Drug-Related Deaths Among Young People in a Scottish Region: A Socio-Ecological Autopsy Approach to Understanding the Context of Drug Deaths.

AIMS: Social, structural and systemic factors are critical to understanding drug-related deaths among adults. The relevance of these factors to young people is not known. This study explores the life experience, drug using histories and the interactions of a group of young people with agencies and services prior to their death. Our approach seeks to look beyond the immediate cause of death and identify broader contextual factors that may have contributed to a death through a "whole-life view". METHODS: The study developed a socio-ecological autopsy approach informed by social autopsy methods

Limbs lost, lives changed: The unprecedented limb amputation crisis in Gaza.

Historically in armed conflicts, traumatic amputations result in long-term impacts on individuals, healthcare systems, and society. Though amputations are not new, the ongoing conflict in Gaza has resulted in an unusually high number of amputations. The scale of conflict-related amputations in Gaza, accompanied by systemic barriers to acute and rehabilitative care, necessitates feasible rehabilitation-focused responses relevant to physiatrists and global health stakeholders. The World Health Organization documentation reflects between 3105 and 4050 amputations between January 10 and May 16, 20

Antimicrobial Resistance in Somalia: A Growing Public Health Crisis in a Fragile Health System-A Narrative Review of Challenges, Healthcare Gaps, and Policy Recommendations.

BACKGROUND: Antimicrobial resistance (AMR) has emerged as one of the most serious global public health threats and represents a growing concern in fragile and conflict-affected countries such as Somalia. The increasing emergence of resistant bacterial pathogens threatens the effectiveness of commonly used antimicrobial therapies and contributes to increased morbidity, mortality, healthcare costs, and prolonged hospital stays. In Somalia, prolonged conflict, weak healthcare infrastructure, population displacement, limited diagnostic capacity, and widespread inappropriate antibiotic use have acc

The consequences of maltreatment on children's lives: a systematic review of data from the East Asia and Pacific Region.

This study explores the consequences of child maltreatment in East Asia and the Pacific region based on the results of a systematic review of 16 English and non-English databases for journal articles and "gray" literature published between January 2001 and November 2010. This review shows that children in the region experiencing maltreatment are at increased risk of experiencing mental health consequences, physical health sequelae, high-risk sexual behaviors, and increased exposure to future violence including intimate partner violence (IPV) as an adult. Children who suffer from child sexual a

[Heat waves: health impacts].

During the summer of 2003, record high temperatures were reported across Europe, causing thousands of casualties. Heat waves are sporadic recurrent events, characterised by intense and prolonged heat, associated with excess mortality and morbidity. The most frequent cause of death directly attributable to heat is heat stroke but heat waves are known to cause increases in all-cause mortality, specially circulatory and respiratory mortality. Epidemiological studies demonstrate excess casualties cluster in specific risk groups. The elderly, those with chronic medical conditions and the socially i

Domestic Violence and Education: Examining the Impact of Domestic Violence on Young Children, Children, and Young People and the Potential Role of Schools.

This article examines how domestic violence impacts the lives and education of young children, children, and young people and how they can be supported within the education system. Schools are often the service in closest and longest contact with a child living with domestic violence; teachers can play a vital role in helping families access welfare services. In the wake of high profile cases of child abuse and neglect, concerns have been raised about the effectiveness of multi-agency responses to children living with abuse. In the United Kingdom, the case of 4-year-old Daniel Pelka who died i

Partnering for rotavirus vaccine introduction in India: A retrospective analysis.

BACKGROUND: The pre-existing partner network created in India for the delivery of polio vaccines was initially used to eradicate polio and later on embedded in the health systems network to promote routine immunization and other health interventions efficiently. The experience from this network offered lessons for strengthening the health care systems and provided a well-established network that could be utilized for other vaccine initiatives. It has also been established that successful partnerships between a broad range of stakeholders provide support, strengthen the health system, and accel

Not what it seems: Distributed agencies and clinical narratives in electronic health records.

Medical records mediate clinical work at multiple levels. Focusing on the transition from paper-based medical records to Electronic Health Records (EHRs), we analyze how the incorporation of digital infrastructures reconfigures healthcare networks and reshapes practices of writing, reading, and narration. Our analysis draws on ethnographic research conducted between 2024 and 2025 in two psychiatric settings in southwestern Colombia. We trace how the implementation of EHRs redistributed agency within clinical documentation and altered relations among clinicians, administrative staff, software s

Optimizing Ethical Care, Quality, and Safety in Long-Term Services and Supports: A Position Paper From the American College of Physicians.

Long-term services and supports (LTSS) in the United States have faced substantial and enduring challenges. They encompass services for persons who can no longer independently care for themselves because of cognitive decline, functional limitations, chronic illness, or the sequelae of such conditions. These services are delivered in institutional and noninstitutional settings, such as nursing homes, assisted living facilities, and home- and community-based programs. This position paper by the American College of Physicians examines the ethical implications of current LTSS business models and p

A Review of Guidelines for the Primary Prevention of Legionellosis in Long-Term Care Facilities.

Legionellosis is an important public health problem in the United States and other countries, and residents of long-term care facilities (LTCFs) are at higher risk for Legionnaires' disease than the general population. In this study, we reviewed published US and international guidelines for the primary prevention of legionellosis in LTCFs, including nursing homes, skilled nursing facilities, assisted living facilities, and aged care facilities. The results of this review indicate that most guidelines emphasize adequate design and maintenance of water systems and water temperatures; however, gu

'I Did Not Choose This': teenage girls and the reproductive pipeline in rural Zimbabwe.

This article rethinks teenage pregnancy in rural Zimbabwe through a decolonial Afro-feminist lens, foregrounding the lived narratives of 26 teenage mothers in Shamva. Rather than pathologising their experiences or framing early pregnancy as a failure of knowledge or morality, we conceptualise the notion of a reproductive pipeline-a system through which poverty, gendered power, inadequate healthcare, and emphasised femininity converge to constrain girls' reproductive autonomy. The analysis reveals how early pregnancy is produced through a reproductive pipeline where poverty, patriarchy, kinship

Development and implementation of a distributed data network between an academic institution and state health departments to investigate variation in time to HIV viral suppression in the Deep South.

BACKGROUND: Achieving early and sustained viral suppression (VS) following diagnosis of HIV infection is critical to improving outcomes for persons with HIV (PWH). The Deep South of the United States (US) is a region that is disproportionately impacted by the domestic HIV epidemic. Time to VS, defined as time from diagnosis to initial VS, is substantially longer in the South than other regions of the US. We describe the development and implementation of a distributed data network between an academic institution and state health departments to investigate variation in time to VS in the Deep Sou

Using normalisation process theory (NPT) to explore implementation of the maternal perinatal death surveillance and response (MPDSR) policy in Uganda: a reflection.

BACKGROUND: The implementation of the maternal perinatal death surveillance and response (MPDSR) policy is among the envisaged strategies to reduce the high global burden of maternal and perinatal mortality and morbidity. However, implementation of this policy across various contexts is inconsistent. Theoretically informed approaches to process evaluation can support assessment the implementation of policy interventions such as MPDSR, particularly in understanding what the actors involved actually do. In this article, we reflect on how the normalisation process theory (NPT) was used to explore
