In our present studies, we seek to determine whether increased osmolarity stimulates deflation-activated receptors (DARs). In anesthetized, open-chest, and mechanically ventilated rabbits, we recorded single-unit activities from typical slowly adapting receptors (SARs; responding only to lung inflation) and DAR-containing SARs (DAR-SARs; responding to both lung inflation and deflation) and identified their receptive fields in the lung. We examined responses of these two groups of pulmonary sensory units to direct injection of hypertonic saline (8.1% sodium chloride; 9-fold in tonicity) into th

OBJECTIVE: To determine the referred pain pattern of the abductor pollicis longus muscle.
DESIGN: Intramuscular hypertonic saline was injected into the abductor pollicis longus of 15 healthy adults to induce muscle pain. Subjects completed pain drawings depicting the pain distribution. The drawings were transferred into the Pain Chart System for analysis.
RESULTS: Referred pain distributions were as follows: the radial aspect of the wrist (61.9%), the dorsal aspects of the third and fourth fingers (14.3%), and a combination of the two distribution patterns (23.8%).
CONCLUSION: Referred pain

Myelinolysis is a neurologic disorder that can occur after rapid correction of hyponatremia. Initially named "central pontine myelinolysis," this disease is now known to also affect extrapontine brain areas. Manifestations of myelinolysis usually evolve several days after correction of hyponatremia. Typical features are disorders of upper motor neurons, spastic quadriparesis and pseudobulbar palsy, and mental disorders ranging from mild confusion to coma. Death may occur. The motor and localizing signs of myelinolysis differ from the generalized encephalopathy that is caused by untreated hypon

Although echinococcal disease of the liver is common in some parts of the world, it is rarely seen in the United States. Even from endemic areas, few reports deal with its treatment during pregnancy. A 25 year old 18 week pregnant Italian woman presented with pruritus and right upper quadrant pain. Ultrasound revealed a cystic lesion in the left lobe of the liver. The cyst was treated by operative insertion of hypertonic saline and capsulorrhaphy. The patient and her fetus had no postoperative complications. We believe that the pregnant patient with symptomatic hepatic echinococcal disease sho

Severe electrolyte disturbances developed after the administration of hypertonic phosphate enemas in 2 chronically obstipated cats. Hyperphosphatemia, hypernatremia, and hypocalcemia were detected in both cats. Physical findings included weakness, anxiety, tachycardia, hypothermia, and dehydration. Intravenous fluid and electrolyte therapy led to prompt, dramatic improvement in both cats. Although well tolerated by most healthy animals, hypertonic phosphate enemas should be avoided in small animals, especially those that are dehydrated, severely obstipated, or suffering from renal or colonic d

Over 200 strains of green algal flagellates, representing about 100 species, were examined for their suitability as experimental organisms for studies of flagellar activity. The cells of all species shed their flagella under unfavourable conditions of temperature or pH, or in the presence of alcohols, detergents or toxic agents of various kinds. For further studies of flagellar activity, motility and autotomy (biologically induced shedding) in particular, we selected four species of Chlamydomonas: C. dysosmos Moewus, C. moewusii Gerloff, C. monoica Strehlow and C. reinhardtii Dangeard. Two patients with myxedema coma and extreme hyponatremia associated with the syndrome of inappropriate secretion of antidiuretic hormone are described. Both recovered following i.v. hypertonic saline, furosemide and thyroxine. A review of 22 cases from the literature suggests that this therapeutic approach could improve the poor prognosis in this condition.

A case of profound hyponatraemia with severe cerebral dysfunction, induced by excess water intake and exacerbated by smoking, is described. Rapid correction with hypertonic saline infusions and frusemide allowed a negative fluid balance and elevation of serum sodium concentration to be achieved without precipitating acute pulmonary oedema.

Ethylenediaminetetraacetate (EDTA) has been previously shown to cause the release of a complex of protein and lipoplysaccharide (PrLPS) from the outer membrane of Pseudomonas aeruginosa. In this present work, cells of P. aeruginosa were incubated in a solution of EDTA, phosphate buffer, and hypertonic sucrose to prepare osmoplasts (osmotically sensitive cells). Osmoplasts were able to undergo self-repair of the outer-membrane damage resulting from EDTA treatment and to regain osmotic stability when incubated in growth medium/hypertonic sucrose for 2 h in the absence of exogenous PrLPS and in t

Gut lavage by ingestion of large volumes of electrolyte solutions has been shown to be an effective method of cleansing the colon before colonoscopy, barium enema or surgery. Absorption of water and electrolytes, which might be hazardous to patients who are unable to readily excrete an additional sodium and/or water load, is prevented by addition of non-absorbable substances to the solutions, but systematic studies are lacking. We have evaluated the influence of three solutions for gut lavage with different electrolyte composition (sodium concentration 67 mmol/l and 125 mmol/l) and addition of

OBJECTIVES: To determine the efficacy and safety of intravascular volume augmentation with a hypertonic saline-hyperoncotic HES solution prior to CABG.
DESIGN: Randomized, double-blind, clinical trial.
PATIENTS: Consecutive sample of 37 patients scheduled for elective CABG; mean age 64.5 (41-80; range) years and weight 74 (51-111) kg.
INTERVENTIONS: Continuous, central-venous infusion of either 250 ml (approx. 3.5 ml/kg) HES (0.9% NaCl/10% hydroxyethyl starch 200.000/0.5) or HT-HES (7.5% NaCl/10% hydroxyethyl starch 200.000/0.5) in 15 minutes, following induction of anesthesia.

The provision of adequate nutrition to hospitalized patients with exceptional caloric requirements has been a problem until the recent advent of intravenous hyperalimentation. With total parenteral nutrition (TPN), the nutritional needs of any patient can be met by infusion. TPN solution is hypertonic, and administration requires central venous cannulation. The subclavian vein is usually chosen as route of access to the superior vena cava. Strict aseptic technique must be used in inserting the catheter and making up and administering the solution. TPN is not without risk. Infection is always a

In cases of hypovolaemic shock and trauma, hypertonic saline (HS) resuscitation is beneficial as it results in rapid and efficient fluid redistribution from interstitial and cellular sources to the intravascular compartment. The ensuing haemodilution and reduction in blood viscosity improves venous return and increases preload, while afterload is simultaneously reduced due to the vasodilatory effects of HS. All these changes promote increased cardiac output and improve haemodynamic stability. HS seems to exert a moderate negative inotropic effect on cardiac function. A fluid replacement regime

OBJECTIVE: Hyperkalemia has been reported upon different hypertonic saline infusion protocols. Since hypertonic saline test has recently been validated for the differential diagnosis of diabetes insipidus (DI), we aimed to investigate the course of plasma potassium during the test.
DESIGN: We analyzed data of 90 healthy volunteers and 141 patients with polyuria-polydipsia syndrome (PPS) from two prospective studies evaluating the hypertonic saline test. Our primary outcome was the incidence rate of hypertonic saline-induced hyperkalemia > 5 mmol/L.
METHODS: Participants received a 250 mL bol
