Pediatric keratoconus - Current perspectives and clinical challenges.

Keratoconus is an ectatic corneal disease characterized by progressive stromal thinning, irregular astigmatism, and defective vision. It can be unilateral or bilateral with asymmetric presentation. It starts at puberty and either progresses rapidly to an advanced stage of the disease or stops in case of delayed onset and slow progression. Pediatric keratoconus is more aggressive than in adults and the management protocols differ because of various rationales such as accelerated progression, advanced stage of disease at the time of diagnosis and co-morbidities. It poses a burden to the society …

Computer aided diagnosis for suspect keratoconus detection.

PURPOSE: To develop a stable and low-cost computer aided diagnosis (CAD) system for early keratoconus detection for clinical use. METHODS: The CAD combines a custom-made mathematical model, a feedforward neural network (FFN) and a Grossberg-Runge Kutta architecture to detect clinical and suspect keratoconus. It was applied to retrospective data of 851 subjects for whom corneal elevation and thickness data was available. These data were divided into four groups: a control group (312 eyes) with bilateral normal tomography, keratoconus suspect (77 eyes) with a clinically diagnosed keratoconus in…

[Morphologic Corneal Changes after Crosslinking for Keratoconus].

Keratoconus is a relatively common (1 : 2000) bilateral disease leading to a change in biochemical and biomechanical corneal structure as well as thinning and ectasia. For more than 10 years, crosslinking has been a therapeutic option in cases of progression. Using riboflavin and UVA-radiation, the anterior corneal stroma (300 µm) gets stiffened by crosslinking of collagen fibers. When protocols and limitations are adhered, the procedure is described to be effective and of low-risk. This review gives an overview about physiologic and pathologic changes in keratoconic corneas before and after c…

Association between Family History and Keratoconus Severity.

PURPOSE: The high prevalence of positive family history of keratoconus (KC) in KC patients is well-known. However, the results regarding the association between family history of KC and disease severity are controversial. The aim of this study was to evaluate the possible association between family history and severity of KC. METHOD: Clinical data of 1496 KC patients were evaluated. All participants were asked if they had had a family member with KC. Topographic and keratometric measurements of KC patients, including central corneal thickness (CCT), thinnest corneal thickness (TCT), mean, fla…

[Keratoconus treatment by corneal cross-linking (CLX)].

Keratoconus is a disease of the cornea that usually begins during puberty and progressively weakens its biomechanical structure. Keratoconic eyes show a conic shape and progressive thinning, both leading to irregular astigmatism and reduced vision that cannot be corrected by glasses. In early cases, special contact lens can partly compensate for the visual loss while they do not stop disease progression. Until recently, the only treatment option was a corneal transplant. In 1999, a technique called corneal collagen cross-linking (CXL) was used in human corneas suffering from keratoconus for th…

Two cases of keratoconus diagnosed after pregnancy.

PURPOSE: This case report aims to demonstrate that pregnancy-induced biomechanical corneal changes in combination with risk factors for ectasia can develop into (the exacerbation of) keratoconus. CASE REPORT: We describe two women who were clinically diagnosed with keratoconus after their second pregnancy. Both women were myopic and had a history of allergies and contact lens wear. It is unclear whether these two women had a newly diagnosed keratoconus that developed during pregnancy or had experienced an exacerbation of an unrecognized, subclinical keratoconus. CONCLUSIONS: Nowadays, docume…
