Single-session use of mineral trioxide aggregate as an apical barrier in a case of external root resorption.

External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system (bacterial re-infection), and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 34-year-old male patient presented with an intra-radicular retainer and an inadequate filling on tooth #21, as well as a radiographic image suggesting periapical bone rarefaction. After ro

The unpredictability of seal after post space preparation: a fluid transport study.

A root canal filling remaining after post space preparation is commonly expected to provide adequate seal. Coronal leakage of 30 endodontically treated teeth was measured before post space preparation using a fluid transport assay. In 10 of these teeth post space was prepared, using a two-step procedure, first to a remaining filling of 6 mm and then to 3 mm, with the leakage studied after each step. In 10 teeth the removal was done in one step to a remaining length of 3 mm. The other 10 teeth, with intact root canal fillings, served as controls and were tested twice for leakage. A significant

The effect of silane on the bond strengths of fiber posts.

OBJECTIVES: Esthetic posts have been developed to maximize the foundation of esthetic restorations. The purpose of this study was to evaluate the effect of silane on the bond strength of three fiber-reinforced resin posts (fiber posts). METHODS: Fifty-four extracted human maxillary central incisors and canines were endodontically treated. D.T. Light Post (DT, Bisco), FRC Postec (FR, Ivoclar Vivadent), and ParaPost Fiber White (PP, Coltène/Whaledent) were inserted using the resin adhesive system provided by the respective manufacturer. For half of the specimens in each group, the fiber posts w

Management of perforating internal root resorption with periodontal surgery and mineral trioxide aggregate: a case report with 5-year follow-up.

Internal root resorption (IRR) is characterized by progressive loss of tooth substance initiating at the root canal wall as a result of clastic activity. The use of periodontal surgery and mineral trioxide aggregate is a good approach to repair lesions with periodontal communication (perforating IRR). This case describes the treatment and follow-up of a maxillary central incisor with perforating IRR in a 56-year-old male patient where root canal treatment, periodontal surgery, and white mineral trioxide aggregate were employed to achieve complete repair and to restore function. Clinical findin

Comparison between prescription of regular or on-demand ibuprofen on postoperative pain after single-visit root canal treatment of teeth with irreversible pulpitis.

INTRODUCTION: Pain management is very important in endodontic practice. The aim of this study was to compare the effect on pain relief of on-demand versus regular prescription of ibuprofen after single-visit root canal treatment in teeth with irreversible pulpitis. METHODS: Sixty mandibular and maxillary molar teeth with irreversible pulpitis without spontaneous pain had single-visit root canal treatment. After this treatment, patients were randomly allocated to 2 groups of 30 patients each. Patients in group 1 received a single dose of 400 mg ibuprofen and a rescue bag of the same medication

One-year results of vital pulp therapy in permanent molars with irreversible pulpitis: an ongoing multicenter, randomized, non-inferiority clinical trial.

OBJECTIVES: Root canal therapy (RCT) and tooth extraction have been conventional treatment options for management of human mature teeth with irreversible pulpitis. Excellent short-term treatment outcomes of vital pulp therapy with calcium-enriched mixture cement (VPT/CEM), as a new treatment option, on postoperative pain relief was demonstrated; if intermediate- and long-term treatment outcomes of the new treatment are also non-inferior compared to RCT, then VPT/CEM may become a viable treatment option for management of mature teeth with irreversible pulpitis. MATERIALS AND METHODS: In 23 hea

A case report of six canals in a maxillary first molar.

Identifying the variations of root canal morphology is crucial prior to commencing any endodontic treatment. The advancement of current endodontic instrumentation and technology has greatly enhanced treatment outcomes, which are now more predictable. This clinical case report presents a case of a maxillary first molar showing six root canals and apical foramina, i.e. three mesiobuccal canals, two palatal canals and one distobuccal canal. The occurrence of bifurcation in the second mesiobuccal canal is also emphasised.

Effect of different protective base materials on hydrogen peroxide leakage during intracoronal bleaching in vitro.

External root resorption may develop following intracoronal bleaching with hydrogen peroxide. The preventive effect of different base materials on the radicular penetration of H2O2 during intracoronal bleaching was assessed. Seventy-two bovine teeth and 20 human teeth were bleached with 30% H2O2. The bovine teeth were divided into four groups and the root canals filled with either IRM, zinc oxide-eugenol, composite resin, or glass ionomer. The radicular H2O2 penetration of each group at different layer thickness was compared. The experiment with the human teeth was performed in three stages. I

Reverse sandwich restoration for the management of invasive cervical resorption: a case report.

