Application of Artificial Intelligence-Based Auxiliary Diagnosis in Congenital Heart Disease Screening.

BACKGROUND: To evaluate the application value of artificial intelligence-based auxiliary diagnosis for congenital heart disease. METHODS: From May 2017 to December 2019, 1892 cases of congenital heart disease heart sounds were collected for learning- and memory-assisted diagnosis. The diagnosis rate and classification recognition were verified in 326 congenital heart disease cases. Auscultation and artificial intelligence-assisted diagnosis were used in 518 258 congenital heart disease screenings, and the detection accuracies of congenital heart disease and pulmonary hypertension were compare

Prevalence of congenital cardiac anomalies at high altitude.

The effect of high altitude on the prevalence of congenital heart disease was tested by examining 1,116 school children at four study sites in the People's Republic of China. Sites ranged in altitude from sea level to 4,500 m above sea level. Children were screened by physical examination, and an echocardiogram and electrocardiogram were performed on each child suspected of having a cardiac anomaly. A high prevalence of patent ductus arteriosus and atrial septal defect was found at the three high altitude sites and the effect of altitude was progressive. Both anomalies were postulated to be th

[Detection of right-to-left shunts by one- and twodimensional contrast echocardiography (author's transl)].

33 children with different congenital cardiac malformations--aged one day to twelve and a half years--were investigated by peripheral and central, 1- and 2 dimensional contrast echocardiography. 11 patients were studied during routinely performed cardiac catheterization and 23 in the early postoperative period (one patient pre- and postoperatively.) Contrast echocardiography peripherally performed represents a very sensitive diagnostic method to detect right-to-left shunting at atrial and/or ventricular levels. Quantification of shunts seems to be possible, if standardization can be achieved.

Tough, but not impossible - Retrieval of large atrial septal occluder devices embolized to left atrium.

Atrial septal defect (ASD) is a congenital heart defect that is being increasingly treated using percutaneous interventions. However, these techniques are not devoid of complications. One such complication is device embolization. Removal of such closure device poses tremendous risk and consequent complications both by percutaneous retrieval and surgical removal. Herein, we present two cases of ASDs that were closed using atrial septal occluder, but the devices were subsequently embolized into left atrium. These devices were then percutaneously retrieved without any further complication or inju

Ductus arteriosus aneurysm in infancy.

An infant with symptoms of congestive heart failure at 6 months of age was found by cardiac catheterization at the age of 1 year to have a fusiform aneurysm of the ductus arteriosus as well as discrete stenosis of the left pulmonary artery and a secundum atrial septal defect. The aneurysm was resected, and a pericardial patch angioplasty corrected the peripheral pulmonary stenosis. The atrial septal defect was closed in a separate procedure. To our knowledge, this case is one of four ductal aneurysms reported in infants that have been diagnosed before operation and successfully treated. The pa

Transcatheter closure of secundum atrial septal defects.

Atrial septal defect (ASD) is one of the most common congenital heart defects, accounting for 7%-10% of all congenital heart disease (CHD) in children and 30%-33% of defects diagnosed in adults with CHD. This review highlights the evolution of transcatheter ASD closure, indications, follow-up, outcomes, and complications with a focus on the erosion issue with certain devices.

Platypnea-Orthodeoxia Syndrome Due to Sinus Venosus Atrial Septal Defect With Partial Anomalous Pulmonary Venous Return.

BACKGROUND: Platypnea-orthodeoxia syndrome is a rare cause of positional hypoxia resulting from right-to-left shunting, often associated with intracardiac defects and abnormal venous return. CASE SUMMARY: A 68-year-old woman with progressive hypoxia presented with positional desaturation and was found to have a superior sinus venosus atrial septal defect with partial anomalous pulmonary venous return. After diagnosis via bubble echocardiography and cardiac magnetic resonance imaging, successful transcatheter closure was performed with resolution of hypoxia. DISCUSSION: This case highlights t

Congenital heart defects in patients with isolated microtia: evaluation using colour Doppler echocardiographic image.

BACKGROUND: The objective of this study was to delineate the characteristics and incidence of congenital heart disease (CHD) in patients with isolated microtia and to determine whether the prevalence of CHD among patients with isolated microtia increases with the severity of microtia. METHODS: A total of 804 consecutive patients had a pre-operative colour Doppler echocardiographic examination. A retrospective study was performed with the clinical and imaging data from November, 2017 to January, 2019. The χ2 test was performed to analyse the interaction between isolated microtia and CHD. Sox9 Expression in the Second Heart Field; A Morphological Assessment of the Importance to Cardiac Development with Emphasis on Atrioventricular Septation.

Failure to form the septal structures that separate the left and right cardiac chambers results in defects that allow shunting of blood from one side of the heart to the other, leading to the mixing of oxygenated and de-oxygenated blood. The atrioventricular (AV) mesenchymal complex, consisting of the AV cushions, the Dorsal Mesenchymal Protrusion (DMP), and the mesenchymal cap, plays a crucial role in AV septation. Cells found in these structures derive from different cell lineages. In this study we have investigated the role of the transcription factor Sox9 in the Second Heart Field (SHF) wi

A Long-Term Complication Occurred After Transcatheter Closure of Large Atrial Septal Defect.

