Epidural analgesia in association with duration of labor and mode of delivery: a quantitative review.

OBJECTIVE: This study was undertaken to quantitatively summarize previous literature on the effects of epidural analgesia in labor on the duration of labor and mode of delivery. STUDY DESIGN: Original studies published in English from 1965 through December 1997 were reviewed and assigned a quality score independently by 2 of the authors. Studies that met the minimal requirements were evaluated further. Data syntheses were performed separately according to study design and outcome measurements, including cesarean delivery, instrumental delivery, oxytocin augmentation, and durations of the firs

Examiner's finger-mounted fetal tissue oximetry.

The best way to assess fetal condition is to observe the oxygen status of the fetus (as well as to assess the condition of infants, children, and adults). Previously, several fetal oximeters have been developed; however, no instrument has been utilized in clinical practice because of the low-capturing rate of the fetal oxygen saturation. To overcome the problem, we developed a doctor's finger-mounted fetal tissue oximeter, whose sensor volume is one hundredth of the conventional one. Additionally, we prepared transparent gloves. The calculation algorithm of the hemoglobin concentration was der

Influence of ultrasound determination of fetal head position on mode of delivery: a pragmatic randomized trial.

OBJECTIVE: To evaluate the influence of ultrasound determination of fetal head position on mode of delivery. METHODS: This was a pragmatic open-label randomized controlled trial that included women with a singleton pregnancy in the vertex presentation at ≥ 37 weeks' gestation, cervical dilation ≥ 8 cm and who received epidural anesthesia. Women were assigned randomly to undergo either digital vaginal examination (VE group) or both digital vaginal and ultrasound examinations (VE+US group) to determine fetal head position. When the ultrasound and digital vaginal findings were inconsistent in th

Impact of different sufentanil concentrations in epidural labor analgesia on maternal and neonatal outcomes: a propensity score-matched cohort study.

PURPOSE: The optimal sufentanil concentration for patient-controlled epidural analgesia (PCEA) during labor remains uncertain. This study evaluated the association between sufentanil concentration strategies and maternal-neonatal outcomes. METHODS: This single-center retrospective cohort study included parturients receiving sufentanil-containing epidural labor analgesia. Patients were stratified by fixed sufentanil concentration in the PCEA formulation. The observed concentration range was 0.25-0.50 µg/mL, and the median value of 0.30 µg/mL defined the Low-concentration group (≤ 0.30 µg/mL) a

Fetal heart rate monitoring practices at a public hospital in Northern Uganda - what health workers document, do and say.

Background: In Uganda, perinatal mortality is 38 per 1000 pregnancies. One-third of these deaths are due to birth asphyxia. Adequate fetal heart rate (FHR) monitoring during labor may detect birth asphyxia but little is known about monitoring practices in low resource settings. Objective: To explore FHR monitoring practices among health workers at a public hospital in Northern Uganda. Methods: A sequential explanatory mixed methods study was conducted by reviewing 251 maternal records and conducting 11 interviews and two focus group discussions with health workers complemented by observations of

[Spontaneous pneumomediastinum during labour].

This is a case report of a 35-year-old women who had spontaneous pneumomediastinum (SPM) during the second stage of labour. Although this condition during labor is very rare, it is described in the literature as Hamman's syndrome. The symptoms are dyspnoea, chest pain and subcutaneous emphysema which leads to much more severe differential diagnoses that should be eliminated quickly. There has not yet been found any dispositions to the condition. Duration of the second stage of labour is a theoretic factor of relevance, which naturally puts nulliparas at a higher risk.

Oxytocin regimen for labor augmentation, labor progression, and perinatal outcomes.

OBJECTIVE: To examine the effects and safety of high-dose (compared with low-dose) oxytocin regimen for labor augmentation on perinatal outcomes. METHODS: Data from the Consortium on Safe Labor were used. A total of 15,054 women from six hospitals were eligible for the analysis. Women were grouped based on their oxytocin starting dose and incremental dosing of 1, 2, and 4 milliunits/min. Duration of labor and a number of maternal and neonatal outcomes were compared among these three groups stratified by parity. Multivariable logistic regression and generalized linear mixed model were used to

The effect of hyoscine butylbromide on the duration and progress of labor in primigravidae: a randomized controlled trial.

Источник пометил публикацию как отозванную

OBJECTIVE: To assess the effect of hyoscine butylbromide (HBB) on duration of the first stage of labor in primigravdae. METHODS: A case-control study conducted on 120 primigravida at term admitted in active labor were divided into three equal groups. A single dose of the drug (placebo or HBB 20 mg or HBB 40 mg) was injected intravenously slowly to groups A, B, and C. The duration of the first stage was calculated from the time of cervical dilatation of three to four centimeters in active labor until a fully dilated cervix was observed. RESULTS: The duration of first stage was significantly s

A numerical study on fetal head molding during labor.

During vaginal delivery, the fetal head molds into an elongated shape to adapt to the birth canal, a process known as fetal head molding. However, excessive molding can occur due to prolonged labor or strong contractions, leading to several disorders on the fetal head. This work aims to perform a numerical study on the biomechanics of fetal head molding by measuring specific diameters and the corresponding molding index. A finite element model of the pelvic floor muscles and the fetal body was used. The fetal head is composed of the skin and soft tissues, the skull with sutures and fontanelles

Timing of cesarean section for prolonged labor in urban Tanzania: A criterion-based audit.

BACKGROUND: Similar to many resource-constrained urban settings, cesarean deliveries in Dar es Salaam, Tanzania, have increased rapidly, from 17% in 2015 to 26% in 2022. Alarmingly, at the population level, the increase was not followed by improvements in perinatal outcomes, suggesting the overuse of cesarean delivery. Prolonged labor is the leading cause of women's first cesarean delivery. Therefore, understanding the management of prolonged labor preceding cesarean delivery is crucial for preventing nonmedically indicated cesarean deliveries across Tanzania and globally. OBJECTIVE: This stu

Accidental vaginal incision during cesarean section. A report of four cases.

BACKGROUND: Transverse vaginal incision during cesarean section, also known as anterior vaginotomy, is a recognized entity, mostly made unintentionally. CASES: At our institution, four patients underwent a transverse vaginal incision during cesarean section over an eight-year period. In three of them, excessive bleeding required blood transfusion. In one case, only hypogastric artery ligation resulted in bleeding control. None of the patients had subsequent vaginal deliveries. CONCLUSION: The exact incidence of anterior vaginotomy is difficult to evaluate. Accidental vaginal incision occurs

Elective fetal monitoring and obstetrical operative frequency.

This paper reports on cardiotocographic findings after the introduction of elective fetal monitoring. A classification into the following four groups is recommended: normal, warning, severe functional hemodynamic disorders (SFHD) and hypoxia. This fetal heart frequency (FHF) classification prefers the evaluation of the effects on fetal condition compared with a descriptive classification. SFHD in the first stage of labor can be observed in 4% of parturient women, increasing to nearly 16% in the second stage. Cesarean section rate in this group amounts to 12.5% and forceps rate exceeds 50%. Und

To push ... or not to push?

I was formally asked by my supervisor of midwives to reflect upon two episodes of care. Both of these involved women who would be termed as 'low risk' primigravida and in both instances the women had considered whether, had they been advised against pushing when it was 'too soon', they might have avoided caesarean sections. To help this process I used my own reflection triangle: 'What happened ... Was it OK ... What about next time?' informed by the Gibbs model (1998). In this paper I present my reflections alongside my reflection triangle.
