Continuous intraoperative neuromonitoring (cIONM) in head and neck surgery-a review.

Although the history of intraoperative neuromonitoring (IONM) dates back to the 19th century, the method did not evolve further than the mere differentiation of nerves until recently. Only the development of continuous IONM (cIONM) has allowed for non-stop analysis of excitation amplitude and latency during surgical procedures, which is nowadays integrated into the software of almost all commercially available neuromonitoring devices. The objective of cIONM is real-time monitoring of nerve status in order to recognize and prevent impending nerve injury and predict postoperative nerve function.

[Study progress of intraoperative neuro-monitoring of recurrent laryngeal nerve in thyroidectomy].

Recurrent laryngeal nerve (RLN) injury is a serious complication of thyroidectomy. Intraoperative neuro-monitoring(IONM) is a technology for identifying the integrity of nerves during operation by electrophysiological means, which included the intermitted IONM and the continuous IONM.This paper aims to compare the superiorities and defects of both IONM methods and summarize the current state and progress of IONM.

The role of intraoperative nerve monitoring (IONM) in thyroidectomy: where do we stand today?

OBJECTIVE: Intraoperative nerve monitoring (IONM) gains daily ground, being used during thyroidectomy to help the surgeon identify the recurrent laryngeal nerve (RLN). But its value still remains debatable among endocrine surgeons. In this review article, we aim to present current knowledge on the subject. DATA SOURCES: The MEDLINE/PubMed database was searched for publications with the medical subject heading "Intraoperative Nerve Monitoring (IONM)" and keywords "thyroidectomy," "recurrent laryngeal nerve (RLN)" or "vocal cord paresis or vocal cord paralysis (VCP)." The search was conducted b

Detection of evolving injury to the brachial plexus during transaxillary robotic thyroidectomy.

OBJECTIVES/HYPOTHESIS: Continuous intraoperative neuromonitoring (IONM) of transcranial electric motor evoked potentials (tceMEPs) and somatosensory evoked potentials (SSEPs) has gained universal acceptance as an efficacious method for detecting emerging positional brachial plexopathy or peripheral nerve compression during spinal and shoulder surgery. This has implications for transaxillary thyroid surgery. STUDY DESIGN: Case report with literature review. METHODS: The patient underwent robotic transaxillary thyroid surgery with continuous tceMEP and SSEP monitoring of brachial plexus functi

Closer to an understanding of fate: the role of vascular complications in free flap breast reconstruction.

BACKGROUND: Flap loss in autologous breast reconstruction is almost invariably a result of a vascular event; however, not all events portend the same fate. The purpose of this study was to determine whether intraoperative vascular complications predict subsequent postoperative vascular thrombosis and flap loss. METHODS: This was a retrospective review of all free flaps performed between 2005 and 2010 in the University of Pennsylvania Health System for breast reconstruction. Details of all vascular thromboses and intraoperative technical difficulties were recorded. Flaps with a routine intraop

Exploring perioperative complications of anterior lumber interbody fusion in patients with a history of prior abdominal surgery: A retrospective cohort study.

BACKGROUND CONTEXT: Anterior lumbar interbody fusion (ALIF) exposes the anterior aspect of the spine through a retroperitoneal approach. Access to the anterior spine requires mobilization of intra-abdominal viscera/vasculature, which can become complicated as scarring and/or adhesions develop from prior abdominal surgical interventions, increasing risk of intraoperative complications. The literature suggests that "significant prior abdominal surgery" is a relative contraindication of ALIF surgery; however, there is no consensus within the literature as to what defines "major/significant" abdom…

The perioperative management of pheochromocytoma in children.

The perioperative experiences of 15 children and adolescents who underwent a total of 18 surgical procedures for resection of a pheochromocytoma were analyzed to determine clinical or hemodynamic events associated with intraoperative or postoperative complications. Of the pre- and intraoperative factors assessed, only preoperative resolution of symptoms and normalization of blood pressure were predictive of uncomplicated outcome. No intraoperative factors were statistically associated with outcome, but the four patients with complicated outcomes had had aggressively administered intraoperative

Intra-arterial computed tomography angiography during fenestrated and branched endovascular aortic repair.

BackgroundImproved intraoperative diagnostic imaging during fenestrated and branched endovascular aortic repair (f-bEVAR) may reveal unfavorable positioning of endograft components, endoleaks, and other procedural complications, enabling immediate corrective maneuvers. Consequently, improved intraoperative imaging could reduce the need for follow-up computed tomography (CT) examinations and reinterventions.PurposeTo assess the feasibility of performing intraoperative CT angiography (CTA) during f-bEVAR, and to compare intraoperative CTA findings with postoperative CTA.Material and MethodsIntra

[Operative complications in transurethral operations: causes and prevention].

Complications resulting from transurethral operations can be classified as intraoperative, directly postoperative and late complications. Specific problems are related to prostatic resection, resection of bladder tumors or lithotripsy of bladder stones. An analysis of these problems is presented based on in the experience in more than 10 000 transurethral operations. Directives are developed to avoid additional intraoperative harm and minimize resulting problems. Methods for correction of iatrogenic complications are explained.

Perioperative management and challenges of robotic-assisted surgery: a narrative review.

Over the past two decades, robotic surgical technology has become increasingly widespread worldwide. RAS (robotic-assisted surgery) has attracted surgeons across various specialties with its advantages of high precision, a shorter learning curve, and reduced postoperative pain, while also imposing new requirements on the entire surgical team, including anesthesiologists. The physiological changes and unique complications associated with RAS have brought major challenges for anesthesiologists, who should respond rapidly to patient responses throughout the entire procedure. This review focuses on

Ureteral complications during colorectal surgery.

Ureteral complications occur rarely during colorectal surgery (0.3 to 1.5%), are mainly diagnosed postoperatively (50-70%), and result in both short- and long-term morbidity. The objective of this update was to report on prevention, diagnosis, and treatment options for ureteral injuries that occur during colorectal surgery. For prevention, it is essential to identify at-risk patients preoperatively. Routine prophylactic insertion of a double-J catheter is not recommended. Intra-operatively, non-invasive techniques to aid in locating the ureters based on intravenous injection of methylene blue
