Helicobacter pylori: beyond peptic ulcer disease.

Beyond peptic ulcer disease, Helicobacter pylori infection is associated with intestinal-type gastric cancer and low-grade gastric mucosa-associated lymphoid tissue (MALT) lymphoma. It is also currently implicated as a possible cause of dyspepsia and extraintestinal disorders such as coronary artery disease, rosacea, chronic urticaria, and delayed growth in children. There are strong epidemiological data from large cohort studies linking H. pylori to gastric adenocarcinoma. Several cofactors, including early childhood acquisition of infection, strain-specific differences, genetic predispositio…

Helicobacter pylori duodenal colonization in children.

To investigate the prevalence and the significance of Helicobacter pylori duodenal colonization, endoscopic duodenal biopsies were performed in 168 children with chronic abdominal pain, gastroesophageal reflux, gastrointestinal bleeding, and malabsorption syndrome. Helicobacter pylori infection was detected in 68 children (40.4%): in 31 of them H. pylori was present in the gastric antrum, and in 37 in the duodenum also. Duodenitis was observed in 25 children with duodenal H. pylori; gastric metaplasia in 3. Scanning electron microscopy revealed the presence of the micro-organism in 3/13 cases;…

Treatment strategies for Helicobacter pylori infection.

Peptic ulcer disease is strongly associated with infection by Helicobacter pylori, a spiral-shaped, flagellated organism found predominantly in the gastric antrum. More than 90 percent of duodenal ulcers and adenocarcinomas of the distal stomach are associated with H. pylori infection. Eradication of the organism effectively prevents relapses of gastroduodenal ulcers associated with H. pylori. In patients undergoing endoscopy, the rapid urease test is highly sensitive and specific in diagnosing H. pylori infection. Noninvasive diagnostic methods include serologic antibody measurements and urea…

Helicobacter pylori and peptic ulcer disease.

Helicobacter pylori is associated with diffuse superficial gastritis and is one of the dominant causal factors for duodenal ulcer disease. More recent knowledge has revealed that apart from NSAID-related ulcers, H. pylori is also highly associated with gastric ulcer disease. Follow-up of duodenal ulcer patients has revealed that the prevalence of duodenal ulcer relapse was sharply diminished after successful cure of H. pylori infection. The few data available concerning relapse rates of gastric ulcer disease indicate a striking reduction of recurrence of gastric ulceration after successful era…

Helicobacter pylori infection in congestive gastropathy.

BACKGROUND: This study determines the prevalence and significance of Helicobacter pylori infection in portal hypertensive patients. MATERIALS AND METHODS: Patients numbered 118 and consisted of 90 patients with portal hypertension (66 men; 24 women; mean age, 49.1 +/- 2.1 years) and 28 noncirrhotic patients with nonulcer dyspepsia, (12 men; 16 women; mean age, 47.6 +/- 2.8 years), who made up the control group. In all patients, diagnostic upper endoscopy was performed, and gastric biopsies were taken for histological examination and diagnosis of H. pylori. RESULTS: Of the portal hypertensive…

Review of Helicobacter pylori eradication regimens.

UNLABELLED: A large number of different treatment regimens aimed at eradicating Helicobacter pylori have been evaluated and reported. A meta-analysis of the pooled results is reviewed here. The treatment strategies have been grouped into 13 treatment classes. The two dual combination therapies reviewed, omeprazole with amoxycillin or clarithromycin, have been analysed with regard to dose and dosing. In all other groups, the total dose of the drugs, number of doses, duration of therapy, and formulation are not taken into account. RESULTS: A sensitivity test comparing the effect of different st…

Helicobacter pylori and peptic ulcer disease.

Since William Beaumont's classic description of gastric physiology in 1847, the focus of clinical practice and basic research concerning gastric and duodenal ulcers has centered on the secretion of acid and pepsin. In addition to these well-known factors (largely determined by heredity, age, and oral intake), the search for other agents that alter the mucosal barrier has identified a bacillus, Helicobacter pylori, as a significant agent in the development of chronic gastritis and benign gastric and duodenal ulcers. This review explores the scientific evidence for an important causal role of H.…

Gastric carcinoma: the Helicobacter pylori trail.

Gastric carcinoma is the world's overall second most common cancer. Besides obvious environmental factors, recent epidemiological studies and a better knowledge of Helicobacter Pylori biological properties revealed that the microorganism is involved in the first steps of gastric carcinogenesis as proposed by the Correa model (from normal gastric tissue through superficial gastritis, multifocal atrophic gastritis, intestinal metaplasia and dysplasia to carcinoma). Significant correlation between the prevalence of H. pylori infection and incidence of gastric carcinoma (mainly the intestinal type…

Helicobacter pylori infection in gastric carcinoma.

PURPOSE: This study was undertaken to compare the pathoclinical findings in gastric adenocarcinoma with serum IgG antibody to Helicobacter pylori. MATERIALS AND METHODS: We examined 185 patients with histologically established gastric cancer. The presence of immunoglobulin (Ig)G antibody in the high molecular cell-associated antigen of H. pylori was determined by enzyme-linked immunosorbent assay. Pepsinogens I and II were measured by radioimmunoassay. The distribution of H. pylori on the gastric mucosa was assessed by the Campylobacter-like organism test and phenol red dye spraying. RESULTS…

[Helicobacter pylori: pathogens, pathomechanisms and epidemiology].

H. pylori organisms are microaerophilic gram-negative curved or spiral bacteria that live in the mucus layer of the gastric epithelium. Since the discovery of H. pylori similar organisms have been found in humans (H. cinaedi, H. fennelliae, H. heilmanii) and animals. H. pylori causes chronic active gastritis and is an important factor in the development of peptic ulceration and gastric cancer. Narrow host range, tissue specificity and chronic inflammation are characteristic features. Putative virulence factors of H. pylori are structural components (flagella, adhesins...) extracellular bacteri

Helicobacter pylori seroepidemiology in risk groups.

Helicobacter pylori is associated with peptic ulcer and chronic active gastritis. The response to infection can be determined by measuring serum titers of anti-H. pylori antibodies. We compared antibody titers in 612 serum samples from 570 individuals considered at risk for H. pylori infection, 170 of them are control sera from 110 adults and 60 children with no gastric alterations. The study groups were 93 institutionalized mentally handicapped children, 40 heterosexual couples, 101 HIV-sero-positive patients, 86 patients with chronic renal failure and 40 individuals (20 adults and 20 childre…

Helicobacter infection and gastric ethanol metabolism.

The organism frequently colonizing the stomach of patients suffering from chronic active gastritis and peptic ulcer disease--Helicobacter pylori--possesses marked alcohol dehydrogenase (ADH) activity. Consequently, Helicobacter infection may contribute to the capacity of the stomach to metabolize ethanol and lead to increased acetaldehyde production. To study this hypothesis, we first determined ADH activity in a variety of H. pylori strains originally isolated from human gastric mucosal biopsies. ADH activity was also measured in endoscopic gastric mucosal specimens obtained from H. pylori-po…
