux surgery is a rare but well known phenomenon. It may occur during the early and late postoperative period. We report on a patient with early onset of dysphagia after surgery due to a tight wrap. Subsequent vomiting and dysphagia increased due to a gastric herniation. After gastroscopy and bougienage, tension pneumothorax developed. The context and relationships are illustrated and discussed referring to the current literature.
