Emerging therapies and respiratory infections: Focus on the impact of immunosuppressants and immunotherapies.

Host defences to infection are based upon an integrated system of physical and biochemical barriers, innate and adaptive immunity. Weakness in any of these defensive elements leads to increased susceptibility to specific pathogens. Understanding how medical therapies disrupt host defences is key to the successful prevention, diagnosis and management of respiratory infection in the immunocompromised host.

EORTC/MSGERC Definitions of Invasive Fungal Diseases: Summary of Activities of the Intensive Care Unit Working Group.

The EORTC/MSGERC recently revised and updated the consensus definitions of invasive fungal disease (IFD). These definitions primarily focus on patients with cancer and stem cell or solid-organ transplant patients. They may therefore not be suitable for intensive care unit (ICU) patients. More in detail, while the definition of proven IFD applies to a broad range of hosts, the categories of probable and possible IFD were primarily designed for classical immunocompromised hosts and may therefore not be ideal for other populations. Moreover, the scope of the possible category of IFD has been dimi…

Adjuvant treatment of severe varicella pneumonia with intravenous varicella zoster virus-specific immunoglobulins.

Varicella zoster virus (VZV) pneumonia is associated with significant mortality, especially in the immunocompromised host. VZV-specific immunoglobulins (VZIG) are currently used as post-exposure prophylaxis for at-risk patients, but not as adjunctive therapy. A novel case of VZV pneumonia in an immunocompromised patient, treated successfully with intravenous VZIG in combination with acyclovir, is reported here.

Update on Legionnaires' disease: pathogenesis, epidemiology, detection and control.

Legionellosis or Legionnaires' disease is an emerging and often-fatal form of pneumonia that is most severe in elderly and immunocompromised people, an ever-increasing risk group for infection. In recent years, the genomics of Legionella spp. has significantly increased our knowledge of the pathogenesis of this disease by providing new insights into the evolution and genetic and physiological basis of Legionella-host interactions. The seventh international conference on Legionella, Legionella 2009, illustrated many recent conceptual advances in epidemiology, pathogenesis and ecology. Experts i

Diarrhea as a Presenting Symptom of Disseminated Toxoplasmosis.

Disseminated toxoplasmosis is uncommon in both immunocompetent and immunocompromised hosts with gastrointestinal involvement being rarely described. We report a case of disseminated gastrointestinal toxoplasmosis in an immunocompromised man who presented with one month of diarrhea and abdominal pain. Imaging showed thickening of the ascending colon and cecum. Esophagogastroduodenoscopy and colonoscopy biopsies revealed Toxoplasma gondii, confirmed by immunostain. Symptoms completely resolved following treatment with pyrimethamine, sulfadiazine, and leucovorin. This case highlights the importan…

Cutting edge: antibody-mediated cessation of hemotropic infection by the intraerythrocytic mouse pathogen Bartonella grahamii.

The genus Bartonella includes important human-specific and zoonotic pathogens which cause intraerythrocytic bacteremia in their mammalian reservoir host(s). It is accepted that cellular immunity plays a decisive role in the host's defense against most intracellular bacteria. Bartonella sp. infection in the immunocompetent host typically leads to immunity against homologous challenge. The basis of this immunity, be it cellular or humoral, is unclear. In this study, the course of Bartonella grahamii bacteremia in immunocompetent and immunocompromised mice was compared. In immunocompetent hosts,

The role of prolonged viral gastrointestinal infections in the development of immunodeficiency-related enteropathy.

Patients with primary immunodeficiencies are prone to develop enteropathy of unknown pathogenesis. We hypothesize that ineffective clearance of gastrointestinal pathogens, particularly viruses, in combination with defective immune regulation may cause inflammatory enteropathy in certain immunodeficient hosts. We reviewed publications related to prolonged enteric viral infection, immunodeficiency, and the subsequent development of inflammatory enteropathy. Prolonged infection with especially enteroviral infections was reported more often in immunocompromised hosts than in healthy individuals. P…

Parasitic infections in solid organ transplant.

Parasitic infections in solid organ transplant recipients are uncommon but potentially devastating. Transmission may occur through donor-der … ogic risk, microscopy, serology, molecular testing, and tissue evaluation, each with limitations in immunocompromised hosts. Prevention relies on targeted donor and recipient screening, prophylaxis when indicated, and careful post-transplant surveillance. Because delayed recognition can lead to severe disseminated disease and high mortality, a systematic risk-based appr …

Scedosporium boydii finding in an immunocompromised patient and review of the literature.

BACKGROUND: Scedosporiasis is an emerging mycosis that has gained importance in recent years due to its worldwide prevalence. It is caused by species of the Scedosporium apiospermum complex. These species can cause opportunistic infections in immunocompromised patients and, occasionally, in immunocompetent patients as well. The high intrinsic antifungal resistance make these infections difficult to manage. AIMS: The objective of this study was to interpret the mycological findings in a transplant patient, together with the images obtained in the radiological studies, in order to provide an ea

Pulmonary Immunocompromise in Chronic and High-dose Steroid Therapy.

Glucocorticoid (GC) use is often the mainstay of treatment in many pulmonary, autoimmune, allergic, and oncologic diseases, along with organ transplantation. Short-term and long-term GC use is estimated to be around 1% to 2% worldwide. It has been associated with significant infectious complications such as streptococcus and influenza, mycobacterial infections, and opportunistic infections, specifically Pneumocystis pneumonia, invasive fungal infections, and strongyloidiasis. It is important to use minimal effective dose for the shortest duration to minimize complications. This review summariz

[Progress of researches on Strongyloides stercoralis co-infection with other pathogens].

Strongyloides stercoralis is an opportunistic pathogenic parasite that can cause severe strongyloidiasis and even death among immunocompromised individuals. Previous clinical studies have reported cases co-infected with S. stercoralis and other pathogens, such as parasites, viruses, bacteria and fungi. This review summarizes strongyloidiasis patients co-infected with pathogens, and analyzes the impact of co-infection on strongyloidiasis, so as to provide insights into the reduction of the morbidity and mortality of disorders associated with S. stercoralis infections.

[Cytomegalovirus coinfection].

The rapid advancement of modern pharmacological and surgical therapeutic interventions is often accompanied by potential disruptions to the immune system, both permanent and transient. Consequently, life-threatening infectious complications may emerge, which were either absent or exceedingly rare in the past. Observational studies have identified pneumocystis and cytomegalovirus pneumonia as one of the most prevalent coinfections. These diseases carry a high risk of a fatal course, making rapid and precise diagnosis and treatment absolutely crucial. Diagnostic and therapeutic procedures for co
