Targeting TLR2 agonists as immunomodulators with broad antiviral activity.

Toll-like receptor-2 (TLR2) serves as an innate immune sensor that recognizes specific viral proteins, thereby initiating signaling pathways that can either enhance antiviral host defenses or induce pathological inflammatory responses. In this forum, we discuss the role of TLR2 and its agonists in enhancing antiviral and vaccine-induced immune responses.

Targeted Inhibition of Intracellular Coxiella burnetii by Overcoming the Vacuolar Barrier with Autologous Exosomal Doxycycline.

Coxiella burnetii, the causative agent of Q fever, is an obligate intracellular bacterium that replicates within the Coxiella-containing vacuole (CCV), a lysosome-like compartment. The acidic microenvironment within the CCV impairs the activity of weak-base antibiotics such as doxycycline, necessitating prolonged, high-dose therapy. To overcome this fundamental delivery barrier, we engineered a biomimetic nanoplatform by encapsulating doxycycline (DOX) within exosomes derived from THP-1 cell-derived macrophages (tExo). DOX@tExo nanoparticles displayed characteristic exosomal morphology and pro

The microbiota-systemic lupus erythematosus axis: mechanisms, diagnostics, and therapeutic frontiers.

Systemic lupus erythematosus (SLE) is a prototypical autoimmune disease in which host-microbiota crosstalk plays a pivotal role in immune dysregulation. Recent metagenomic studies have revealed that disease-specific dysbiosis--characterized by the expansion of pathobionts and depletion of immunoregulatory commensals--occurs across the gut, oral cavity, skin, and genital tract. Integrative multi-omics analyses have identified three mechanistic pathways linking microbial imbalance to autoimmunity: (1) microbial peptides trigger molecular mimicry and epitope spreading, activating autoreactive lym

Scutellarin suppresses Mycobacterium tuberculosis-induced pyroptosis in macrophages by inhibiting the HIF-1α-mediated Warburg effect.

BACKGROUND: Mycobacterium tuberculosis (Mtb), the causative agent of tuberculosis (TB), remains a major global health threat due to prolonged treatment and drug-resistant strains. Host-directed therapy (HDT), which modulates host-pathogen interactions, offers potential to shorten treatment and limit resistance. This study investigates the effects of Scutellarin (SCU), a flavonoid from Scutellaria baicalensis, on Mtb-infected macrophages within the HDT framework. METHODS: Anti-pyroptotic and anti-inflammatory effects of SCU were assessed in Mtb-infected THP-1 and J774A.1 macrophages, and in a …

Primary antifungal prophylaxis in hematopoietic stem cell transplant recipients: clinical implications of recent studies.

PURPOSE OF REVIEW: Infectious complications after hematopoietic stem cell transplant, especially those caused by fungi, contribute to poor outcomes. Availability of new antifungal drugs has led to creative attempts to prevent infections caused by both yeasts and molds with new antifungal prophylaxis strategies; these studies will be reviewed. RECENT FINDINGS: Trials evaluating mold-active azoles, posaconazole and voriconazole, demonstrate that invasive aspergillosis infections can be prevented by prophylactic therapy in patients with hematologic malignancies and after allogeneic hematopoietic

Structural and biochemical insights into the inhibition of Mycobacterium tuberculosis cyclic dinucleotide phosphodiesterase by a sulfur-modified cyclic dinucleotide analog.

Cyclic dinucleotide (CDN) signaling plays a pivotal role in bacterial physiology and host-pathogen interactions. Mycobacterium tuberculosis (Mtb) releases a cyclic dinucleotide phosphodiesterase (CdnP) in the macrophages, which hydrolyzes Mtb-derived 3'3'-c-di-AMP and 3'3'-c-di-GMP, and a host-derived 2'3'-cGAMP STING agonist, to evade the host's innate immune response mediated by the STING protein. Therefore, by inhibiting CdnP released into host cells, the STING pathway can be potentiated, leading to improved bacterial clearance, which represents a potential novel approach for anti-tuberculo

Human cytomegalovirus-encoded MicroRNAs: A master regulator of latent infection.

Human cytomegalovirus (HCMV) can modulate the host cell microenvironment to cause latent infection and is therefore considered a major health concern in immunocompromised patients. HCMV-encoded microRNAs (miRs) have emerged as a key player in regulating the expression of the host cell and viral genes to induce latent infection. HCMV-encoded miRs can inhibit antiviral immune responses, such as proinflammatory mediators production, antigenic presentation, and apoptosis. In addition, HCMV miRs can reduce viral DNA replication. In this review, we describe the mechanisms underlying HCMV-encoded miR

Costimulatory protein B7-1 enhances the cytotoxic T cell response and antibody response to hepatitis B surface antigen.

There is a need for more effective therapy for chronic virus infections. A principle natural mechanism for elimination of virus-infected host cells is activation of viral antigen-specific cytotoxic T lymphocytes (CTL). In an effort to develop methods of inducing virus-specific CTL responses that might be utilized in therapy of virus infections, we have investigated the effect of B7, a costimulatory factor for T-cell activation. In this study we show that delivery of genes encoding human B7-1 and a viral antigen in the same recombinant viral vector to cells of mice induces a greater viral antig

The pathogenesis and clinical management of cytomegalovirus infection in the organ transplant recipient: the end of the 'silo hypothesis'.

