Effect of Interlaminar Epidural Steroid Injection in Patients with Central Cervical Spinal Stenosis.

OBJECTIVE: To investigate the effect of interlaminar epidural steroid injection (ESI) for management of central cervical spinal stenosis-induced posterior neck pain. METHODS: This retrospective study recruited 45 patients with posterior neck pain caused by central cervical spinal stenosis. For the ESI procedure, a 21-gauge Touhy needle was inserted into the epidural space between C7 and T1, and 20 mg (40 mg/mL) of dexamethasone with 4 mL of normal saline was injected. The effect of the procedure was evaluated using a numerical rating scale at 1, 2, and 3 months after the procedure. Successful

Outcome of Transforaminal Epidural Steroid Injection According to Severity of Cervical Foraminal Stenosis.

BACKGROUND: Cervical transforaminal epidural steroid injection (TFESI) is considered one of the most effective treatments for radicular pain induced by cervical foraminal stenosis. PURPOSE AND DESIGN: We sought to evaluate the treatment outcome of TFESI according to the severity of cervical foraminal stenosis. In our retrospective study there were 53 patients with cervical radiculopathy due to cervical foraminal stenosis. OUTCOME MEASURES: Pain intensity was evaluated using a numeric rating scale (NRS) at pretreatment, 2 weeks, and 1, 2, and 3 months after TFESI. METHODS: Patients were divi

Time-dependent changes in epidural catheter aspirate after injection of a local anesthetic.

STUDY OBJECTIVE: A glucose check is used for investigation of a suspected accidental dural puncture in epidural anesthesia. However, glucose-positive clear fluid is sometimes aspirated from an epidural catheter in cases without clinical evidence of puncture. The goal of the study was to investigate time-dependent changes in the aspirate composition after injection of a local anesthetic into the epidural space. DESIGN: Observational study. SETTING: Operating rooms at Hamamatsu University Hospital. PATIENTS: The subjects were 30 patients (ASA I or II) undergoing surgery with combined epidural

The effects of a forceful transforaminal epidural steroid injection on radicular pain: a preliminary study.

BACKGROUND: Lumbar transforaminal epidural steroid injections (TFESIs) are performed to provide symptom relief in patients with radicular pain. Recent articles suggested that injected volume itself have analgesic effects and higher volumes are associated with better outcomes. To date, few studies have been conducted to investigate the effects of volume. Therefore, well-designed controlled studies were necessary to confirm the effect of volume itself on pain relief. The purpose of this study was to examine the effectiveness of a forceful saline injection on lumbar TFESI using non-particulate st

Whole cerebrospinal axis infection after lumbar epidural injection: a case report.

PURPOSE: Lumbar epidural injection is a popular treatment for degenerative lumbar disease. Although post-procedural epidural infection is rare, meningitis and epidural abscess are life-threatening conditions, and need additional medical and surgical intervention. The purpose of this article is to report a patient with fatal whole cerebrospinal axis infection after lumbar epidural injections. METHODS: A 64-year-old female patient presented with septic shock and quadriparesis. In the past, this patient had received lumbar epidural injections several times for degenerative spondylolisthesis at L

Correlation between severity of lumbar spinal stenosis and lumbar epidural steroid injection.

OBJECTIVE: Lumbar spinal stenosis (LSS) is a narrowing of the spinal canal that causes mechanical compression of the spinal nerve roots. The compression of these nerve roots can cause leg pain, as well as neurogenic claudication. Lumbar epidural steroid injections have commonly been used in patients with LSS. The aim of our study was to determine the relationship between the severity of LSS using a grading system (grade 1 = mild stenosis with separation of all cauda equina; grade 2 = moderate stenosis with some cauda equina aggregated; grade 3 = severe stenosis with none of the cauda equina se

Motor Weakness after Caudal Epidural Injection Using the Air-acceptance Test.

Air injected into the epidural space may spread along the nerves of the paravertebral space. Depending on the location of the air, neurologic complications such as multiradicular syndrome, lumbar root compression, and even paraplegia may occur. However, cases of motor weakness caused by air bubbles after caudal epidural injection are rare. A 44-year-old female patient received a caudal epidural injection after an air-acceptance test. Four hours later, she complained of motor weakness in the right lower extremity and numbness of the S1 dermatome. Magnetic resonance imaging showed no anomalies o

Cauda equina syndrome after spinal epidural steroid injection into an unrecognized paraganglioma.

