The arterial anatomy of the posterior duodenum - A computed tomography angiography analysis with implications for abdominal surgery.

The main objective of the present study was to clarify the anatomical characteristics of the GDA. In order to fulfill this objective, novel classification systems of both the origin and branching pattern of the said vessel were created. Being aware of the variable anatomy of the GDA is of great importance when performing hepatopancreaticobilliary procedures. The results of 75 consecutive patients who underwent abdomen computed tomography angiography (CTA) were analyzed. A total of 74 GDA were analyzed. Of these, 42 were from women (56.8%) and 32 were from men (43.2%). The most common direction

Dual-view cross-semantic graph neural network for predicting intraoperative complications in patients with acute myocardial infarction undergoing percutaneous coronary intervention.

For acute myocardial infarction (AMI) patients undergoing percutaneous coronary intervention (PCI), accurate prediction of intraoperative complications (IOCs) in AMI patients undergoing PCI is critical for timely risk stratification and clinical intervention. However, existing approaches lack a patient-indicator dual-view synergy modeling framework, leading to semantic isolation between patient and indicator representations. Yet current risk prediction approaches often treat patient characteristics and clinical indicators as separate, disconnected pieces of information. What's missing is a uni

Intraoperative Discovery of Type II Choledochal Cyst, Partial Annular Pancreas, and Duodenal Stenosis in a 45-Day-Old Term Born Female Infant.

Type II choledochal cysts (CCs), a rare form of congenital biliary dilation characterized as a true diverticulum along the extrahepatic duct, comprise less than 2% of all cases. The coexistence of CC, annular pancreas (AP), and duodenal stenosis (DS) has been scarcely reported. We present the case of a 45-day-old female with a rare combination of congenital upper gastrointestinal anomalies discovered intraoperatively after imaging identified malrotation with concern for possible volvulus. The patient underwent exploratory laparotomy revealing a Type II CC, partial AP, and DS. Management includ

Infratentorial neuroparacoccidioidomycosis: A case report.

BACKGROUND: Neurological manifestations of paracoccidioidomycosis are uncommon, and the clinical presentation can include a variety of symptoms that depends specially on the lesion's site. CASE DESCRIPTION: This article reports the case of a 63-year-old rural worker who presented with progressive imbalance and diplopia. Magnetic resonance imaging suggested a likely inflammatory/infectious etiology. The diagnosis was confirmed by the intraoperative frozen section and direct microscopic examination. Treatment started with amphotericin B and, later on, with itraconazole due to decreased renal fu

Mature Teratoma Arising in the Fallopian Tube.

Mature teratoma of the fallopian tube is extremely rare. Most cases are incidentally diagnosed as originating from the fallopian tube following cesarean section, diagnostic laparoscopy, or examination of a surgical specimen. Here, we present a case of mature teratoma of the fallopian tube identified during laparoscopic surgery. A 5-cm mature teratoma was detected in the pelvic cavity of a 30-year-old Filipino female using transvaginal ultrasonography, computed tomography, and magnetic resonance imaging (MRI). Laparoscopy revealed a cystic tumor arising from the isthmus of the right fallopian t

Patient in Mayfield Head Pins Requiring Prone Bronchoscopy for Mucus Plug Obstruction: A Case Report.

Mucus plug obstruction is a common complication in prone patients associated with loss of ventilation and hemodynamic instability. This case presents a 62-year-old female with chronic bronchitis who underwent posterior cervical fusion for a type III dens fracture with extension into the pars articularis and pedicles. Glycopyrrolate was administered to assist with fiberoptic intubation. After successful intubation, bronchoscopy revealed copious endotracheal secretions requiring preoperative therapeutic removal. Despite extensive removal of thick endotracheal secretions preoperatively, obstructi

Right basal bronchial fistula due to amebic infection: a case report.

BACKGROUND: Pleuropulmonary amebiasis is the second most common form of extraintestinal invasive amebiasis, but cases that include bronchopleural fistula are rare. CASE PRESENTATION: A 43-year-old male was referred to our hospital for liver abscess, right pleural effusion, and body weight loss. He was diagnosed with a bronchopleural fistula caused by invasive pleuropulmonary amebiasis and human immunodeficiency virus (HIV) infection. After initial medical treatment for HIV infection and invasive amebiasis, he underwent pulmonary resection of the invaded lobe. Intraoperative inspection reveale

Two cases of an internal hernia after laparoscopic sigmoid resection.

Two patients with a laparoscopic resection of the sigmoid colon in their surgical history were diagnosed with an internal hernia because of a mesenteric gap underneath the descending neocolon. While the first case demonstrated a strongly symptomatic patient with a closed-loop obstruction of the small bowel, the second case was less obvious and correct diagnosis was achieved after a few weeks by finally performing a diagnostic laparoscopy. Since internal hernias after laparoscopic sigmoid resection are a rare complication and as presented in the second case can display very unspecific symptoms,

Right diaphragm metastasis of endometrial cancer: a case report.

A diaphragmatic tumor is usually caused by metastasis from lung cancer, malignant mesothelioma, and malignant thymoma. Endometrial cancer is rarely involved in metastasis to the diaphragm. A right anterior mediastinal tumor was found in a 60-year-old woman who was initially diagnosed with endometrial carcinoma. There was initially no relationship between the right anterior mediastinal tumor and endometrial carcinoma. Radical curative surgery was performed for endometrial carcinoma. The endometrial carcinoma stage was IA. The patient was admitted to the Department of Thoracic Surgery 6 months a

Acupuncture-Induced Cranial Epidural Abscess: Case Report and Review of the Literature.