PURPOSE OF REVIEW: Cytomegalovirus infection is initiated when tumor necrosis factor binds to the cytomegalovirus receptors of latently infected cells, resulting in the reactivation of the virus and the production of clinical disease of two types: direct infection causing pneumonia, mononucleosis, colitis and other viral-related syndromes, and indirect infection in which an array of cytokines are released by the host that produce much the same effect as does the rejection process. RECENT FINDINGS: These effects fall into three categories: allograft injury, an increase in superinfection with o

A Review of Hepatitis B Virus and Hepatitis C Virus Immunopathogenesis.

Despite the advances in therapy, hepatitis B virus (HBV) and hepatitis C virus (HCV) still represent a significant global health burden, both as major causes of cirrhosis, hepatocellular carcinoma, and death worldwide. HBV is capable of incorporating its covalently closed circular DNA into the host cell's hepatocyte genome, making it rather difficult to eradicate its chronic stage. Successful viral clearance depends on the complex interactions between the virus and host's innate and adaptive immune response. One encouraging fact on hepatitis B is the development and effective distribution of t

Microbes that chew allergens mitigate anaphylaxis.

In the current issue of Cell Host & Microbe, Sánchez-Martínez et al. demonstrate that oral and small-intestine small intestinal bacteria can modulate IgE-mediated anaphylaxis through metabolic degradation of immunodominant peanut allergens. This study provides insights into microbial mechanisms underlying food-induced anaphylaxis and may point to new directions for advancing food allergy therapy.

Acetate modulates host physiology and bacterial energetics to improve antibiotic efficacy during obesity-associated infection.

UNLABELLED: Antibiotic efficacy is conventionally attributed to the susceptibility of the infecting pathogen. However, obese patients experience disproportionately poor infection outcomes despite receiving antibiotics to which pathogens remain genetically susceptible. These observations suggest that host-associated factors can influence treatment outcomes independently of the bacterial genotype, although the underlying mechanisms remain poorly understood. Using a mechanistic comparison of diet-induced obese and healthy mice infected with Acinetobacter baumannii, we demonstrate that the effect

BCMA-targeted therapy enables combined haploidentical stem cell and kidney transplantation in a highly sensitized patient.

A 64-year-old woman with multiple myeloma and end-stage kidney disease was initially deemed ineligible for combined haploidentical hematopoietic stem cell and kidney transplantation due to positive flow cytometry crossmatch from high levels of donor-specific antihuman leukocyte antigen antibodies (DSA). She was treated with elranatamab, a B cell maturation antigen-directed CD3 bispecific T cell engager, which induced a deeper myeloma remission and led to a marked reduction in DSAs. Her calculated panel reactive antibodies decreased significantly from 97% to 24%. Once DSA levels fell below 1,00

Paraneoplastic Minimal Change Disease Signaling Post-Transplant AML Relapse: Two Cases and a Literature Review.

Membranous nephropathy (MN) and minimal change disease (MCD) are the most common causes of nephrotic syndrome following hematopoietic stem cell transplantation (HSCT), a complication conventionally attributed to chronic graft-versus-host disease (GVHD). Paraneoplastic MCD is well described in lymphoid malignancies but is rarely reported in myeloid neoplasms. We report two cases of biopsy-confirmed MCD presenting as the initial manifestation of acute myeloid leukemia (AML) relapse following allogeneic HSCT. Both patients were White men in their sixties with relapsed/refractory AML who developed

Whole-genome sequencing reveals complex genomic features underlying anti-CD19 CAR T-cell treatment failures in lymphoma.

CD19-directed chimeric antigen receptor (CAR-19) T cells are groundbreaking immunotherapies approved for use against large B-cell lymphomas. Although host inflammatory and tumor microenvironmental markers associate with efficacy and resistance, the tumor-intrinsic alterations underlying these phenomena remain undefined. CD19 mutations associate with resistance but are uncommon, and most patients with relapsed disease retain expression of the wild-type receptor, implicating other genomic mechanisms. We therefore leveraged the comprehensive resolution of whole-genome sequencing to assess 51 tumo

Treatment with Atorvastatin Provides Additional Benefits to Imipenem in a Model of Gram-Negative Pneumonia Induced by Klebsiella pneumoniae in Mice.

The clinical pathogen Klebsiella pneumoniae is a relevant cause of nosocomial infections, and resistance to current treatment with carbapenem antibiotics is becoming a significant problem. Statins are inhibitors of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) used for controlling plasma cholesterol levels. There is clinical evidence showing other effects of statins, including decrease of lung inflammation. In the current study, we show that pretreatment with atorvastatin markedly attenuated lung injury, which was correlated with a reduction in the cellular influx into the alveolar space and

The "yeast-drop" treatment protocol in the Drosophila model for rapid and cost-effective TB drug testing.

BACKGROUND: Pulmonary tuberculosis (TB), caused by Mycobacterium tuberculosis, is the leading infectious disease globally. The lengthy treatment regimen and the potential side effects increase the probability of relapse and of developing drug resistance. These factors highlight the need for new therapeutic strategies, including host-directed therapies and anti-virulence approaches. However, the drug discovery pipeline is often limited by the simplicity of in vitro models and the cost and scalability challenges of mammalian in vivo models. In this study, we developed a cost-effective administra

Host-targeting therapies for hepatitis C virus infection: current developments and future applications.

Chronic hepatitis C virus (HCV) infection is a leading cause of chronic liver diseases and hepatocellular carcinoma (HCC) worldwide. In the past few years, anti-HCV therapies have undergone a revolution with the approval of multiple direct-acting antivirals (DAAs), which enable interferon-free treatments with considerable improvement of sustained virologic response in patients. Today, DAAs have become the standard of care for HCV therapy. However, several limitations remain, which include access to therapy, treatment failure in a subset of patients and persistent risk of HCC development follow
