Polymyositis in an immunodeficiency disease in monkeys induced by a type D retrovirus.

Fifty percent of primates with acquired immunodeficiency caused by a well-characterized type D retrovirus (SAIDS D) developed clinical, laboratory, and histologic features of polymyositis. By use of specific antisera and immunochemical techniques, we found the virus in the lymphoid cells surrounding muscle fibers and invading the endomysia septa. SAIDS D virus was isolated from the involved muscles and infected myotubes of normal muscle in tissue culture. These results suggest that retroviruses, a group of viruses increasingly associated with human diseases, can cause polymyositis with immunod

The thymus in congenital heart disease.

Some cases of thymus developmental defects (DiGeorge's syndrome) are associated with the development of defects of the heart and great vessels. To see if anomalies of the heart are also associated with anomalies of the thymus, a material of thymus tissue removed during operation for congenital heart disease was compared with thymus tissue from a forensic autopsy material representing the same age group, but without cardiac lesions. Among 27 cases of congenital heart disease, occurrence in the thymus of solid epithelial cords were seen in 6 and cyst- or duct-like structures were found in 7. Amo

Serum immunoglobulins in the nephrotic syndrome. A possible cause of minimal-change nephrotic syndrome.

To assess immunologic factors in the pathogenesis of idiopathic minimal-change nephrotic syndrome (INS), serum immunoglobulin concentrations were measured in 37 children with this syndrome and compared with those found in 36 with nephrotic syndrome secondary to chronic glomerulonephritis (CGN). Serum IgG and IgA levels were significantly reduced in nephrotic patients with either INS or CGN, IgG averaging 18.5 and 25.9 per cent of normal (P less than 0.001) and IgA 59.8 and 44.1 per cent of normal (P less than 0.01) respectively. Values increased after treatment of INS with prednisone, but mean

Fulminant autoimmune hepatitis in a girl with 22q13 deletion syndrome: a previously unreported association.

We report a 7-year-old girl with 22q13 deletion syndrome, 46,XX,Ish del(22)(q13.3)(ARSA-; D22S1726), who developed a fulminant autoimmune hepatitis requiring orthotopic liver transplantation. Recently, it has been suggested that the Shank3 gene product, whose deficiency is responsible for the features observed in this syndrome, could play a role in immunological response. Despite an increased incidence of respiratory infections, autoimmune diseases have thus far not been reported in patients with this syndrome. This is the first case of fulminant autoimmune hepatitis associated with the 22q13

Study of autoimmunity in Klinefelter's syndrome and idiopathic hypogonadotropic hypogonadism.

Sex hormones play an important role in determining the progression and severity of autoimmune diseases, but the in vivo mechanisms underlying this relation are poorly understood. The main objective of current study has been to compare the changes in neuroendocrine immune features and autoantibody profile in male patients with hypogonadotropic and hypergonadotropic hypogonadism, and to determine the relationships between sex hormones and immunologic parameters. Thirty-seven male patients with Klinefelter's syndrome and 35 men with idiopathic hypogonadotropic hypogonadism who had no history of p

Otitis Media and Inborn Errors of Immunity.

PURPOSE OF REVIEW: The aim of this review is as follows: (1) to present the role of otitis as a warning sign for inborn errors of immunity (IEI), (2) to establish which patients presenting otitis should be investigated for IEI, (3) to review data about main IEI associated with otitis-prone patients. RECENT FINDINGS: Otitis media is a very common infection in general population. The concept of otitis-prone children established a certain frequency of the infections in order to look for conditions leading to them. The confirmation of middle ear impairment by specialists has demonstrated better c

[Clinical and immunological criteria for predicting the course of uveitis].

Clinical and immunological parameters are studied in 205 patients with uveites of different etiology in order to define the criteria permitting the prediction of uveitis course and relapses. The prognosis is unfavorable for autoimmune conditions, including peripheral, systemic, syndromal, tuberculous uveites and mixed infections. Clinical cure is unstable in diffuse inflammatory changes, persistent cellular infiltration of the vitreous body, formation of cyclic membranes in it, in cataract complications, and exudative hemorrhagic chorioretinitis. High titers of antibodies to tissue antigens an

Chronic diarrhea associated with thymoma and hypogammaglobulinemia (Good's syndrome).

We report the case of a patient with severe diarrhea and malabsorption who was subsequently found to have hypogammaglobulinemia and thymoma (Good's syndrome). The mechanism by which hypogammaglobulinemia and/or thymoma causes diarrhea is unclear. It may be related to malabsorption caused by a mucosal lesion resembling villous atrophy, which may resolve with restoration of immunologic status. Diarrhea in some patients may respond to commercial gamma globulin injections, fresh frozen plasma, or cholestyramine therapy. The etiologic relationship between thymoma and acquired hypogammaglobulinemia

Lymphocyte subpopulations in allergic granulomatosis and angiitis (Churg-Strauss syndrome).

