This study investigated the pathophysiology of preoperative hyponatremia in elderly patients with a large pituitary tumor. The tumor size, initial symptoms, and preoperative pituitary hormonal function were analyzed in 96 patients, consisting of 82 younger than 70 years old (mean age 49.7 years) and 14 older than 70 years old (mean age 72.0 years). Seventy-four patients with pituitary adenoma were treated by surgery, including 51 transsphenoidal and 23 transcranial approaches, without death, postoperative leakage of CSF and early-period intracranial infection. Many patients had a transient diabetes insipidus after operation. In discussion we attended the difference of attacking age of pituitary adenoma between male and female, the significances of CT and MRI as well as the indication and the important points of various approaches.

Neurohypophyseal function has been investigated in 11 children undergoing pituitary or suprasellar surgery. All had corticosteroid and thyroxine replacement; 9 developed diabetes insipidus (DI) within 1-12 h of operation. At the onset of DI, the plasma vasopressin (AVP) concentration was 3.9 +/- 1.2 pmol/l, considerably higher values usually associated with cranial DI (less than 0.9 pmol/l). Endogenous opioid peptides have been implicated in the cardiovascular depressant actions of clonidine.
