We report a case of bilateral acute angle-closure glaucoma in a patient with undiagnosed granulomatosis with polyangiitis (Wegener's). A 59-year-old man presented with a severe headache, ocular pain, blurred vision, shortness of breath, and mild fever. Clinical examination revealed conjunctival chemosis, corneal edema, and shallow anterior chambers. Closed angles were observed bilaterally on gonioscopy. The patient was treated with intravenous mannitol, oral acetazolamide, and anti-glaucomatous eye drops. Over the following two days, his vision improved and intraocular pressures decreased. Sub

In author's opinion development of posterior vitreous detachment (PVD) is an involutional factor which participates in pathogenesis of primary angle closure glaucoma (PACG) in predisposed eyes. Initial increase of intraocular pressure in the eyes with narrow anterior chamber angle and predisposition to PACG is closely connected with the moment of PVD development, which can be detected with ultrasonography. The author supposes that ultrasonic examination of the posterior globe to detect PVD can be used as one of monitoring method in patients predisposed to PACG.

AIM: To estimate the prevalence of primary angle closure glaucoma (PACG) in European derived populations.
METHOD: Systematic review and modelling of PACG prevalence data from population studies. PACG was defined according to the ISGEO definition requiring structural and/or functional evidence of glaucomatous optic neuropathy. Prevalence estimates were applied to the 2010 United Nations projected population figures to estimate case numbers.
RESULTS: The prevalence of PACG in those 40 years or more is 0.4% (95% CI 0.3% to 0.5%). Age-specific prevalence values are 0.02% (CI 0.00 to 0.08) for th

PURPOSE: To identify the prognostic factors for successful laser iridotomy for acute angle-closure glaucoma (AACG).
METHODS: We retrospectively reviewed the medical records of 77 eyes of 77 patients with AACG with initial intraocular pressure (IOP) above 40 mmHg. All of the patients received maximum tolerable medical therapy (MTMT) followed by laser iridotomy. In order to comparatively analyze the factors affecting successful laser iridotomy, an increase in IOP on follow-up was defined as increase in IOP greater than 21 mmHg requiring medical or surgical treatment.
RESULTS: Successful laser

INTRODUCTION: Plateau iris predisposes to the onset of an angle-closure glaucoma attack without pupillary block. This is a morphological anomaly of the iris characterized by anterior insertion, root thickness greater than the norm, and anterior rotation of the ciliary processes into the posterior chamber, pushing the base of the iris into the angle. Argon laser iridoplasty is useful to retract the peripheral iris using the thermal effect of the laser to widen the iridocorneal angle.
MATERIALS AND METHODS: The study investigated nine eyes of five patients who had had iridoplasty for iris plate

UNLABELLED: Primary angle closure glaucoma remains a disease with high visual morbidity worldwide. Patients with primary angle closure glaucoma may have dramatic signs and symptoms that can lead to permanent blindness in a short period of time if not properly treated. Conventionally, medical treatment was used to lower the intraocular pressure (IOP), followed by laser peripheral iridotomy. It acts by elevating pupillary block present in the condition. Laser peripheral iridoplasty is a tehnique using low energy contraction burns to mechanically pull open the angle, thereby reducing the IOP.

Two independent, prospective trials were recently conducted to assess the efficacy of latanoprost in reducing intraocular pressure (IOP) in patients with primary angle-closure glaucoma (PACG). The first study was a 2-week, randomized, double-masked comparison of latanoprost treatment and timolol treatment in patients with PACG. Patients were randomized to one of two parallel treatment groups, receiving either placebo in the morning and latanoprost 0.005% in the evening, or timolol 0.5% twice daily. The mean IOP reduction in latanoprost group was 8.8 +/- 1.1 mm Hg (mean +/- SEM, p < 0.001; 34.2

Biometric measurements were made by ultrasonography in 44 left eyes of 44 patients with primary angle-closure glaucoma. We defined the ratio between the lens thickness and the axial length (lens thickness/axial length factor) as a representative and unifying unit for biometric assessment of the eye. This factor defined the relationship between the lens, iris, and cornea and thus the status of the angle. Lens thickness/axial length factor values were found to be age dependent and were greater than normal for most age groups with angle-closure glaucoma. The mean normal value was 1.91 +/- 0.44; t

PURPOSE: Despite the different etiology of primary open angle glaucoma (POAG), primary angle closure glaucoma (PACG), and pseudoexfoliative glaucoma (PEXG), several studies have suggested that these forms of glaucoma have overlapping genetic risk factors. Therefore, the aim of this study was to evaluate the role of genetic variants recently associated with POAG in different types of glaucoma in Pakistani POAG, PACG, and PEXG patient cohorts.
METHODS: Six variants in CDKN2B-AS1 (rs4977756), CDKN2B (rs1063192), ATOH7 (rs1900004), CAV1 (rs4236601), TMCO1 (rs4656461), and SIX1 (rs10483727) were g

Objective: It is plausible that common disease mechanisms exist in glaucoma pathophysiology. Accordingly, we investigated the genetic association of two previously reported primary open-angle glaucoma (POAG)-related gene polymorphisms, rs2472493 (A > G) in ABCA1 and rs7636836 (C > T) in FNDC3B, in primary angle-closure glaucoma (PACG) and pseudoexfoliation glaucoma (PXG). Methods: TaqMan genotyping was performed in a total of 442 subjects consisting of 246 healthy controls, 102 PACG patients, and 94 PXG patients. Statistical evaluations were performed to detect allelic and genotype association

Ultrasound cycloplasty (UCP) is a minimally invasive, non-incisional glaucoma procedure that has emerged in recent years and is generally associated with few complications. We report a case of a patient with acute angle-closure glaucoma underwent UCP for IOP control. She developed iris neovascularization and neurotrophic keratopathy(NK)at postoperative week 6, followed by hypopyon, flattening of the anterior chamber, and thinning of the peripheral cornea at postoperative week 11, 12, and 14, respectively. We concluded that UCP can lead to anterior segment ischemia (ASI) and NK, which manifest

Charles Bonnet syndrome (CBS) presents with complex visual hallucinations in visually impaired individuals with preserved insight. While most commonly associated with macular degeneration, it may occur in glaucoma. CBS is often under-reported due to stigma and may progress to psychiatric morbidity. A 63-year-old man with bilateral visual loss following acute angle-closure glaucoma experienced vivid hallucinations of animals and snakes. Initially concealed due to embarrassment, the symptoms were revealed during psychiatric assessment. Ophthalmic evaluation confirmed glaucomatous changes; MRI br

BACKGROUND: Primary angle-closure glaucoma (PACG) carries a high risk of irreversible blindness. This case highlights the challenges of managing refractory PACG in a patient with a history of prematurity and the subsequent development of postoperative complications.
CASE PRESENTATION: A 45-year-old woman with a history of prematurity and monocular vision (no light perception in left eye) presented with acute angle closure in the right eye (RE). Despite maximal medical therapy, the RE intraocular pressure (IOP) remained at 54 mmHg. Less invasive options like laser iridotomy or standalone lens
