Top advances of the year: Leukemia.

In the year 2021, there were three new Food and Drug Administration approvals for all leukemia types: asciminib (Scemblix) for chronic myeloid leukemia, brexucabtagene autoleucel (Tecartus) for relapsed/refractory B-cell acute lymphocytic leukemia, and asparaginase erwinia chrysanthemi (recombinant)-rywn (Rylaze) for acute lymphocytic leukemia. This is down from 2017-2018 when eight new therapies were approved for acute myeloid leukemia alone. However, this decrease from prior years does not imply that little progress was made in our understanding or treatment of leukemias in 2021. Asciminib a

Progressive multifocal leukoencephalopathy associated with chemotherapy induced lymphocytopenia in solid tumors - case report of an underestimated complication.

BACKGROUND: JC virus reactivation causing progressive multifocal leukoencephalopathy (PML) occurs preferentially in human immunodeficiency virus (HIV) positive individuals or patients suffering from hematologic neoplasms due to impaired viral control. Reactivation in patients suffering from solid malignancies is rarely described in published literature. CASE PRESENTATION: Here we describe a case of PML in a male patient suffering from esophageal cancer who underwent neoadjuvant radiochemotherapy and surgical resection in curative intent resulting in complete tumor remission. The radiochemothe…

The Role of Tumor-Associated Macrophages in Hematologic Malignancies.

The tumor microenvironment includes dendritic cells, T-cytotoxic, T-helper, reactive B-lymphoid cells and macrophages; these reactive cells could interplay with malignant cells and promote tumor growth and survival. Among its cellular components, tumor-associated macrophages (TAM) represent a component of the innate immune system and play an important role, especially in hematologic malignancies. Depending on the stimuli that trigger their activation, TAM are polarized towards form M1, contributing to antitumor responses, or M2, associated with tumor progression. Many studies demonstrated a co

Transient increase in blasts mimicking acute leukemia and progressing myelodysplasia in patients receiving growth factor.

Previous studies of the hematologic effects of granulocyte colony-stimulating factor (G-CSF) and granulocyte-macrophage colony-stimulating factor (GM-CSF) have emphasized the morphologic changes induced by these growth factors, but few have reported increases in blasts. Here, we report six cases in which growth factor treatment resulted in a marked but temporary increase in peripheral and bone marrow blasts that led to diagnostic confusion with acute leukemia and high-grade myelodysplastic syndromes. Five of the six patients were receiving treatment for hematologic malignant neoplasms, and one

Isolated del(5q) with myeloproliferative driver mutations: A systematic review of published cases and clinical implications.

BACKGROUND: Myeloid neoplasms harboring both an isolated deletion of chromosome 5q (del(5q)) and myeloproliferative neoplasm (MPN) driver mutations (JAK2, CALR, MPL) constitute a rare and diagnostically challenging subset, often described with features overlapping those of myelodysplastic syndromes (MDS) and MPN. METHODS: We systematically reviewed published case reports and case series of adult patients with myeloid neoplasms and concomitant isolated del(5q) and MPN driver mutations. A comprehensive search of PubMed and Google Scholar from inception through 31 December 2024 was performed usi

A 26-Year-Old Female with Systemic Mastocytosis with Associated Myeloid Neoplasm with Eosinophilia and Abnormalities of PDGFRB, t(4;5)(q21;q33).

Various translocations involving the PDGFRB gene are identified in myeloid neoplasms. However, the PRKG2/PDGFRB fusion gene associated with t(4;5)(q21;q33) has previously been reported in only 3 patients. We present the case of a 26-year-old woman with microcytic anemia, basophilia, thrombocytosis, and massive splenomegaly, who was found to have systemic mastocytosis and associated clonal hematological non-mast cell lineage disease (SM-AHNMD), with myeloid neoplasm with PRKG2/PDGFRB rearrangement. Initial findings included basophilia (37%, 4.1 k/μL), hypercellular marrow with eosinophilia, and

Acute lymphoblastic leukemia and lymphoma in the context of constitutional mismatch repair deficiency syndrome.

Constitutional mismatch repair deficiency (CMMRD) syndrome is one of the rare diseases associated with a high risk of cancer. Causative mutations are found in DNA mismatch repair genes PMS2, MSH6, MSH2 or MLH1 that are well known in the context of Lynch syndrome. CMMRD follows an autosomal recessive inheritance trait and is characterized by childhood brain tumors and hematological malignancies as well as gastrointestinal cancer in the second and third decades of life. There is a high risk of multiple cancers, occurring synchronously and metachronously. In general, the prognosis is poor. About

Aggressive mediastinal amyloidosis mimicking malignancy: a case report.

