No wrong door: Can clinical care facilitate veteran engagement in housing services?

It is well established that providing stable housing to homeless persons improves health outcomes. It is less clear whether engagement in clinical care facilitates housing outcomes. We present a post hoc analysis of a prospective, community-based randomized controlled trial of homeless veterans not actively receiving or assigned to a primary care. Study subjects were interviewed at baseline, 1 month and 6 months and survey results were supplemented/verified by review of all notes in their VA electronic medical record for 6 months postenrollment. A total of 142 subjects with complete data were …

Provider perceptions on HIV risk and prevention services within permanent supportive housing.

Permanent supportive housing (PSH) is an evidence-based solution to homelessness for persons experiencing chronic or long-term homelessness and one or more physical or behavioral health problems. Health services through PSH typically focus on physical and behavioral health. With the exception of programs specifically designed for persons living with HIV/AIDS, little attention has focused on services through PSH to prevent transmission of HIV or other sexually transmitted infections (STIs), yet sexual risk behavior continues after homeless persons move into PSH. The purpose of this study was to…

Screening for Housing Instability and Homelessness Among Families Undergoing Child Maltreatment Investigation.

This paper reports results of mixed methods, population survey of housing instability, and homelessness. Child welfare personnel conducted the Quick Risks and Assets for Family Triage (QRAFT), a three-question screening tool intended to identify housing instability and homelessness. The QRAFT requires users to assess family housing history, current housing arrangement, and current housing condition, on a four-point scale from "asset/not a risk" to "severe risk." The QRAFT was completed among 6828 families undergoing new child maltreatment investigations. Approximately 5.4% of families demonstr…

Larger cages with housing unit environment enrichment improve the welfare of marmosets.

The provision of adequate space for laboratory animals is essential not only for good welfare but accurate studies. For example, housing conditions for primates used in biomedical research may negatively affect welfare and thus the reliability of findings. In common marmosets (Callithrix jacchus), an appropriate cage size enables a socially harmonious family environment and optimizes reproductive potential. In this study, we investigated the effects of cage size on body weight (BW), behavior, and nursing succession in the common marmoset. Large cages (LCs) with environment enrichment led to an…

Factors contributing to eviction from permanent supportive housing: Lessons from HUD-VASH.

INTRODUCTION: Eviction from housing is associated with several negative outcomes, further exacerbated among high-need populations requiring financial and supportive services to maintain housing stability. This study investigated risk and protective factors-both characteristics and precipitating events of tenant eviction-informing permanent supportive housing (PSH) programs' efforts to identify tenants at risk and intervene. METHODS: Using administrative data for a cohort of 20,146 Veterans participating in PSH, this study assessed differences in Veterans who exited the program due to eviction…

Perceived Housing Discrimination and Self-Reported Health: How Do Neighborhood Features Matter?

BACKGROUND: While the association between perceived discrimination and health has been investigated, little is known about whether and how neighborhood characteristics moderate this association. PURPOSE: We situate discrimination in the housing context and use relative deprivation and social capital perspectives to fill the knowledge gap. METHODS: We applied multilevel logistic modeling to 9,842 adults in 830 neighborhoods in Philadelphia to examine three hypotheses. RESULTS: First, the detrimental effect of discrimination on self-reported health was underestimated without considering neigh…

The interplay of rural issues, mental illness, substance use and housing problems.

BACKGROUND: People with mental illness and substance use problems form a significant subgroup of the homeless population. International research has begun to document the complex experiences of this vulnerable group; however, less attention has focused on those living in rural areas. AIM: This study sought to determine the experiences of people with mental illness and/or substance use issues, experiencing significant housing problems in rural areas. METHODS: Within a qualitative framework, individual interviews were conducted with 40 respondents in Australia. RESULTS: Themes generated a dis…

Promoting health and advancing development through improved housing in low-income settings.

There is major untapped potential to improve health in low-income communities through improved housing design, fittings, materials and construction. Adverse effects on health from inadequate housing can occur through a range of mechanisms, both direct and indirect, including as a result of extreme weather, household air pollution, injuries or burns, the ingress of disease vectors and lack of clean water and sanitation. Collaborative action between public health professionals and those involved in developing formal and informal housing could advance both health and development by addressing ris…

A Sort of Homecoming: Incarceration and the housing security of urban men.

While individuals returning from prison face many barriers to successful re-entry, among the most serious are the challenges they face in securing housing. Housing has long been recognized as a prerequisite for stable employment, access to social services, and other aspects of individual and family functioning. The formerly incarcerated face several administrative and de facto restrictions on their housing options; however, little is known about the unique instabilities that they face. We use a longitudinal survey of urban families to examine housing insecurity among nearly 3,000 urban men, in…

Addiction treatment and stable housing among a cohort of injection drug users.

BACKGROUND: Unstable housing and homelessness is prevalent among injection drug users (IDU). We sought to examine whether accessing addiction treatment was associated with attaining stable housing in a prospective cohort of IDU in Vancouver, Canada. METHODS: We used data collected via the Vancouver Injection Drug User Study (VIDUS) between December 2005 and April 2010. Attaining stable housing was defined as two consecutive "stable housing" designations (i.e., living in an apartment or house) during the follow-up period. We assessed exposure to addiction treatment in the interview prior to th…

Housing stability over two years and HIV risk among newly homeless youth.

