[Giant-cell temporal arteritis--indications for biopsy].

The authors summarize the knowledge of diagnostics of giant cell arteritis and attempt to determine the criteria for indication of temporal artery biopsy. The difficulties of diagnostics are documented by case-report of 75-year-old patient in whom considerably varied complex of symptoms of temporal arteritis combined with development of polymyalgia rheumatica has developed.

[Giant cell arteritis. A study of 191 patients].

BACKGROUND: The aim of the present was to study the clinical features of a wide series of patients with giant cell arteritis (GCA) diagnosed with accurate criteria and to evaluate the sensitivity of the criteria proposed by the ACR for classification of GCA. METHODS: A retrospective analysis of 191 patients with GCA, 184 of whom were diagnosed by biopsy and 7 due to their clinical manifestations was carried out. RESULTS: The age was 73 +/- 7 years with the most frequent symptoms being headache (87%), abnormalities in the temporal arteries (75%), general malaise (60%), rheumatic polymyalgia (

Color Doppler hemodynamics of giant cell arteritis.

OBJECTIVES: To determine quantitative and qualitative hemodynamic alterations within the ophthalmic, central retinal, and short posterior ciliary arteries in patients with giant cell arteritis (GCA) proved by biopsy specimen. DESIGN, PATIENTS, AND SETTING: A consecutive case series of patients with GCA referred to an urban eye hospital who were evaluated with color Doppler imaging that was used to analyze orbital blood flow velocities and vascular resistance in 22 consecutive patients with GCA compared with age and sex-matched controls. RESULTS: Patients with GCA all demonstrated significant

Atypical visual loss in giant cell arteritis.

Three patients with atypical ocular involvement due to histologically verified giant cell arteritis are reported. Prior to diagnosis, the first patient had periods of amaurosis fugax. He presented with normal vision. In spite of high-dose systemic corticosteroid therapy, he became blind in the terminal stage of his disease due to bilateral occipital cortex infarctions, verified by CT-scan. Autopsy revealed involvement of several intracranial arteries. In case No. 2 there was severe unilateral visual loss and cotton-wool exudates in both eyes. Central vision recovered after corticosteroid thera

[Giant cell arteritis--presentation of 3 cases].

Three patients with giant cell arteritis (GCA) observed recently in our department are described. One of them demonstrated clinical picture of polymyalgia rheumatica, the other two presented with clinical symptoms of GCA. This presentation seems to be very helpful for correct diagnosis in cases with difficult recognition.

[Giant cell arteritis--a genetically-determined disease?].

We report on two siblings with giant cell arteritis (GCA) and polymyalgia rheumatica (PMR). Temporal artery biopsies revealed GCA in three cases, and sclerosis of the wall of the temporal artery in one case. The HLA haplotype was identical in the first pair, and in the second pair HLA-A2, HLA-B39 and HLA-DR53 were identical. In three patients HLA-DR4 was demonstrated. Cases of familial aggregation of GCA have been reported in the literature. In several reports an increased prevalence of HLA-DR4 was described (about 40% of patients with GCA, compared with about 13% in the European population).

Giant cell lesions of the maxilla and paranasal sinuses.

Giant cell lesions of the maxilla and paranasal sinuses represent a rare, locally aggressive disorder which present as a soft tissue mass with distinct histologic and clinical features. There is considerable controversy on the therapeutic modalities, the prediction of clinical behavior based on histologic features, and whether these growths are reactive or neoplastic in nature. We present our clinical experience with four of these lesions. Follow-up ranged from 3 to 11 years. Our results and the rationale for aggressive surgical treatment will be discussed. We hypothesize that giant cell "gran

[Lymphomatoid papulosis and anaplastic giant-cell lymphoma].

INTRODUCTION: The association between lymphomatoid papulosis and malignant Hodgkin or non-Hodgkin lymphoma is well known but still raises the problem of nosology between these two pathologies. Is lymphomatoid papulosis a pseudolymphoma, a prelymphomatous state or a true skin lymphoma? CASE REPORT: We observed a patient who had lymphomatoid papulosis and anaplastic large-cell lymphoma within an interval of 8 years between. This case was particularly interesting because identical immunophenotypes were observed in the atypical large-cells of the skin and the lymphomatous cells of the lymph nodes

Giant cell arteritis involving the cerebral artery.