OBJECTIVE: Clinically significant spinal hemorrhage is an extremely rare but potentially devastating complication of spinal epidural steroid injection. We report a rare case of cauda equina syndrome after spinal epidural injection that inadvertently penetrated an unrecognized spinal paraganglioma. METHODS: The clinical records for a patient presenting with cauda equina syndrome were retrospectively reviewed. A literature search was performed to identify reports of cauda equina syndrome in patients undergoing spinal epidural steroid injection, as well as recent large series describing complica

The risks of epidural and transforaminal steroid injections in the Spine: Commentary and a comprehensive review of the literature.

BACKGROUND: Multiple type of spinal injections, whether epidural/translaminar or transforaminal, facet injections, are offered to patients with/without surgical spinal lesions by pain management specialists (radiologists, physiatrists, and anesthesiologists). Although not approved by the Food and Drug Administration (FDA), injections are being performed with an increased frequency (160%), are typically short-acting and ineffective over the longer-term, while exposing patients to major risks/complications. METHODS: For many patients with spinal pain alone and no surgical lesions, the "success"

[Assessment of the effectiveness of lumbar transforaminal epidural steroid injection for low back pain].

OBJECTIVES: Transforaminal epidural steroid injection (TFESI) is a minimally invasive technique used for low back pain. We aimed to assess the efficiency of TFESI in patients with low back pain who did not respond to conservative or surgical methods. METHODS: This study was a retrospective analysis of the records of 37 patients (range: 24-80 years) who presented to our clinic with low back pain and were treated by TFESI. Outcomes were assessed by Visual Analog Scale (VAS) measured at the 3rd week and 6th month. RESULTS: Median age of the patients was 50 years, and mean duration of symptoms w

Lumbosacral epidural abscess as a complication of post-injection phlegmon of the forearm. Case report.

We present the case of a 50-year-old male with lumbosacral epidural abscess as a distant iatrogenic sequella of a post-injection phlegmon of the forearm. The progressive and inconspicuous course of the illness, its complex clinical picture, neurologic manifestation in the form of cauda equina syndrome, and multidisciplinary approach to diagnosis and treatment in this patient make this case worthy of particular attention. We decided on a combined therapy consisting of surgery and pathogen-directed antibiotic treatment. Continuous follow-up confirmed therapeutic success and restoration of a norm

MRI prediction of therapeutic response to epidural steroid injection in patients with cervical radiculopathy.

OBJECTIVE: To analyze whether magnetic resonance imaging (MRI) findings can be used to predict therapeutic responses to cervical epidural steroid injections (CESI) in patients with cervical radiculopathy. DESIGN: Retrospective single-center study. Patients were categorized by presence or absence of four types of cervical MRI findings (disk herniation, nerve root compromise, neuroforaminal stenosis, and central canal stenosis). Thus, for each of the four types of MRI findings, there was a positive-MRI arm and a negative-MRI arm. Severities of these MRI findings were also noted. The treatment o

Vertebral osteomyelitis: a potentially catastrophic outcome after lumbar epidural steroid injection.

BACKGROUND AND OBJECTIVE: Epidural steroid injections are commonly used to palliate the symptoms of spinal stenosis. Deep tissue infection is a known potential complication of these injections. There have been no previous published cases of osteomyelitis without epidural abscess after such injections. We present a case in an elderly patient who presented only with persistent axial low back pain following a lumbar epidural steroid injection (LESI). We emphasize early patient evaluation, consideration of infectious predisposing factors, sterile technique, and skin disinfectant. DESIGN: Open-lab

Sciatica: a review of history, epidemiology, pathogenesis, and the role of epidural steroid injection in management.

Radicular pain in the distribution of the sciatic nerve, resulting from herniation of one or more lumbar intervertebral discs, is a frequent and often debilitating event. The lifetime incidence of this condition is estimated to be between 13% and 40%. Fortunately, the majority of cases resolve spontaneously with simple analgesia and physiotherapy. However, the condition has the potential to become chronic and intractable, with major socio-economic implications. This review discusses the history, epidemiology, pathophysiology, and natural history of sciatica. A Medline search was performed to o

Outcome assessment after epidural corticosteroid injection for low back pain and sciatica.

Epidural corticosteroid injection is a relatively effective treatment for low back pain and sciatica with a low incidence of side effects, although the patient's perspective in terms of outcome and satisfaction has not been studied well. The assessment of outcome in low back pain patients is complex. Three types of measure were compared: 1) a clinical score, 2) patient ratings of pain and disability, and 3) patient satisfaction ratings. There were significant discrepancies among the results. Consequently, clinical trials using different outcome measures may reach different conclusions. It ther
