[Transtracheal jet ventilation in the operating room].

For the last 40 years transtracheal ventilation has been suggested as a means of bypassing the glottis in emergency in patients unable to be intubated or ventilated by mask. A catheter has been designed to be easily inserted into the crico-thyroid membrane. The outer part of the device, with its dual attachment system, can be connected to conventional resuscitation equipment by its 15 mm male end or to a high-pressure oxygen source by its Luer-Lock fitting. The advantages of transtracheal ventilation are: a clear vision of the operative field, a good gas exchange, an elimination of airway trau

Ventilator Management in Extremely Preterm Infants.

Advances in ventilation strategies for infants in the NICU have led to increased survival of extremely preterm infants. More than 75% of infants born at less than or equal to 27 weeks' gestation require initial mechanical ventilation for survival due to developmental immaturity of their lungs and respiratory drive. Various ventilators using different technologies and involving multiple management strategies are available for use in this population. Centers across the world have successfully used conventional, high-frequency oscillatory and high-frequency jet ventilation to manage respiratory f

Ventilation Strategies in a Pediatric Patient Undergoing Resection and Anastomosis for a Juxta Carinal Tumor.

Tumors close to the carina pose a unique challenge to the Anesthesiologist in terms of lung isolation and ventilation strategies. We present the case of a 15-year-old patient who was diagnosed with a myofibroblastic tumor in the left main bronchus and carina, posted for a robotic assisted sleeve resection and anastomosis. The surgical approach was from the right side. Hence, the traditional strategies for right lung ventilation could not be employed. A combination of endobronchial intubation, jet ventilation via airway exchange catheter (AEC) and cross field ventilation enabled us to complete

Effect of ventilator modes on neonatal cerebral and peripheral oxygenation using near-infrared spectroscopy.

AIM: The effect of ventilator modes on regional tissue oxygenation in premature neonates with respiratory distress syndrome (RDS) has yet to be delineated. Previous studies have looked at global oxygen delivery and have not assessed the effects on regional tissue oxygenation. Our aim in this study was to assess such tissue oxygenation in premature babies with RDS in relation to differing modes of ventilation using near-infrared spectroscopy (NIRS). METHODS: In 24 stable preterm infants with RDS, undergoing elective wean in ventilator mode, cerebral and muscle tissue oxygenation were assessed

Simulated laryngoscopy with supraglottic high-pressure source ventilation: an in vitro study of tracheal airflow to determine optimal positioning of laryngoscope and jet ventilation cannula.

PURPOSE: To investigate how variations in positioning of laryngoscope and location of jet cannula on the laryngoscope body influence tracheal airflow during simulated high-pressure source supraglottic (HPSV) jet ventilation laryngoscopy using an anatomical model. METHODS: A Broncho Boy Bronchoscopy model was modified to allow recording of tracheal airflow. A laryngoscope was suspended and positioned to simulate laryngoscopy. HPSV was delivered by a jet cannula attached to the body of the laryngoscope. Different combinations of laryngoscope angulation and cannula attachment were used and air f

Chest high-frequency oscillatory treatment for severe atelectasis in a patient with toxic epidermal necrolysis.

Atelectasis is a significant risk factor for the development of pneumonia, especially in pediatric populations more prone to alveolar collapse or those who may have weakened muscular tone. The Metaneb system is a pneumatic, noninvasive physiotherapy technique that delivers chest high-frequency oscillations. Chest high-frequency oscillations have been shown to enhance mucociliary clearance of secretions and help resolve patchy atelectasis. This report describes the case of a 17-year-old female patient who developed significant left-sided atelectasis after extubation and was effectively managed

Experimental analysis of pendelluft flow generated by HFOV in a human airway model.

HFOV (High frequency oscillatory ventilation) is an effective artificial respiration technique for the treatment of pulmonary disease patients. In HFOV, the pendelluft is one of the important factors in gas transport and mixing mechanisms. Experimental analysis of the pendelluft flow generated in a bronchial tube model of single bifurcation with different volume ratios of daughter branches has been carried out by using time-resolved micro-particle image velocimetry obtaining instantaneous velocity distributions of oscillatory flows in the bronchial tube model at a frequency range of 10-15 Hz.