We describe a 59-year-old Japanese man with rare complications of giant cell arteritis. He presented with aortic insufficiency due to dilatation of the aortic root. Nine years after the occurrence of aortic insufficiency, he suffered from a cerebral infarction due to occlusion of the left middle and anterior cerebral arteries, showing giant cell arteritis.

Pulmonary giant cells and their significance for the diagnosis of asphyxiation.

This study was performed to prove whether the detection of polynuclear giant cells in lungs is useful for the diagnosis of asphyxiation due to throttling or strangulation. Therefore, lung specimens of 54 individuals with different natural and unnatural causes of death were investigated. In most lungs examined numerous alveolar macrophages with 1-2 nuclei were found. Polynuclear giant cells, which were arbitrarily defined as alveolar macrophages containing 3 or more nuclei, were observed in all groups investigated except in the cases of hypoxia due to covering the head with plastic bags. Appare…

Giant cell arteritis in the ocular ischemic syndrome.

Oculomotor nerve paresis, ocular hypotony, anterior segment ischemia, and the ocular ischemic syndrome are uncommon manifestations of giant cell arteritis. Four patients with these findings had giant cell arteritis documented by temporal artery biopsy. Cerebral angiography or ultrasonography, or both, performed in three patients, excluded hemodynamically significant stenosis of the internal carotid artery as the cause of ocular ischemia and cerebral aneurysms as the cause of oculomotor nerve paresis. Corticosteroid treatment, administered to three patients, resulted in resolution of the oculom

Clinical and laboratory findings with giant cell arteritis.

Out of 66 patients who were diagnosed as suffering from polymyalgia rheumatica (PMR; n = 40), temporal arteritis (AT; n = 14) or both (n = 12) in a 6.5 year period (incidence 3.4/100,000 per year), 9 died and 49 were followed up for an average period of 28 months. Exacerbations of the illness (n = 24) and complications in the course (n = 32) were more frequent with an initial ESR greater than 90 mm/h. Postural vertigo (n = 11), amaurosis fugax (n = 11) and polyneuropathy (n = 8) were the most frequent neurological complications. Persisting unilateral blindness and aromatic anosmia developed in

Relation of giant cell arteritis to polymyalgia rheumatica.

Polymyalgia rheumatica and temporal arteritis appear to be separate syndromes rather than two manifestations of an underlying giant cell arteritis. Polymyalgia rheumatica is a synovitis that may be persistent or recurrent, while temporal arteritis is almost always a single episode; documented recurrences are rare. The two syndromes frequently occur in the same patient although not necessarily at the same time and they may be separated by a long interval. In some patients with polymyalgia rheumatica, giant cell arteritis is found on biopsy of an asymptomatic temporal artery. The frequency of th

Prognosis of polymyalgia rheumatica and giant cell arteritis.

Polymyalgia rheumatica and giant cell arteritis are amongst the most satisfying conditions for clinicians to diagnose and treat because the unpleasant effects and serious consequences of these conditions can be almost entirely prevented by corticosteroid treatment; the fact that the side-effects of this treatment sometimes seem to be more serious than the complications of the disease is an indication of its effectiveness. Unfortunately, there is no objective way of determining the prognosis in the individual, and decisions concerning duration of treatment remain empirical.

Anticardiolipin and antineutrophil antibodies in giant cell arteritis.

IgG anticardiolipin antibodies (aCL) were found in a patient with Takayasu's arteritis and then in 11 of 22 patients with temporal arteritis (TA) studied prospectively. In comparison IgG aCL were found in only one of 18 elderly controls with rheumatoid arthritis and one of 47 healthy elderly controls. In TA IgG aCL were more frequent in those with acute disease (8/12), in those with an accompanying history of polymyalgia rheumatica (9/13), p less than 0.05); and levels normalized with corticosteroid treatment. Antineutrophil cytoplasmic antibodies (c-ANCA) were present in one patient with TA w