Transcatheter closure of ostium secondum atrial septal defect has become an alternative method to surgical closure. However, the incidence of complications and long-term results of using large size (> 40 mm) Amplatzer septal occluders are unknown. This case reported a 59 years old woman, whom received transcatheter closure of atrial septal defect (36 mm) with a 40 mm Amplatzer septal occluder 10 years ago and was diagnosed with heart failure. Transthroacic echocardiography showed severe mitral valve regurgitation. Intra-operatively, we confirmed and removed the large device, but we found that

The effect of propranolol upon chick embryo cardiogenesis.

The teratogenic effects of propranolol HCl on cardiac development were studied in chick embryos of days 3 and 4 of incubation. Propranolol was injected into the yolk sac at doses ranging from 0.05 to 0.6 mg per egg. All the treated and control embryos were examined on day 7. The LD50 for the embryos treated on the 3rd and 4th day was 0.15 and 0.35 mg per embryo, respectively. Cardiac anomalies such as aortic stenosis ventricular septal defects and common truncus arteriosus were observed. Other malformations included atrial septal defects, thin atrial wall and defects of the pulmonic, aortic an

Navigating the clinical complexity of acute heart failure in an untreated adult with dextrotransposition of the great arteries (D-TGA) and multiple cardiac anomalies: a case report and review of literature.

BACKGROUND: Transposition of the great arteries is a critical congenital heart defect characterized by the reversal of the aorta and pulmonary arteries, resulting in two parallel circulatory pathways. This condition is typically incompatible with life unless a shunt, such as an atrial or ventricular septal defect, facilitates the adequate mixing of oxygenated and deoxygenated blood in the heart. Survival into adulthood without surgical correction is exceptionally rare. While a limited number of cases have been reported globally, none, to the best of our extensive search, have been associated w

Positional cyanosis reveals platypnea-orthodeoxia-syndrome.

Platypnea-orthodeoxia syndrome is a rare cause of dyspnea, which presents upon standing and resolves when supine. Etiology is multifactorial with a functional component and an anatomical shunt. The most commonly reported shunt is an atrial septal defect, however a shunt can occur in advanced hepatic or pulmonary disease. Treatment is dependent on the type and location of the shunt. In atrial septal defects percutaneous repair is preferred. To follow is a case of iatrogenic platypnea-orthodeoxia syndrome secondary to heart failure guideline directed medical therapy in a 79 year-old woman.

[A case of two-chambered right ventricle complicating severe cyanosis due to tricuspid regurgitation and patent foramen ovale].

A 63-year-old female of two-chambered right ventricle (TCRV) associated with tricuspid regurgitation (TR), patent foramen ovale (PFO) and small ventricular septal defect (VSD) underwent corrective surgery successfully. She suffered severe heart failure and cyanosis with 47.7% of right-to-left shunt through PFO. The VSD was so small that no significant shunt was shown in catheterization data. Operative findings suggested that TR was caused by elongation of the chorda as a consequence of long-term pressure load of right ventricular inflow chamber. Among reported cases of TCRV, the present case i

[The P loop in atrioventricular septal defect. Embryologico-vectorcardiographic correlation].

Atrial vectorcardiograms (vcg) were analysed in 23 patients with partial (n = 15) or complete (n = 8) forms of atrioventricular septal defect (AVSD). The rotation and the projection of the maximum left atrial vector (MLAV) on the frontal plane (FP) and on the horizontal plane (HP) were noted. To assess the possible influence of the degree of left-to-right shunting and the right atrial and right ventricular pressures on the rotation and MLAV projection, these vcg data were correlated with hemodynamic values obtained at the time of preoperative cardiac catheterization. Vcg characteristics of the

Atrial septal defect closure.

Congenital heart disease accounted for 0.3% of US hospital admissions in 2007, with 48% related to atrial septal defects (ASDs). More than one-fourth of adult congenital heart defects are ASDs, 75% of which are ostium secundum ASDs. The progressive impact of volume overload of the right cardiac chambers can be halted by ASD closure. This review focuses on percutaneous ASD closure.

Hybrid treatment of a dislocated atrial septal occluder device at the bifurcation of the left and right common iliac artery.

Percutaneous closure of secundum atrial septal defects (ASDs) with atrial septal occluders (ASOs) avoids sternotomy and cardiopulmonary bypass, and thus is commonly preferred to open heart surgery. However, rare reports of dislocation of the ASO into the systemic circulation do exist. We report the use of an emergent hybrid procedure in the dislocation of an ASO at the bifurcation of the left and right common iliac artery. The dislocation was diagnosed 7 days after percutaneous closure of an ASD.

Anatomical-embryological correlates in atrioventricular septal defect.

Recent embryological studies have supported the consideration that the ventricular septum is multifocal in origin. These data have also provided excellent correlation of the morphology of malformed hearts with their embryology. In particular, atrioventricular septal defect correlates accurately with these observations on ventricular septation. Many of the names given to atrioventricular septal defect (for example ostium primum, persistent atrioventricular canal, endocardial cushion defect) indicate attempts at correlating the anatomy with embryology. None of these has been very convincing. In
