Reduction in Length of Hospital Stays for Allogeneic Hematopoietic Stem-Cell Transplantation in the Letermovir Era.

BACKGROUND: In recent years, letermovir has been routinely used for cytomegalovirus (CMV) infection prophylaxis in patients receiving allogeneic hematopoietic stem-cell transplantation (HSCT). The reduction effect of letermovir on CMV infection rates and the impact on survival have been studied, but other potential benefits of letermovir remain underexplored. METHODS: This retrospective study included patients who underwent first-time allogeneic HSCT between October 2013 and August 2023. We compared the length of hospital stay between eras before and after the introduction of letermovir ("non

Implications of a Reduced Length of Postpartum Hospital Stay on Maternal and Neonatal Readmissions, an Observational Study.

INTRODUCTION: Reducing the Length Of postpartum Stay (LOS) is associated with lower hospital costs, a major reason for initiating federal projects in Belgium. Disadvantages following the reduction of LOS are the risks of maternal and neonatal readmissions. This study compares readmissions with or without reduced LOS, by introducing the KOZI&Home program in the university hospital Brussels. METHODS: This is an observational study comparing the readmission rates of the length of postpartum hospital stay between two groups: the non-KOZI&Home group (> 2 days for vaginal birth and > 4 days for cae

Sedation for Forearm Fracture Reduction in the Pediatric Emergency Department: Impact on Hospitalization and Length of Stay.

BACKGROUND: Children with forearm fractures who present to the emergency department (ED) often need a closed reduction. In our institution, until 2017, pediatric trauma patients presented to the general trauma ED (GTED) where no sedation services for pediatric patients were available. From 2017, patients presented to the pediatric emergency department (PED) where closed reductions were performed under sedation when appropriate. OBJECTIVES: To compare GTED and PED with regard to length of stay (LOS) and hospitalization rates of pediatric patients with forearm fractures who needed a closed redu…

The effect of point-of-care ultrasound on length of stay in the emergency department in children with neck swelling.

BACKGROUND: Ultrasound is the imaging modality of choice in children presenting to the emergency department (ED) with soft tissue neck swelling. Point of care ultrasound (POCUS) has good accuracy when compared to comprehensive radiology department ultrasound (RADUS). POCUS could potentially improve ED length of stay (LOS) by improving efficiency. We aimed to evaluate the LOS of pediatric patients seen in ED with soft tissue neck swelling who received POCUS compared to RADUS. We determined unscheduled 30-day return visit rates in both groups as a balancing measure. METHODS: We performed a retr…

Suicide attempts by jumping and length of stay in general hospital: A retrospective study of 225 patients.

OBJECTIVE: Violent suicide attempts, such as jumping from a height, frequently lead to hospitalization in general hospital with high length of stay (LOS). We investigated features associated with LOS in this context. METHODS: We retrospectively included all patients admitted after suicide attempts by jumping in non-psychiatric wards of a university hospital between 2008 and 2016. Several socio-demographic and clinical data were collected, including psychiatric diagnoses, coded with the International Classification of Diseases-10th Revision. We used general linear models to identify factors as…

Is there need for technical investigations in order to predict potential length of hospital stay of oral infections?

OBJECTIVE: Oral and maxillofacial infections are generally treated by primary dental or medical caregivers. Nevertheless, because these infections are known to have life-threatening complications, there is a need of clear indicators for emergency services medical staff, particularly in determining when morbidity can be expected and when in-hospital treatment is required. This retrospective study aimed to identify variables that were observable at admission, which could indicate high complication rates, long hospital stays, and/or a need for tracheostomy. MATERIALS AND METHODS: We examined dat

The Predictive Factors on Extended Hospital Length of Stay in Patients with AMI: Laboratory and Administrative Data.

The length of hospital stay (LOS) is an important measure of efficiency in the use of hospital resources. Acute Myocardial Infarction (AMI), as one of the diseases with higher mortality and LOS variability in the OECD countries, has been studied with predominant use of administrative data, particularly on mortality risk adjustment, failing investigation in the resource planning and specifically in LOS. This paper presents results of a predictive model for extended LOS (LOSE - above 75th percentile of LOS) using both administrative and clinical data, namely laboratory data, in order to develop

The impact of psychiatric diagnosis on length of stay in a university medical center in the managed care era.

Inpatient use data were examined for fiscal years 1999-2001. Patients with and without psychiatric diagnoses were compared for length of hospital stay and complexity of illness. Patients with psychiatric disorders represented 33%-35% of total cases. Substance use (9,824 cases), mood disorders (2,524 cases), and cognitive disorders (2,362 cases) were the most common psychiatric illnesses. Patients with substance use disorders or no psychiatric diagnosis had the shortest adjusted length of stay, whereas the small number with adjustment disorders (N=147) had the longest. Other psychiatric patient…

The role of ketorolac in decreasing length of stay and narcotic complications in the postoperative pediatric orthopaedic patient.

The control of postoperative pain in the pediatric orthopaedic patient is a challenging endeavor. Several studies have shown the efficacy of … ts (4% vs. 32%; p < 0.05). Finally, the patients in the ketorolac group had a significantly shorter length of stay (3.63 +/- 1.64 days vs. 4.74 +/- 1.76 days; p < 0.05). There were no bleeding complications in either group. Ketorolac is thus a safe and effective means of controlling postoperative pain in the pediatric orthopaedic population while avoiding the tr …

The Effect of COVID-19 on Length of Stay in Hospital and Patient Population Following Burn Injury.

Acute burn care is heavily resource-dependent and thus was significantly impacted by the COVID-19 pandemic. This study sought to examine the relationship between COVID-19 and the length of stay (LOS) in hospital following burn injury, as prolonged admissions have implications on both individuals and healthcare systems. Additionally, this study explored how COVID-19 affected the homeless burn population, as homelessness has been associated with longer hospital admissions due to limited post-discharge resources. Single-center, retrospective cohort study using data from the Burn Registry and medi…

Associations Between Time-To-Surgery and Postoperative Length of Stay and Complications in Patients With Acute Appendicitis.

INTRODUCTION: Acute appendicitis is a common surgical emergency. The optimal time-to-surgery interval remains debated, as studies reported increased complication risk beyond 12 or 24 hours respectively. We hypothesized that time-to-surgery of up to 24 hours after admission was not associated with increased postoperative length of stay (LOS) or complications. METHODS: This retrospective observational cohort study included adults and adolescents (≥ 15 years) undergoing laparoscopic appendectomy from January 2017-July 2025 in the Capital and the Zealand Regions of Denmark. Data were extracted fr…