INTRODUCTION: Amyloidosis is characterized by extracellular protein fibril deposition. Light-chain (AL) amyloidosis may be associated with plasma cell dyscrasias. Isolated mediastinal involvement is exceptionally rare and may mimic malignant tumors. CASE PRESENTATION: A 69-year-old woman presented with a progressively enlarging anterior chest wall mass caused by an invasive mediastinal lesion. Biopsy revealed amyloid deposition. She underwent bilateral thoracotomy, sternectomy, en-bloc chest wall resection, and reconstruction. Final pathology confirmed AL amyloidosis associated with a kappa-r

Blastic plasmacytoid dendritic cell neoplasm: ontogeny, immunophenotype, genetics and epigenetics, immune dysregulation, skin tropism and therapeutic implications.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, highly aggressive hematologic malignancy characterized by frequent cutaneous involvement, rapid systemic dissemination, and poor clinical outcomes. Although initially misclassified due to overlapping morphologic and immunophenotypic features with other acute leukemias, lymphomas, and NK-cell neoplasms, BPDCN is now recognized as a distinct hematologic malignancy. Advances in immunophenotyping, transcriptional profiling, and genomic analysis have clarified the cellular origin of BPDCN and revealed that the disease commonly arises f

An Uncommon Link: Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) in the Setting of Myeloproliferative Neoplasms.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a well-known cause of hyponatremia, frequently associated with small-cell lung carcinoma. Its association with myeloproliferative neoplasms (MPNs), however, is rare and underreported. We describe the case of an 81-year-old female with a history of chronic hyponatremia attributed to SIADH, requiring multiple hospitalizations. During her hospitalizations, she would be on fluid restriction and urea tablets. Her medical history included paroxysmal atrial fibrillation, a stable pulmonary nodule, and seropositive rheumatoid arthriti

Multilineage lymphoid clonal hematopoiesis: a cross-sectional, correlative analysis of plasma cell dyscrasias and large granular lymphocytic leukemia.

Clonal expansions in large granular lymphocytic leukemia (LGLL) may arise in response to immune dysregulation in the context of other hematologic malignancies. This study aimed to investigate the co-occurrence of LGLL and plasma cell dyscrasias (PCDs) to assess its prevalence, features, and potential underlying mechanisms. We conducted a cross-sectional study involving 2064 PCD cases and 534 LGLL cases. Of the 534 LGLL cases, 20% co-occurred with PCD, while LGLL was present in 2% of the 2064 PCD cases. Among 117 patients with both conditions, PCDs were predominantly associated with IgM M-prote

Molecular Abnormalities and Associated Clinical Features in Polycythemia Vera.

Background/Objectives: Myeloproliferative neoplasms are a group of clonal myeloid malignancies that affect bone marrow and include polycythemia vera (PV), essential thrombocythemia (ET) and primary myelofibrosis (PMF). PV is caused in most patients by the JAK2-V617F mutation and is characterized at the phenotypic level by overproduction and accumulation of red blood cells. PV is associated with significant morbidity, including risk of thrombotic events and of hematologic evolution (myelofibrotic or leukemic transformation) and reduced survival. In addition to the JAK2-V617F mutation, PV patien

PPM1D in Solid and Hematologic Malignancies: Friend and Foe?

In the face of constant genomic insults, the DNA damage response (DDR) is initiated to preserve genome integrity; its disruption is a classic hallmark of cancer. Protein phosphatase Mg2+/Mn2+-dependent 1D (PPM1D) is a central negative regulator of the DDR that is mutated or amplified in many solid cancers. PPM1D overexpression is associated with increased proliferative and metastatic behavior in multiple solid tumor types and patients with PPM1D-mutated malignancies have poorer prognoses. Recent findings have sparked an interest in the role of PPM1D in hematologic malignancies. Acquired somati

B-cell acute lymphoblastic leukemia in patients with germline RUNX1 mutations.

Germline RUNX1 mutations underlie a syndrome, RUNX1-familial platelet disorder (RUNX1-FPD), characterized by bleeding symptoms that result from quantitative and/or qualitative defect in platelets and a significantly increased risk for developing hematologic malignancies. Myeloid neoplasms are the most commonly diagnosed hematologic malignancies, followed by lymphoid malignancies of T-cell origin. Here, we describe the first 2 cases of B-cell acute lymphoblastic leukemia (B-ALL) in patients with confirmed germline RUNX1 mutations. While 1 of the patients had a known diagnosis of RUNX1-FPD with

Therapy-associated myelodysplastic syndrome with monosomy 7 arising in a Muir-Torre Syndrome patient carrying SETBP1 mutation.

Muir-Torre Syndrome (MTS) is a rare hereditary autosomal dominant cancer syndrome and is linked to hereditary non-polyposis colorectal carcinoma (Lynch Syndrome). Individuals develop various skin neoplasms in addition to colorectal, endometrial and upper gastrointestinal malignancies. Therapy-associated myelodysplastic syndrome (T-MDS) is an aggressive hematologic malignancy and is considered a pre-leukemic phase. T-MDS is associated with prior exposure to chemo- and radiotherapy that potentially results in DNA damage. The current case report presents a 74-year-old male MTS patient with prior
