Search for leukemia-restricted targets: a new paradigm for precision immunotherapy in acute myeloid leukemia.

Acute myeloid leukemia (AML) remains a major therapeutic challenge despite remarkable advances in genomic characterization. Although molecular profiling has transformed disease classification and risk stratification, treatment for most patients continues to rely on cytotoxic chemotherapy regimens developed more than 50 years ago. Numerous attempts to improve outcomes through treatment intensification, alternative chemotherapy approaches, and targeted agents have produced incremental benefits for selected patient populations, but have failed to fundamentally alter outcomes for many high-risk AM…

Leukemia cutis of the scalp as a first manifestation of B-cell lymphoblastic leukemia: an unusual presentation.

BACKGROUND: Leukemia cutis (LC) is an extramedullary manifestation of leukemia that occurs when neoplastic cells infiltrate the epidermis, dermis, or subcutaneous tissue of the skin. The clinical presentation of LC can vary significantly and is typically a late sign in advanced disease. In this case report, we describe a diagnostically challenging case of LC in a 10-year-old girl who initially presented with a solitary morphea-like plaque on her scalp, which ultimately led to the diagnosis of B-cell acute lymphoblastic leukemia (B-cell ALL). CASE PRESENTATION: We present a challenging case of…

Transcriptomic signature can distinguish chronic neutrophilic leukemia from ambiguous neutrophilic leukemias.

BACKGROUND: Identifying uncommon neutrophilic leukemias presents a challenging task, owing to the analogous morphological characteristics and the dearth of molecular markers. The transcriptomic profile of bone marrow cells in this disease subset has been rarely explored. MATERIAL AND METHODS: The OHSU-CNL dataset, encompassing clinical parameters and parallel transcriptomic matrix, was downloaded from the Genomic Data Commons (GDC) database. Distinctive co-expressed gene modules and pivotal genes for chronic neutrophilic leukemia (CNL) were identified using R software. Subsequently, a diagnos…

Preclinical targeting of leukemia-initiating cells in the development future biologics for acute myeloid leukemia.

INTRODUCTION: Leukemia-initiating cells (LICs) are a critical subset of cells driving acute myeloid leukemia (AML) relapse and resistance to therapy. They possess unique properties, including metabolic, epigenetic, and microenvironmental dependencies, making them promising therapeutic targets. AREAS COVERED: This review summarizes preclinical advances in targeting AML LICs, including strategies to exploit metabolic vulnerabilities, such as the reliance on oxidative phosphorylation (OXPHOS), through the use of mitochondrial inhibitors; target epigenetic regulators like DOT1L (Disruptor of Telo…

A tour of leukemia progress in 2025, viewed through the MD Anderson leukemia research lens.

Advances in the prognostication, monitoring, and treatment of both the acute and chronic leukemias have led to drastically improved outcomes over the past 2 decades. With the advent of targeted therapies, including antibodies such as blinatumomab and inotuzumab and small molecule inhibitors, such as the BCR::ABL1 tyrosine kinase inhibitors, Bruton tyrosine kinase inhibitors, and venetoclax, the treatment landscape of leukemia has drastically changed, improving survival outcomes while relying less on overall chemotherapy intensity in many leukemia types. This progress has allowed the categoriza…

Practical Management of the Venetoclax-Treated Patient in Chronic Lymphocytic Leukemia and Acute Myeloid Leukemia.

Venetoclax is a potent oral, highly selective small-molecule inhibitor of the antiapoptotic B-cell lymphoma 2 protein approved for chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma in treatment-naive patients (in combination with obinutuzumab) or for patients with relapsed/refractory CLL (in combination with rituximab). Venetoclax, in combination with azacitidine, decitabine, or low-dose cytarabine, is also approved in the United States for the treatment of newly diagnosed acute myeloid leukemia (AML) in adults who are ≥ 75 years or have comorbidities that preclude use of intens…

Simultaneous occurrence of chronic myeloid leukemia and chronic lymphocytic leukemia: report of an unusual case.

The coexistence of chronic myeloid leukaemia(CML) and chronic lymphocytic leukemia(CLL) has been reported occasionally in literature, with only seven cases of simultaneous occurrence of these two diseases. We present here a case of 57 yr male patient where a complete blood count and differential done using volume conductivity scatter (VCS) technology suggested a diagnosis of CML with CLL. It was further confirmed by immunophneotyping and cytogenetic analysis. The patient was started on tyrosine kinase inhibitor, 400 mg once daily. Four months after the treatment, patient is doing fine with a c…

Targeting cAMP in chronic lymphocytic leukemia: a pathway-dependent approach for the treatment of leukemia and lymphoma.

