Acute Kidney Injury Associated With Cholera.

Cholera is an acute infectious disease caused by Vibrio cholerae. Its clinical course varies from mild diarrhea to severe complications with hypokalemia, hyponatremia or hypernatremia, hypocalcemia, metabolic acidosis, and acute kidney injury. This is a case of a 20-year-old Asian man with recent travel history from Bangladesh who presented to the emergency department with abdominal pain and multiple episodes of watery diarrhea. He developed acute renal failure secondary to severe gastroenteritis, the cause of which was later confirmed to be cholera.

Central pontine myelinolysis: an immunofluorescent study.

A young pregnant woman showing clinical symptoms of numbness, vertigo, diplopia, and amnesia with associated hypernatremia ran a fulminant course. At autopsy, an area or extensive demyelination in the center of the pons was found and a pathological diagnosis of central pontine myelinolysis was made. Since definitive etiology and pathogenesis of the lesion have not been established, we investigated the role of humoral immunity in the pathogenesis of this condition by retrospective immunofluorescent study; the result was negative. Possible factors related to the etiology and pathogenesis of the …

[Incidence of hypernatremic dehydration in acute infantile gastroenteritis].

A retrospective study on the incidence in our hospital of hypernatremic dehydration was performed. Authors observed a linear decrease both in the frequency of hypernatremic dehydration and in the incidence of hypernatremias above 160 mmol/l, during the 8-year period included in the study. For the past few years a tendency to see affected babies of older age was also observed. We believe that the reasons for these facts are the new dietary habits in infancy feeding and specially the greater use of formulas with low solute and protein content.

Panhypopituitarism in a Patient with Burkitt Lymphoma: A Diagnostic and Therapeutic Challenge.

Pituitary infiltration by systemic lymphoma is an exceedingly rare occurrence. Given its high mortality rate, it is crucial to recognize its … onfirmed Burkitt lymphoma (BL). During hospitalization, the patient developed polyuria, polydipsia, hypernatremia, fluid-resistant hypotension and hypoglycaemia. Corticosteroid therapy was initiated due to suspected adrenal insufficiency, resulting in clinical improvement but exacerbation of polyuria and hypernatremia. Plasma and urinary osmolarity confirmed ar …

RESULTS: Significant hypernatremia was found in the sodium pyruvate group. During hypoxia, control and MNA group developed profound STU depressions on ECG while no changes were observed in MNA pyruvate and sodium pyruvate group. Postoperative hypernatremia occurred in 37 patients (44.6%), and hyponatremia occurred in 60 patients (72.3%), the average time of the occurrence of hpernatremia and hyponatremia was 1.4 and 3.7 days after surgery. Phenotypic variability in Bartter syndrome type I.

Limited phenotypic variability has been reported in patients with Bartter syndrome type I, with mutations in the Na-K-2Cl cotransporter gene … of the previous group, suggesting a single common ancestor. The third patient presented with severe hypernatremia and hyperchloremia for about 2 months, and a diagnosis of nephrogenic diabetes insipidus was hypothesized until the diagnosis of Bartter syndrome type I was established by molecular evaluation. We conclude that in some patients with Bartter syndrome …

Clinically predictive baseline labs for post-operative outcomes of brain tumors using NSQIP database.

PURPOSE: This study was performed to assess the effect of baseline Preoperative Laboratory Values (PLV) on post-operative Brain Tumor Resection (BTR) outcomes in a large national registry. METHODS: We extracted data from the National Surgical Quality Improvement Program (NSQIP) database for BTR patients 2015-2019 (n = 3 0,951). Uni- and multivariate analyses were performed for PLV and key surgical outcomes. Treatment of hyponatremia in children with acute bacterial meningitis.

PURPOSE: Few studies have evaluated hyponatremia management in children with bacterial meningitis (BM). Thus, we aimed to describe variation … remia at 48 h after treatment initiation. Although rare, persistent moderate/severe hyponatremia or hypernatremia at 48 h after treatment initiation was associated with a high mortality rate (57.1%). Adrenal Emergencies.

The adrenal glands drive physiologic homeostasis, with dysregulation in any direction causing multisystem dysfunction. Adrenal excess states include hyperaldosteronism which manifests with refractory hypertension and electrolyte abnormalities including hypernatremia and hypokalemia. Paragangliomas including pheochromocytoma can cause multisystem end-organ dysfunction due to catecholaminergic storm, which require rapid blood pressure control with phentolamine and identification of lesions amenable to surgical resection. Adrenal insufficiency states in contrast can result in hypotension and deco…

Central neurogenic diabetes insipidus, syndrome of inappropriate secretion of antidiuretic hormone, and cerebral salt-wasting syndrome in traumatic brain injury.

Central neurogenic diabetes insipidus, syndrome of inappropriate secretion of antidiuretic hormone, and cerebral salt-wasting syndrome are secondary events that affect patients with traumatic brain injury. All 3 syndromes affect both sodium and water balance; however, they have differences in pathophysiology, diagnosis, and treatment. Differentiating between hypernatremia (central neurogenic diabetes insipidus) and the 2 hyponatremia syndromes (syndrome of inappropriate secretion of antidiuretic hormone, and cerebral salt-wasting syndrome) is critical for preventing worsening neurological outc…

[Life threatening metabolic disorders after application of a sodium phosphate containing enema in the dog and cat].

Application of a sodium phosphate containing enema caused life-threatening metabolic disorders in a Dachshund and two cats. Clinical signs were characterised by dehydration and various neurological deficits including seizures. Most striking laboratory abnormalities were hypernatremia, hypocalcemia, hyperphosphatemia, and polycythemia. Despite intensive treatment the dog died, whereas the cats recovered completely. In face of possible severe and potentially fatal metabolic abnormalities sodium phosphate enemas, such as Practo-Clyss, should be used with caution or not at all in cats and small do…

Recurrent Severe Hyponatremia in a Patient with Sjögren's Syndrome.

Sjögren's syndrome (SS) is an autoimmune disease that presents with exocrine gland dysfunction. Renal involvement is common in SS and often results in tubulointerstitial nephritis, renal tubular acidosis, and Fanconi's syndrome. Electrolyte imbalances are commonly the first symptom of renal involvement of SS. The most common feature of dysnatremia in SS is hypernatremia with diabetes insipidus. However, cases of hyponatremia with syndrome of inappropriate antidiuretic hormone secretion (SIADH) are rarely reported in patients with SS. Herein, we report a case of recurrent severe SIADH in a pati…

Is severe electrolyte imbalance a contraindication for heart transplant?

Owing to the increasing numbers of patients with end-stage congestive heart failure awaiting heart transplant and the limited number of suitable donor organs, a decrease in stringency of donor criteria has become widely accepted over the last decade. Here, we present a case of a heart donor with severe electrolyte imbalance, specifically severe hypokalemia, and severe hypernatremia. Despite this, heart transplant was a success, and the recipient was discharged from the hospital in good general condition. We recommend further study of this issue with a larger sample size.
