Understanding the decline in the autopsy rate can be furthered through analysis of Foucault's idea of the medical gaze and the ancient Greek idea of theoria. The medical gaze has shifted over time from the surface of the body to the inner organs to the cellular and subcellular levels. Physicians and loved ones of the deceased person are not likely to "gaze" at the same levels. Patients' loved ones might not theorize as physicians do; they have different interests, which suggest the need for more attention to informed consent for autopsies. Responding to this need should take priority over effo…

Genomic research results and incidental findings with health implications for a research participant are of potential interest not only to the participant, but also to the participant's family. Yet investigators lack guidance on return of results to relatives, including after the participant's death. In this paper, a national working group offers consensus analysis and recommendations, including an ethical framework to guide investigators in managing this challenging issue, before and after the participant's death.

Like all other areas of transplantation, vascularized composite allografts (VCA) has the capacity to transform the lives of patients, for the better or for the worse. It is this duality that mandates VCA be performed in centers prepared for the intricacies accompanying other transplant procedures. Similarly, the complexities of VCA require that the procedures be driven by surgeons and physicians with experience in the multidisciplinary management of immunocompromised postsurgical patients. Furthermore, the grafts should be considered as organs rather than tissues from a regulatory and a biolog

A recent French law, passed on July 4th, 2001, authorizes a person of age to voluntarily undergo a sterilisation on her request. The practitioner has the legal obligation to inform the person of the consequences and the possible complications of the operation. Recent technical progress as well as its assessment on a large epidemiological scale has modified the given data. In this perspective, we are presenting the different and current techniques of tubal sterilisation, their efficiency and their immediate and long-term complications. During the consultation preceding the operation, the doctor

UNLABELLED: Ectopic pregnancy is a implantation occurring elsewhere than in the cavity of the uterus, whereas ninety-nine percent of extrauterine pregnancies occur in the fallopian tube. The incidence of extrauterine pregnancy has increased from 0.5% thirty years ago, to a present day 1-2%. The most frequent cause of tubal pregnancy is previous salpingitis. Mortality rates for tubal pregnancies used to be approximately 1.7% in the 1970s but dropped to 0.3% in 1980s.
DIAGNOSIS: Using transvaginal ultrasound it is possible to obtain positive evidence of an ectopic pregnancy at a very early stag

The nurse is never "too prepared" when caring for the multiple organ donor. Some the many complications encountered include tachycardia, hyper/hypotension, hyper/hypothermia, diabetes insipidus, critical fluid and electrolyte imbalances, and hypoxia these are a result of brain stem herniation where the medulla herniates into the foramen magnum eventually leading to decreased cardiac output, impaired gas exchange, and impaired tissue perfusion if managed inappropriately. The challenge is on! Drug and fluid resuscitation have firm limits and guidelines which are of great importance to retrieval

INTRODUCTION: Guidelines on consent for anaesthesia were first issued by the Association of Anaesthetists in 1999 and last revised in 2017. These new guidelines respond to the changing ethical, legal and clinical landscape in which anaesthetists practice. Since 2017, the Montgomery judgment has changed medical consent practice. The General Medical Council standards on decision-making and consent and the National Institute for Health and Care Excellence guidance on shared decision-making also reinforce the need for high standards of patient advice and counselling.

BACKGROUND: Palliative care research must be inclusive of those with impairments in decision-making to provide a representative evidence base for care and treatment. When capacity to consent to research is impaired, proxies may be asked to support decision-making. However, little is known about how proxies navigate this role or what supports their involvement.
AIM: To identify, describe, and synthesise empirical evidence on the use of proxies to support the involvement of adults with impaired mental capacity in palliative care research.
DESIGN: A scoping review conducted and reported followi

Five years after changing from opt-in to opt-out consent systems in England and the Netherlands, we examine the impact on donor registrations, consent rates, donor numbers, and reflect on lessons learned. Opt-out legislation was implemented in May 2020 in England and July 2020 in the Netherlands. In England, 44% of the population is now registered; 40% recording an opt-in decision and 4% an opt-out decision. In the Netherlands, mandated donor registration resulted in the entire adult population having their decision recorded: "Yes, I want to be a donor" (34%), "No objection" (24%), or "No, I d

BackgroundInformed choice is of the highest importance in health care. However, confusion and challenges remain toward how it is conceptualized and measured.PurposeThis umbrella review aimed to establish how informed choice is operationalized in health care and the characteristics and performance of the most commonly used measurement instruments.Data SourcesFour electronic databases (Ovid MEDLINE, Ovid EMBASE, APA PsycINFO, and Cochrane Library) were searched up to January 29, 2024. Reference lists of included studies were hand searched for further relevant publications.Study SelectionAfter th