We report a case of allergic granulomatosis and angiitis (Churg-Strauss syndrome) in which immunologic parameters, including lymphocyte subpopulations, were determined in the acute phase of the disease and during remission. Hyperimmunoglobulinemia E and immune complexes were present. A low proportion of suppressor/cytotoxic (T8+) lymphocytes and a high helper/suppressor ratio were seen throughout the course of the disease, although immunoglobulin levels and circulating immune complex levels decreased with therapy. We hypothesize that the deficiency of suppressor cells may play a role in the pa

The hypercoagulable state. Who, how, and when to test and treat.

Patients with a family history of thrombosis, early-onset or recurring thrombosis, thrombosis at unusual sites, or warfarin-induced skin necrosis should be investigated for a possible underlying inherited hypercoagulable disorder. These include AT-III deficiency, protein C and S deficiencies, and APC resistance. Many patients should also be evaluated for the antiphospholipid syndrome, an acquired disorder. Functional assays are more useful than immunologic assays for diagnosing AT-III deficiency, protein C and S deficiencies, and APC resistance. A molecular probe is now available for the abnor

AIDS and the gut.

The epidemiological, immunological and early virological observations on the acquired immune deficiency syndrome (AIDS) suggested that an agent was involved which was sexually, parenterally and perinatally transmitted and perhaps tropic for T helper lymphocytes. A new subgroup of human T lymphotropic retroviruses have been identified ans seroepidemiological studies suggest that they are aetiologically related to AIDS. The syndrome is characterised by the development of tumors: such as Kaposi's sarcoma and non-Hodgkins lymphoma, with an aggressive clinical course and infection by a wide spectru

[Immunological investigation of hypogammaglobulinemia].

Immunoglobulin deficiency syndromes should be considered in patients with recurrent respiratory tract infections or in patients with infections caused by agents of low virulence. We present two patients with a clinical history of recurrent respiratory tract infections. Both patients had a humoral immunodeficiency characterised as common variable immunodeficiency (CVID). We give a brief survey of CVID with emphasis on the immunological investigation of hypogamma-globulinaemia. Analysis of serum, immunoglobulins should be performed in patients with recurrent infections Patients with humoral immu

[Primary lymphoma of the central nervous system in a patient with AIDS].

The increasing incidence of primary brain lymphoma appears to be real in immunologically normal people. However, lymphoma of the central nervous system has a much higher incidence in patients with immune deficient status as in renal and cardiac transplants, patients with IgA deficiency, or Wiskott-Aldrich syndrome. Thus, the increased predisposition of AIDS patients to brain lymphoma is not surprising, this being the second most frequent cause among cerebral mass lesions in the AIDS population. We report a case of a 36 year old white woman with AIDS and a cerebral mass lesion which we demonstr

Lessons from the American College of Allergy, Asthma and Immunology inborn errors of immunity survey: Advancing diagnostic and therapeutic strategies for the practicing allergist-immunologist.

BACKGROUND: Inborn errors of immunity (IEIs) have been increasingly well characterized on the molecular and genetic levels. Their clinical recognition and management among practicing allergist-immunologists remain challenging. OBJECTIVE: To evaluate the consultative practices of allergist-immunologists in diagnosing and managing IEIs by identifying subtypes encountered, current approaches, multidisciplinary collaboration, barriers, and educational needs. METHODS: The American College of Allergy, Asthma and Immunology distributed a 30-question web-based survey assessing IEI experience through

Repurposing eravacycline as an immunomodulator for immunodeficiencies driven by anti-IFN-γ autoantibodies.

INTRODUCTION: Adult-onset immunodeficiency (AOID) driven by pathogenic anti-interferon-γ (IFN-γ) autoantibodies (AIGA) mimics inborn errors of IFN-γ signaling, causing life-threatening nontuberculous mycobacterial and fungal infections. Current B-cell depletion or prolonged antimicrobials fail to directly reverse autoantibody-mediated IFN-γ blockade. We hypothesized that small molecules binding AIGA-targeted IFN-γ epitopes could sterically disrupt pathogenic autoantibody-cytokine complexes and restore JAK-STAT1 signaling. METHODS: We conducted structure-based high-throughput virtual screening

Decreased vitamin A levels in common variable immunodeficiency: vitamin A supplementation in vivo enhances immunoglobulin production and downregulates inflammatory responses.

BACKGROUND: Vitamin A has a broad range of immunological effects, and vitamin A deficiency is associated with recurrent infections. Common variable immunodeficiency (CVI) is a group of B-cell deficiency syndromes with impaired antibody production and recurrent bacterial infections as the major manifestations, but the immunological dysfunctions may also include T cells and macrophages. In the present study we examined the possible role of vitamin A deficiency in CVI. PATIENTS AND METHODS: We analysed plasma vitamin A levels in 20 CVI patients and 16 controls, and examined the relationships bet