The stability of living situation was examined as a predictor of young people's HIV-related sexual and drug use acts two years after leaving home for the first time. Newly homeless youth aged 12-20 years were recruited in Los Angeles County, California, U.S.A. (n = 261) and Melbourne, Australia (n = 165) and followed longitudinally at 3, 6, 12, 18, and 24 months. Their family history of moves and the type and frequency of moves over the two years following becoming newly homeless were examined. Regression analyses indicated that recent sexual risk two years after becoming newly homeless was no

Tenant outcomes in supported housing and community residences in New York City.

OBJECTIVE: This study examined whether outcomes in housing, clinical status, and well-being of persons with severe mental illness and a history of homelessness differ between those in supported housing and those in community residences, two housing arrangements that substantially differ in the level of independence that is offered to its tenants. METHODS: A quasi-experimental 18-month follow-up study was conducted with 157 persons newly entering supported housing and community residences. The housing models accepted persons with similar illness characteristics and homelessness histories, so t…

Screening offenders with histories of mental illness and violence for supportive housing.

This article describes a community mental health agency's process of screening supportive housing applicants with histories of violent felonies and serious mental illness. The agency adopted its corporate intranet as a tool so that geographically dispersed senior staff could participate in information gathering in order to ensure expert input in admissions decisions. This broad-based participation was designed to maintain community safety, while making the agency's residential resources available to people with mental illness and criminal histories. Considering the high recidivism rate of ex-o…

Decision polarization among rehabilitation team recommendations concerning discharge housing for stroke patients.

This study sought to identify if decision polarization effects were operating in rehabilitation teams when making discharge housing recommendations for stroke patients. Using a Social Judgment Theory approach, individual clinicians were asked to nominate discharge housing for stroke patients. Teams were then assembled and clinicians repeated the task. The research was conducted at a sample of seven in-patient rehabilitation hospitals. The subjects were 74 clinicians who formed 13 teams. All subjects were volunteers, and represented the following professions: medicine, nursing, occupational the…

Brain opioid receptors in relation to stereotypies, inactivity, and housing in sows.

When animals encounter difficult conditions, endogenous opioids are often released and may help in coping with the difficulties. In sows, prolonged confinement results in behavior abnormalities: high levels of stereotypies or excessive inactivity and unresponsiveness. The possibilities of causal links between endogenous opioids and these behavioral indicators of poor welfare have been raised. Mu receptor density was found to be greater in tethered sows than in group-housed sows and to be positively correlated with time spent inactive. There were negative correlations between both mu and kappa

Methods of assessing mental health consumers' preferences for housing and support services.

The authors reviewed 21 studies assessing housing preferences of mental health consumers to examine current methods of assessment, to obtain information on the reliability and validity of the assessment instruments used, and to determine whether a better approach to assessing preferences might be found. The review found little data on the reliability or validity of the assessment instruments and revealed heavy reliance on the use of fixed-choice questions that limit consumer expression of preferences. Responses to such questions can be misleading because they may or may not reflect real-life c…

Evaluation of Federally Mandated Smoke-Free Housing Policy and Health Outcomes Among Adults Over the Age of 50 in Low-Income, Public Housing in New York City, 2015-2022.

INTRODUCTION: Effective July 2018, the U.S. Department of Housing and Urban Development issued a rule requiring all public housing authorities to implement smoke-free housing (SFH) policies in their developments. We examined the differential impacts of SFH policy on hospitalizations for myocardial infarction (MI) and stroke among adults aged ≥50 years old living in New York City (NYC) Housing Authority (NYCHA) versus a matched-comparison population in NYC. AIMS AND METHODS: We identified census block groups (CBGs) comprised solely of 100% NYCHA units (N = 160) and compared NYCHA CBGs to a sel…

"We Have to Evict Certain People Sometimes": A Descriptive Mixed-Methods Study of Service Providers' Perceptions of the Impacts of High-Risk Issues in Supportive Housing and Housing First.

High-risk issues, such as overdose, suicidality, hoarding, violence, property damage, and apartment takeovers, are known challenges in supportive housing and Housing First programs. However, the effects of these incidents on residents, service providers, and programs have been minimally studied. The objective of this mixed-methods study was to understand what service providers perceived as the impacts of various high-risk issues in supportive housing and Housing First programs, with an emphasis on housing tenure. This descriptive study used an exploratory sequential mixed-methods design, with …

Housing First for homeless people with severe mental illness: extended 4-year follow-up and analysis of recovery and housing stability from the randomized Un Chez Soi d'Abord trial.

AIMS: Housing First (HF), a recovery-oriented approach, was proven effective in stabilising housing situations of homeless individuals with severe mental disorders, yet had limited effectiveness on recovery outcomes on a short-term basis compared to standard treatment. The objective was to assess the effects of the HF model among homeless people with high support needs for mental and physical health services on recovery, housing stability, quality of life, health care use, mental symptoms and addiction issues on 4 years of data from the Un Chez Soi d'Abord trial. METHODS: A multicentre random…