INTRODUCTION: Invasive cervical resorption (ICR) may occur as a consequence of trauma, orthodontic treatment, intracoronal bleaching, and surgical procedures and may lead to the progressive and destructive loss of tooth structure. The restoration of these resorptive defects is a challenging task because no single restorative material possesses good sealing ability, reinforcement of remaining tooth structure, and biocompatibility simultaneously. METHODS: This case presented with invasive cervical resorption class 2 (Heithersay) and apical inflammatory resorption caused by trauma in tooth #9. T

A clinical trial of pulpotomy vs. root canal therapy of mature molars.

Источник пометил публикацию как отозванную

Root canal therapy (RCT) and tooth extraction are the main treatment options for irreversible pulpitis or its sequelae. Pulpotomy is an alternative treatment; however, more evidence is required. If outcomes of pulpotomy with a calcium-enriched mixture (PCEM) are non-inferior for mature molars with irreversible pulpitis compared with those from one-visit RCT (ORCT), this may be a beneficial treatment option. Four hundred seven individuals met the inclusion criteria and were randomly allocated [PCEM (n = 205), ORCT (n = 202)]. We used NRS questionnaires to record pain intensity. Six-month clinic

The effect of pulpotomy using a calcium-enriched mixture cement versus one-visit root canal therapy on postoperative pain relief in irreversible pulpitis: a randomized clinical trial.

The purpose of this noninferiority trial was to compare postoperative pain relief after one-visit root canal therapy (ORCT) with a pulpotomy performed with a new endodontic calcium-enriched mixture cement (PCEM) in human permanent molars with irreversible pulpitis. A total of 407 selected patients were randomly allocated into the ORCT group (n = 202) or the PCEM group (n = 205). Numerical Rating Scale questionnaires were used to record pain intensity (PI) by the patients during the first 7 days after treatment. While there was no statistically significant difference in the mean PI at baseline

Root Canal Filling after Revascularization/Revitalization.

INTRODUCTION: Revascularization/revitalization therapy is considered an alternative procedure for management of teeth with an immature apex and necrotic pulp, mainly when root development is interrupted in the early phases of formation. However, this clinical treatment protocol should be considered a permanent procedure? METHOD: A maxillary central incisor with a previous and successful RR treatment was intentionally filled with a biocompatible material with the periapical tissues due to the patient's lack of adherence to the follow-up protocol. RESULTS: The 20-month follow-up showed absence

Internal and external root resorption: aetiology, diagnosis and treatment options.

UNLABELLED: Root resorption is a pathological process that may occur after surgical, mechanical, chemical or thermal insult. Generally, it can be classified as internal and external root resorption. Depending on the diagnosis, an orthograde, surgical or a combined approach is used in management of these cases. CLINICAL RELEVANCE: General dental practitioners can face difficulties in diagnosis and treatment planning for cases with root resorption. An understanding of the aetiology and pathogenesis of root resorption is critical for diagnosis, effective management and improves outcome.

The effect of intracanal anti-inflammatory medicaments on external root resorption of replanted dog teeth after extended extra-oral dry time.

The prognosis of replanted teeth depends on several factors, the most important being the length of extra-oral dry time. Studies show that after 60 min dry time, root resorption is predicted. Immediate intracanal placement of Ledermix, a paste containing triamcinolone (corticosteroid) and demeclocycline (tetracycline) has been shown to inhibit root resorption after extended dry time. However, discoloration is possible from the tetracycline in Ledermix. To evaluate the individual influence of corticosteroid and tetracycline on external root resorption after extended extra-oral dry time. Sixty-n

In vitro comparison of the cytotoxicity of twelve endodontic materials using a new technique.

An in vitro method for the cytotoxicity testing of endodontic materials is described which aims to simulate the clinical situation. Materials can be tested in the presence or absence of a compacted layer of dentine chips mimicking the periapical dentine plug. A total of twelve materials were tested. In the absence of dentine, Kloroperka, Biocalex, Diaket and Endomethasone were slightly cytotoxic; AH26 with and without silver, Sealapex, Tubliseal and Kerr's pulp canal sealer were moderately cytotoxic, while Forfenan, Spad and Kri paste were strongly cytotoxic. In the presence of dentine the cyt

Case report: endodontic and surgical treatment of an upper central incisor with external root resorption and radicular cyst following a traumatic tooth avulsion.

In the age group between 6 and 12 years, trauma to the upper incisors happens frequently. In the case of avulsion, a replantation is the state-of-the-art treatment; however, it may lead to several complications, particularly if suitable posttraumatic management is not carried out. External cervical resorptions as well as apical granuloma and cysts due to microbial contamination of the root canal are common complications. In the presented trauma case, a conservative approach was chosen to treat a large cystic lesion combined with cervical and apical resorptions. After initial placement of Leder
