Continuation of Potentially Inappropriate Life-Sustaining Therapies: Provider Perspectives.

BACKGROUND: Prior work examines provider perspectives on withholding potentially inappropriate life-sustaining treatments over family objections; however, data are limited regarding how physicians approach situations in which they are considering withholding or withdrawing life-sustaining therapies against the wishes of the patient's family. MEASURES: This study sought to better understand the way in which pediatric critical care clinicians navigate situations in which families request life-sustaining therapies that the team feels may be inappropriate. OUTCOMES: We conducted a series of semi

Cardiac Interventions for Patients With Trisomy 13 and Trisomy 18: Experience, Ethical Issues, Communication, and the Case for Individualized Family-Centered Care.

This report is informed by the themes of the session Trisomy 13/18, Exploring the Changing Landscape of Interventions at NeoHeart 2020-The Fifth International Conference of the Neonatal Heart Society. The faculty reviewed the present evidence in the management of patients and the support of families in the setting of trisomy 13 and trisomy 18 with congenital heart disease. Until recently medical professionals were taught that T13 and 18 were "lethal conditions" that were "incompatible with life" for which measures to prolong life are therefore ethically questionable and likely futile. While th

Palliative Care in Children with Inherited Metabolic Diseases: Why does it matter?

BACKGROUND: Inherited metabolic diseases (IMD) bring considerable burden on the child and family. Challenging areas for health care include the identification of distressing symptoms, prognostic uncertainty, and bereavement. Literature regarding the impact of paediatric palliative care (PPC) is scarce. OBJECTIVE: This study aims to evaluate children with IMD referred to a PPC team (PPCT) and to analyse its impact on home care, decision to limit treatment (DLT), use of hospital resources (emergency department admissions - EDA, hospital admissions - HA, intensive care admissions - ICA) and end

The role of mindful self-care in enhancing professional quality of life among hospice medical staff in Poland.

PURPOSE: Given the emotionally demanding nature of hospice care, identifying protective factors is essential for promoting caregiver well-being. This study examines compassion fatigue and compassion satisfaction among Polish hospice medical workers, focusing on mindful self-care practices and perceived work support. It also explores whether professional quality of life differs between those who engage in after-work rituals and those who do not. METHODS: In a cross-sectional study, 97 hospice medical workers (nurses, physicians, physiotherapists, psychologists, and medical caregivers) complete

'I was cracking more than everyone else': young adults' daily life experiences of hypermobility and jaw disorders.

Most studies on general joint hypermobility (GJH) and temporomandibular disorders (TMD) are quantitative and have concluded that joint hypermobility is a risk factor for development of TMD. The present qualitative study aimed to explore young adults' daily life experiences of GJH, specifically these relating to jaw function, and their experiences of medical and dental care providers. Semi-structured interviews were conducted with nine young adults (18-22 yr of age) and data were analysed using qualitative content analysis. The overarching thematic category that emerged was 'Hypermobility in da

End-of-life decision-making and changing preferences in patients with a left ventricular assist device for destination therapy: insights from advance directives in Japan.

End-of-life decision-making and the evolution of patient preferences over time remain insufficiently explored in destination therapy with left ventricular assist device (DT-LVAD) in Japan. This retrospective observational study analyzed standardized advance directives from DT-LVAD patients in Japan. The advance directives comprised (1) preferred end-of-life medical care, (2) designated surrogate decision-makers, and (3) personal life wishes. This study comprises two components: a cross-sectional analysis of all patients at the time of LVAD implantation, and a longitudinal analysis evaluating c

[Surgery meets palliative care : Should palliative care be integrated into surgical training?].

The transfer of knowledge, experience and practical skills in the care of patients in palliative settings takes place in a largely unstructured and sporadic manner in surgical resident training in Austria. The present work assesses the status quo using an online survey of residents in general and visceral surgery, presents their subjective experiences with palliative care and expectations on palliative care providers and their training needs. Over 80% of residents have frequent contact with patients in palliative settings and nearly 60% care for patients in end-of-life situations. Support from

Healthcare utilization and end-of-life care based on life-sustaining treatment decisions in patients with cancer.