BACKGROUND: Acupuncture is a common form of alternative medicine that is used for pain control among other modalities of treatment. It is a relatively safe procedure, but complications, including those of infectious etiology, may still occur. CASE DESCRIPTION: A 47-year-old immunosuppressed woman presented with fever, altered level of consciousness, dysphasia, and a left occipital subgaleal fluctuant mass after acupuncture for headaches in the same area. Imaging demonstrated subgaleal and epidural collection localized in the left occipital region. She underwent urgent surgical evacuation of b

Aberrant positioning of the ulnar nerve found during carpal tunnel release: A case report.

Aberrant positioning of the ulnar nerve volar to the carpal tunnel is a rare anatomic variation. We present the case of a 55-yearold female with unique ulnar nerve anatomy that was discovered intraoperatively during carpal tunnel release. The ulnar nerve was running directly adjacent to the median nerve in the distal forearm and as the median nerve traversed dorsal to the transverse carpal ligament (flexor retinaculum) to enter the carpal tunnel the ulnar nerve continued directly volar to this structure before angling towards Guyon's Canal. The unique ulnar nerve anatomy was successfully ident

Acute Presentation of Lumbar Spinal Schwannoma Due to Torsion: A Case Report.

Although schwannomas are common spinal tumors with insidious presentations, acute neurological deterioration is an extremely rare manifestation that can occur in the setting of tumor torsion and infarction. The present case reports an unusual presentation of a spinal schwannoma that underwent torsion and infarction. A 65-year-old male presented initially with acute radicular pain progressing to cauda equina syndrome and confusion. MRI of the lumbar spine revealed an intradural extramedullary lesion at the level of L1/L2 measuring 1.1x0.9 cm. Intraoperatively, a reddish mass was seen caudally t

Laparoscopic resection of a bulky primary adrenal hydatid cyst.

A 55-year-old man with no medical history of relevance was referred to our department for chronic back pain. Plain radiographs of the abdomen showed a round calcified image with liquid level inside in the upper-left quadrant. On computed tomography (CT) a bulky solid-cystic mass with calcified wall of 65 × 34 mm was shown in the left adrenal gland with no-contrast enhancement. The patient underwent a laparoscopic approach. Intraoperatively there was a large hard stony mass depending on the left adrenal gland, which was entirely supplanted by the tumor. Complete excision was done. The patient u

A microscopically calcified Meckel's diverticulum: a histopathological perspective of a case of both gastric and pancreatic mucosae.

A 2-year-old child presented with a 1-week history of abdominal pain and non-bilious vomiting. Upon examination, the abdomen was distended and faecal aspirate was noted from a nasogastric tube. Ultrasound scan and a failed air enema demonstrated intestinal intussusception warranting a surgical intervention. The intraoperative findings were of an ileocolic intussusception that was reduced and a Meckel's diverticulum (MD) was noted as a lead point necessitating resection with an end-to-end anastomosis. Histopathological analysis revealed a heterotopic MD containing both gastric and pancreatic mu

Concomitant inverted papilloma and fungus ball in unilateral maxillary sinus.

PROBLEM: The concomitant appearance of an inverted papilloma and a fungus ball in unilateral maxillary sinus is rare. These disease entities may be difficult to distinguish before surgery. METHODOLOGY: A male patient presented with the characteristic symptoms of chronic rhinosinusitis. A preoperative sinus computed tomography scan revealed unilateral sinus opacification, hyperdense calcified spots, and bony erosion of the medial maxillary sinus wall. RESULTS: During the operation, a cheesy, clay-like fungus ball was removed. In addition, a papillary appearance of the sinus mucosa led to the

Double duct-to-mucosa pancreaticojejunostomy for bifid pancreatic duct.

We report a pancreaticojejunostomy with double duct-to-mucosa anastomotic technique after pyloruspreserving pancreaticoduodenectomy for chronic pancreatitis with bifid pancreatic duct. A 49-year-old Japanese man was diagnosed preoperatively as having chronic pancreatitis with common bile duct stricture and pseudocyst of the pancreatic head. In a pancreaticoduodenectomy, the main pancreatic duct (7mm in diameter) and a secondary pancreatic duct (4mm in diameter) were identified intraoperatively at the transected surface. Pancreatography showed the main pancreatic duct as well as thesecondary pa

Residual unabsorbed midazolam: a case report.

BACKGROUND: Oral midazolam is commonly administered to reduce anxiety in children presenting for medical procedures or surgery. It is unclear what volume of medication remains unabsorbed in the stomach when the child presents for anesthetic induction prior to these procedures. The presence of any significant residual medication in the stomach has significant clinical implications in the postoperative period. CASE PRESENTATION: A 5-year-old white Caucasian boy presented for upper gastrointestinal endoscopy after receiving oral midazolam liquid. Insertion of the endoscope into the stomach revea

Appendiceal duplication an unusual cause of abdominal pain: A case report.

Appendiceal duplication is exceedingly rare, with a prevalence of 0.004% to 0.009% in appendectomy specimens. Appendiceal duplications can occur alone or in conjunction with cecal duplication. The persistence of the temporary embryologic second cecal appendix is hypothesized to cause appendiceal duplications. We present a case of appendiceal duplication in a 26-year-old Ethiopian female patient who had been experiencing abdominal pain in the right lower quadrant for 1 week. She developed anorexia, a loss of appetite, and a low-grade fever as a result of this. She reported direct and rebound mi
