Individualized dynamic risk assessment for multiple myeloma.

BACKGROUND: Individualized treatment decisions for patients with multiple myeloma (MM) requires accurate risk stratification that takes into account patient-specific consequences of genetic abnormalities and tumor microenvironment on disease outcome and therapy responsiveness. METHODS: Previously, SYstems Genetic Network AnaLysis (SYGNAL) of multi-omics tumor profiles from 881 MM patients generated the mmSYGNAL network, which uncovered different causal and mechanistic drivers of genetic programs associated with disease progression across MM subtypes. Here, we have trained a machine learning (

Variability in individual native fibrin fiber mechanics.

Fibrin fibers are important structural elements in blood coagulation. They form a mesh network that acts as a scaffold and imparts mechanical strength to the clot. A review of published work measuring the mechanics of fibrin fibers reveals a range of values for fiber extensibility. This study investigates fibrinogen concentration as a variable responsible for variability in fibrin mechanics. It expands previous work to describe the modulus, strain hardening, extensibility, and the force required for fiber failure when fibers are formed with different fibrinogen concentrations using lateral for

Mortality and Causes of Death Among Individuals With Keratoconus.

PURPOSE: The purpose of this study was to determine whether there is an increased risk of mortality among individuals with keratoconus. METHODS: This was a retrospective, case-control study using the Utah Population Database. Cases were defined as individuals diagnosed with keratoconus from 1996 to 2020 and were matched 5:1 with controls on birth year, sex, whether born in Utah, and follow-up time in Utah. Individuals diagnosed with trisomy 21 or connective tissue disease were excluded. Main outcome measures were the all-cause and cause-specific mortality among keratoconus cases compared with

[Complex optical systems for individualized correction of presbyopia].

A multitude of available intraocular lens (IOL) models enable a personalized approach to presbyopia correction in order to meet each patient's needs. This review article discusses more complex approaches which can be useful in selected cases. The concept of reversible trifocality enables correction of presbyopia using a supplementary intraocular lens (IOL), which can be removed if necessary (e.g., intolerance to multifocal optics). The use of capsulotomy-fixated IOLs enables high precision for positioning of the lens and better stability compared to conventional capsular bag-fixated IOLs, whic

Reasons for Hospital Admission in Individuals With Multiple Sclerosis.

BackgroundHealth care utilization is higher in individuals with multiple sclerosis (MS) than in the general population. However, there are limited data on the reasons for their hospital admissions. Our primary objective is to analyze the reasons for the hospitalization of individuals with MS with the goal of identifying preventable causes.MethodsWe conducted a retrospective analysis of a cohort of adults with a confirmed diagnosis of MS admitted to Duke University Hospital between January 2018 and January 2020. This yielded a cohort of 210 individuals. Data were analyzed using descriptive stat

The Individual as Moderator of Variable Relations - A Configural Approach.

Moderators are variables that change the relations among other variables. Moderators are variables that are substantive just as the variables whose relations are moderated. In the present article, we propose using individuals as moderators. Specifically, we propose using Configural Frequency Analysis, that is, investigating moderators from a person-oriented perspective. The question asked is whether variable relations vary across individuals. Base models are specified for Configural Frequency Analysis that allow one to identify individuals that differ in variable relations. In a data example,

Affective predictors of social support in individuals with schizophrenia.

BACKGROUND: Social Support has been found to contribute to lower mental illness burden, higher treatment adherence, enhanced social functioning, and better quality of life. Individuals with schizophrenia report lower social support compared to non-clinical populations, yet the factors contributing to this discrepancy are not fully clear. Specifically, the person-related variables that may enhance or hinder SS in people with schizophrenia are unknown. Evidence from affective neuroscience research has implicated emotion awareness and regulation as key predictors of social functioning, a correlat

The effect of exercise on daytime sleepiness in healthy individuals.

The effects of exercise on daytime sleepiness remain unclear, with conflicting findings in the literature. We reviewed the existing literature on the relationship between exercise and daytime sleepiness in healthy individuals. We conducted a systematic search of PubMed and Google Scholar (1991 to present) for interventional studies that used the Epworth Sleepiness Scale (ESS) to measure change in self-reported degree of sleepiness before and after an exercise regimen. Seven studies were included in the review. Exercise significantly improved self-reported sleepiness after the intervention, as

Analysis of veering during gait in blind individuals.

BACKGROUND: Veering occurs during blind walking, which causes individuals to deviate from crosswalks or fall off platforms. Despite being linked to static postural control, the relationship between veering and gait function (gait variability and plantar pressure), which is presumed to be related to dynamic postural control, has yet to be fully understood. RESEARCH QUESTION: How do gait variability and plantar pressure affect veering? METHODS: This experiment involved a walking task in which 11 blind adults were instructed to walk along a straight path using a white cane. We measured the veer

Hemispheric Asymmetries of Individual Differences in Functional Connectivity.

Resting-state fMRI studies have revealed that individuals exhibit stable, functionally meaningful divergences in large-scale network organization. The locations with strongest deviations (called network "variants") have a characteristic spatial distribution, with qualitative evidence from prior reports suggesting that this distribution differs across hemispheres. Hemispheric asymmetries can inform us on constraints guiding the development of these idiosyncratic regions. Here, we used data from the Human Connectome Project to systematically investigate hemispheric differences in network variant

Heterogeneous Comorbidity in Individuals With Different Phenotypes of Obesity.

Introduction The prevalence of obesity is steadily increasing worldwide. Obesity is one of the most potent risk factors for various diseases and is simultaneously a heterogeneous condition. Different types of obesity could be identified according to body mass index (BMI), waist circumference, and visceral fat level; these conditions may present individually or in combination and pose a risk of developing certain comorbidities. However, the current obesity classification systems do not allow for accurate diagnosis and prediction of the comorbidity risk of patients, which is crucial for their cl