PURPOSE: Since the implementation of the 2018 Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment (LST), South Korea has legalized the advance directive (AD) and LST Plan-an immediately actionable medical order-and permitted family members to make related decisions. However, the respective roles and clinical influences of these decision types remain unclear. Therefore, this study aims to compare healthcare utilization and end-of-life (EOL) care based on LST decisions. METHODS: A retrospective chart review was conducted of 801 patients with cancer who died at Chonnam

Are aggressive treatment strategies less cost-effective for older patients? The case of ventilator support and aggressive care for patients with acute respiratory failure.

OBJECTIVES: A common assumption is that life-sustaining treatments are much less cost-effective for older patients than for younger patients. We estimated the incremental cost-effectiveness of providing mechanical ventilation and intensive care for patients of various ages who had acute respiratory failure. DESIGN: Retrospective analysis of data on acute respiratory failure from Study to Understand Prognoses and Preferences for Outcomes and Risks of Treatments (SUPPORT). SETTING: Acute hospital. PARTICIPANTS: 1,005 with acute respiratory failure; 963 received ventilator support and 42 had v

Noninvasive positive pressure ventilation in critical and palliative care settings: understanding the goals of therapy.

OBJECTIVE: Although noninvasive positive pressure ventilation (NPPV) is a widely accepted treatment for some patients with acute respiratory failure, the use of NPPV in patients who have decided to forego endotracheal intubation is controversial. Therefore, the Society of Critical Care Medicine charged this Task Force with developing an approach for considering use of NPPV for patients who choose to forego endotracheal intubation. DATA SOURCES AND METHODS: The Task Force met in person once, by conference call twice, and wrote this document during six subsequent months. We reviewed English-lan

Toward a better death: applying Burkean principles of symbolic action to interpret family adaptation to Karen Ann Quinlan's coma.

This article considers the manner that families come to accept the collapse of identity in a family member who has entered a medical crisis with no hope of returning from it. The transformation is regarded as a "right of passage" and is characterized in terms of both the conditions that bring about resistance to the passage as well as the sorts of symbolic activities that ultimately allow the transformation to occur. The theoretical source that is used to discuss both these issues is Kenneth Burke's (1969) theory of Dramatism, regarded herein as a template that guides both interpersonal action

Hospice Palliative Care: A Model for Quality End-of-Life Care

Abstract Hospice is a program of care provided across a variety of settings, based on the understanding that dying is part of the normal life cycle. As people experience this last phase of life, hospice provides comprehensive palliative medical and supportive services, compassion, and care with the goals of comfort and quality of life closure. A hospice supports the patient through the dying process and the family through the experience of caregiving, the patient’s illness, dying, and their own bereavement. Understanding that the last phase of life is as individual as each person who experienc

Respecting Spinal Cord Injury Patient Autonomy in the Context of Terminal Extubation.

Terminal extubation, or palliative or compassionate extubation, is a legally and ethically accepted practice in the United States where mechanical ventilation is withdrawn when ongoing life-sustaining treatment no longer aligns with a patient's stated goals of care. Nearly 42% of intensive care unit deaths involve withdrawal of such measures. Despite its legal acceptance, terminal extubation remains a nuanced decision, particularly in patients with spinal cord injury (SCI). Approximately 18,000 new traumatic SCIs occur annually in the US, with respiratory failure as the leading cause of death.

Characterization of limitations of life-sustaining therapy decisions and their impact on outcomes in adults with septic shock - Insights from a retrospective cohort.

PURPOSE: In-ICU and in-hospital deaths in critically ill patients are often subsequent to decisions of limitations of life-sustaining therapies (LLST). While septic shock remains associated with high mortality and reduced quality of life in survivors, we addressed the frequency, the decision-making process, the determinants and the attributable mortality of LLST decisions in this setting. MATERIALS AND METHODS: A retrospective (2008-2021) single-center study including consecutive adult patients with septic shock. LLST were distributed into early and delayed when taken within and after the fir