[Differential lung ventilatory management during bronchoplasty].

We compared two methods of respiratory managements during bronchoplasty surgery. In one lung ventilation group (OLV-G), 10 patients were ventilated with Broncho-cath tube or Univent tube. On another 10 patients, ventilation was performed with Univent tube following insertion of bronchial blocker into main bronchus of dependent lung. Dependent lung was then ventilated using high frequency jet ventilation (HFJV) through bronchial blocker superimposed with low tidal volume IPPV (selective HFJV, S-HFJV-G). Oxygenation index (O.I.) of S-HFJV-G was significantly higher than that of OLV-G when bronch

Redefining Safe Practices in Airway TruBlue Laser Surgery-A Systematic Evaluation.

OBJECTIVES: Fire risk during laser airway surgery is well-documented, but there is limited information regarding this risk with the novel TruBlue laser. This study evaluates its safety under different experimental conditions. METHODS: Using a cadaveric porcine larynx and lungs model, we conducted trials with laser-safe endotracheal tubes (LSETTs), high-flow ventilation (HFV), and jet ventilation. We varied distance, FiO2 levels, and laser power settings to observe the occurrence of uncontrolled fires, brief flames, and sparks. RESULTS: LSETT trials showed no fire events. Pledget fires were t

Revisiting Fire Safety Guidelines in CO2 Laser Airway Surgery.

INTRODUCTION: The potential for airway fire during endoscopic laser surgery is well known, typically addressed by reducing FiO2 levels. This study revisits these established practices in CO2 laser tubeless airway surgery. MATERIALS AND METHODS: Using a cadaveric porcine larynx and lung model, we conducted trials with high-flow (HFV) and jet ventilation. We varied FiO2 levels and laser power settings to record the onset and number of brief, expansive, and blowtorch flames. RESULTS: Flames were observed in all jet ventilation trials. Multivariable model results showed that increasing laser wat

Acute respiratory distress syndrome in a child with severe epileptic disorder treated successfully by extracorporeal membrane oxygenation: a case report.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is now a candidate therapy for children with acute respiratory failure. CASE PRESENTATION: We report our experience of using central ECMO therapy for acute respiratory distress syndrome followed by seizure in a 15-month-old girl with a severe epileptic disorder. Her respiratory distress was refractory to standard medical treatment and mechanical ventilatory support. Her condition was complicated by development of a pneumothorax. The patient was successfully weaned off ECMO and discharged without deterioration of her neurological status.

Use of jet ventilation in thoracoscopic tracheo-esophageal fistula repair-can both surgeons and anesthesiologists be happy?

Laparoscopic and open thoracic surgery in the neonate typically results in hypercapnea and low cardiac output with often poor surgical visualization as the anesthesiologist attempts to correct the respiratory derangements usually seen. We describe three cases in which jet ventilation provided not only superior ventilation with a return to normocapnea but also ideal operating conditions. In addition, jet ventilation utilizes lower mean airway pressures which typically results in improved cardiac output.

Oscillations in stroke volume and cardiac output arising from oscillatory ventilation in humans.

Oscillations in the cardiovascular system have been observed in patients with periodic breathing. It is not clear whether these are driven by primary oscillations in the respiratory system or whether an intrinsic cardiovascular instability is required, as previous studies with subjects performing voluntary periodic breathing have failed to produce the cardiovascular oscillations. We investigated whether cardiovascular oscillations occurred in healthy controls performing voluntary periodic breathing. Six healthy subjects performed voluntary periodic breathing with guidance from a real-time comp

Adult respiratory failure.

Pulmonary complications following thoracic surgery are common and associated with significant morbidity and mortality. In particular, acute respiratory distress syndrome (ARDS) can occur postoperatively or after trauma. This syndrome, when complicated by multisystem organ failure, often leads to a poor outcome. This article describes the etiology and pathophysiology of ARDS and evaluates recent advances in pharmacological and nonpharmacological therapies. In addition, newer modalities of mechanical ventilatory support are reviewed.