INTRODUCTION: Cyclic AMP (cAMP) promotes growth arrest and/or apoptosis of various types of lymphoma, in particular chronic lymphocytic leukemia (CLL). These responses have spurred the interest in developing agents that increase cAMP to treat such malignancies and to identify mechanisms of the responses. AREAS COVERED: The murine T-lymphoma cell line S49, has provided an important, pioneering model to define mechanisms of cAMP-mediated lymphoid cell death. Studies with S49 cells demonstrated that cAMP, acting via protein kinase A (PKA), is pro-apoptotic through a mitochondria-dependent pathwa…

p27kip1 maintains a subset of leukemia stem cells in the quiescent state in murine MLL-leukemia.

MLL (mixed-lineage leukemia)-fusion genes induce the development of leukemia through deregulation of normal MLL target genes, such as HOXA9 and MEIS1. Both HOXA9 and MEIS1 are required for MLL-fusion gene-induced leukemogenesis. Co-expression of HOXA9 and MEIS1 induces acute myeloid leukemia (AML) similar to that seen in mice in which MLL-fusion genes are over-expressed. p27(kip1) (p27 hereafter), a negative regulator of the cell cycle, has also been defined as an MLL target, the expression of which is up-regulated in MLL leukemic cells (LCs). To investigate whether p27 plays a role in the pat…

Development of hairy cell leukemia in familial platelet disorder with predisposition to acute myeloid leukemia.

Abstract Familial platelet disorder with predisposition to acute myeloid leukemia (FPD/AML) is an autosomal dominant disorder characterized by mild to moderate thrombocytopenia with or without its impaired function, inherited RUNX1 mutation and high incidence of myeloid malignancy, such as myelodysplastic syndrome or acute myeloid leukemia. A 72-year-old male visited our institute because of gradually progressive pancytopenia and splenomegaly, and was diagnosed as having hairy cell leukemia. He was administered one course of intravenous cladribine (0.12 mg/kg, day 1-5) and achieved hematologic…

New strategies in the chemotherapy of leukemia: eradicating cancer stem cells in chronic myeloid leukemia.

Chronic myeloid leukemia (CML) is a myeloproliferative disorder caused by the Philadelphia-positive chromosome deriving from a translocation between chromosomes 22 and 9. The oncogenic product of this aberrant chromosome is the constitutively active tyrosine kinase BCR-ABL that is responsible for leukemic cell growth, proliferation and survival driven by the dysregulation of a large array of signal transduction pathways. Inhibition of BCR-ABL with tyrosine kinase inhibitors proved to be an efficient therapy of CML in the chronic phase. Unfortunately, the impressive success of BCR-ABL inhibitor…

New developments in the treatment of chronic myeloid leukemia and Philadelphia-positive acute lymphoblastic leukemia.

Although imatinib revolutionized the management of chronic myeloid leukemia (CML), recent data indicate a transformation in the treatment approach likely in the near future. The superiority of second-generation tyrosine kinase inhibitors (TKIs) over imatinib in newly diagnosed disease has been recognized. Several investigational agents specific for those patients with the T315I mutation remain under evaluation. In Philadelphia-positive (Ph-positive) acute lymphoblastic leukemia (ALL), the addition of imatinib improved response rates. However, short remission durations with single agent therapy…

Defining leukemia stem cells in MLL-translocated leukemias: implications for novel therapeutic strategies.

Hematological malignancies and probably many other tumors are dependent on highly proliferating and self-renewing cancer stem cells. An important question in the development of novel, less toxic antileukemic strategies specifically targeting leukemia stem cells is how closely leukemia stem cells are related to normal hematopoietic stem cells. It has been recently demonstrated that leukemia stem cells can be derived from different stages in normal hematopoiesis and have unique phenotypic and genetic features. Introduction of Mixed-lineage leukemia ( MLL)-fusion oncoproteins, frequently found in…

Burkitt-type acute leukemia in a patient with B-prolymphocytic leukemia: evidence for a common origin.

Burkitt-type acute leukemia cells were present in the bone marrow of a patient with B-prolymphocytic leukemia diagnosed from peripheral blood cell morphology. Immunophenotype analysis confirmed morphological patterns. Cytogenetic and fluorescence in situ hybridization (FISH) analysis showed an identical t(8;22)(q24;q21) with MYC locus rearrangement in blood and bone marrow cells, with additional chromosome abnormalities in the bone marrow. In addition, the loss of one copy of the TP53 gene and identical IGH DNA clonal rearrangements were shown with FISH and polymerase chain reaction analysis r…
