Corneal ulcers associated with extended-wear soft contact lenses.

We treated nine myopic and nine aphakic eyes in patients who developed corneal ulcers while wearing extended-wear soft contact lenses. Bacteria were recovered from the corneal ulcers of 13 of 18 patients with Pseudomonas aeruginosa being the pathogen most frequently isolated. Chemical disinfection was used by seven of the nine patients with myopia and eight of the nine with aphakia. Two of nine patients in the myopic group had visual loss to 20/200 and hand movements; five of nine patients in the aphakic group had visual loss to 20/50 or worse. Invasion of the corneal stroma by bacteria may oc

Assessment of risk factors and preventive measures for inorganic and organic GSR secondary transfer in arrest scenarios.

This study monitored the transfer of inorganic and organic gunshot residues (IGSR and OGSR) in cases where indirect exposure might be questioned, such as during arrests. Mock arrest scenarios (n = 180) were created to assess the effect of two variables on the secondary transfer. First, the level of contact between the detained and the officer (low, medium, and high) during the arrest protocols. Second, the activities preceding the arrest include situations where the officer or person of interest (POI) discharges a firearm or handles a gun without firing. Additionally, measures to mitigate the

Formative Research for Adapting the Cholera-Hospital-Based-Intervention-for-7-Days (CHoBI7) Water Treatment and Hygiene Mobile Health Program for Scalable Delivery in Rural Bangladesh.

The Cholera-Hospital-based-Intervention-for-7-Days (CHoBI7) mobile health (mHealth) program is a targeted water treatment and hygiene (WASH) program for the household members of diarrhea patients, initiated in the healthcare facility with a single in-person visit and reinforced through weekly voice and text messages for 3 months. A recent randomized controlled trial of the CHoBI7 mHealth program in urban Dhaka, Bangladesh, found that this intervention significantly increased WASH behaviors and reduced diarrhea prevalence. The objective of this present study was to conduct formative research us

Effects of ultra-pure soft water on the hands of nurses in a neonatal intensive care unit: A randomized crossover study.

BACKGROUND: Healthcare workers (HCWs) wash their hands with tap water (TW) and soap. However, hard TW causes dermatitis. OBJECTIVES: The present study aimed to compare the effects of ultra-pure soft water (UPSW) with those of TW on the hands of HWCs. METHODS: The present study was a prospective randomized trial with a crossover design. All the nurses in the neonatal intensive care unit (NICU) at the study centre were divided into Sequence 1 (UPSW to TW) or 2 (TW to UPSW) and washed their hands with TW or UPSW in alternating 4-week periods with a 4-week washout period. Evaluation of facial cleanliness and environmental improvement activities: Lessons learned from Malawi, Tanzania, and Uganda.

The World Health Organization promotes the SAFE (Surgery, Antibiotics, Facial cleanliness, and Environmental improvements) strategy for trachoma control and prevention. The F&E components of the strategy focus on promotion of healthy hygiene and sanitation behaviors. In order to monitor F&E activities implemented across villages and schools in Malawi, Tanzania, and Uganda, an F&E Monitoring and Evaluation (FEME) framework was developed to track quarterly program outputs and to provide the basis for a pre and post evaluation of the activities. Results showed an increase in knowledge at the scho

Achieving optimal technology and behavioral uptake of single and combined interventions of water, sanitation hygiene and nutrition, in an efficacy trial (WASH benefits) in rural Bangladesh.

BACKGROUND: Uptake matters for evaluating the health impact of water, sanitation and hygiene (WASH) interventions. Many large-scale WASH interventions have been plagued by low uptake. For the WASH Benefits Bangladesh efficacy trial, high uptake was a prerequisite. We assessed the degree of technology and behavioral uptake among participants in the trial, as part of a three-paper series on WASH Benefits Intervention Delivery and Performance. METHODS: This study is a cluster randomized trial comprised of geographically matched clusters among four districts in rural Bangladesh. We randomly alloc

A Systematic Review and Meta-Analysis of the Association between Water, Sanitation, and Hygiene Exposures and Cholera in Case-Control Studies.

Case-control studies are conducted to identify cholera transmission routes. Water, sanitation, and hygiene (WASH) exposures can facilitate cholera transmission (risk factors) or interrupt transmission (protective factors). To our knowledge, the association between WASH exposures and cholera from case-control studies has not been systematically analyzed. A systematic review was completed to close this gap, including describing the theory of risk and protection, developing inclusion criteria, searching and selecting studies, assessing quality of evidence, and summarizing associations between cho

Water, sanitation, hygiene, and soil-transmitted helminth infection: a systematic review and meta-analysis.

BACKGROUND: Preventive chemotherapy represents a powerful but short-term control strategy for soil-transmitted helminthiasis. Since humans are often re-infected rapidly, long-term solutions require improvements in water, sanitation, and hygiene (WASH). The purpose of this study was to quantitatively summarize the relationship between WASH access or practices and soil-transmitted helminth (STH) infection. METHODS AND FINDINGS: We conducted a systematic review and meta-analysis to examine the associations of improved WASH on infection with STH (Ascaris lumbricoides, Trichuris trichiura, hookwor

[Novel influenza H1N1 pandemic: lesson learned].

The efforts made by the government of Japan and its people to prevent and control the spread of the disease and to limit its health impact had three major characteristics: (1) Suspension of schools in wide geographical areas particularly at the early stage of the outbreak. (2) High proportion of the infected persons who were given antiviral drugs. (3) High level of public awareness and personal hygiene such as regular hand washing. Although more research needs to be done, it seems fair to say that the three characteristics mentioned above contributed to Japan's globally one of the lowest morta

Universal precautions--a critical review.

The route of HIV transmission are now well defined. For health care workers the major occupational risk is from parenteral exposure to infected blood or other body fluids. To prevent such exposures, it would be prudent for HCWs to assume that all patients are potentially infected and a set of precautions applicable universally be followed in contacts with all patients. The provisions of "Universal Precautions" apply to blood, CSF, genital secretions and all body fluids. It is essential that barrier protection and washing of hands be practiced, body fluids be handled with care, correct steriliz

Social and behavioral factors in transmission and response to shigellosis.

Biomedical approaches to shigellosis tend to emphasize unique etiologic and clinical features of the disease, e.g., those related to the development of vaccines or case management. The biologic characteristics of the organism and sociocultural responses to it imply that it is unlikely that a purely technologic solution (vaccine) to shigellosis will be found soon. Infrastructural development (protected water supply and sanitary disposal of feces) may be too expensive to be feasible due to the economic, environmental, and demographic conditions in developing countries. Behavioral change to reduc
