Plasma bubble formation induced by holmium laser: an in vitro study.

OBJECTIVES: To use the elementary physical measurements of temperature and size to prove that the thermal effects produced by the holmium laser's pulses are due to the formation of a plasma bubble. The physical phenomenon related to high temperatures generated during procedures with the holmium laser (holmium:yttrium-aluminum-garnet) was the object of our interest. METHODS: Using a double micrometric slide attached to a 550-microm optic fiber and two thermocouples submerged in water, a series of pulses of 0.8 J at 10 to 30 Hz was delivered from a holmium:yttrium-aluminum-garnet laser, and we

Comparison of electrocautery versus holmium laser energy source for transurethral ureterocele incision: an outcome analysis from a tertiary care institute.

Transurethral endoscopic incision is an established treatment option for management of obstructing ureterocele. It can be performed using monopolar electrocautery or holmium laser as an energy source. The present study was carried out to evaluate outcomes of transurethral ureterocele incision (TUI) by two different energy sources, i.e., monopolar electrocautery versus holmium laser. A retrospective review of the data of all patients who underwent endoscopic TUI from 2007-2017 was performed. Preoperative clinical, biochemical, and radiological characteristics and operative parameters were revie

Ex Vivo Renal Stone Dusting: Impact of Laser Modality, Ureteral Access Sheath, and Suction on Total Stone Clearance.

Introduction and Objectives: Conventional renal stone dusting is challenging; the holmium (Ho:YAG) laser and holmium with MOSES effect (Ho:YAG-MOSES) fail to uniformly produce fragments ≤100 μm (i.e., dust). The superpulse thulium fiber laser (sTFL) may more effectively render uroliths into "dust," and may thus improve stone-free rates. Accordingly, we performed ex vivo evaluations with all three laser modalities, assessing stone fragments and stone clearance. Methods: Seventy-two ex vivo porcine kidney-ureter models were divided into 12 groups of 6: laser type (Ho:YAG, Ho:YAG-MOSES, sTFL), ur

Temporal evolution of minimally invasive pediatric urolithiasis treatment over 30 years.

Urolithiasis in children has increased substantially over the past 30 years, and surgical management maintains an important role in treatment. Technological advances such as lasers and miniaturization have broadened treatment options, and researchers investigate the best indications for each procedure. The aim was to identify publication trends in the field and explore reasons why Extracorporeal Shockwave Lithotripsy is being gradually less applied in the treatment of pediatric urolithiasis. A Reverse Systematic Review of the literature was conducted regarding the surgical treatment of urolith

[Pancreaticoscopy: experiences, possibilities and limits].

Cholangioscopy in the mother-baby technique is well accepted and widespread these days. On the other hand, the peroral pancreaticoscopy (POPS) is still in the initial phase of development. In cases of doubtful stenoses, duct discontinuation, and radiolucent shadows during an endoscopic retrograde cholangiopancreatography (ERCP) the POPS can be carried out as a clarifying examination. Two variations of babyscopes are currently available. The ultra-thin endoscope allows only limited results due to the lack of tip control and irrigation. On the other hand, devices with an outer diameter of 3.2 mm

Strategy and technique of laparoscopic common bile duct exploration.

The laparoscopic approach to common bile duct exploration enables the complete clearance of stones from the bile duct without damage to structures of physiological importance such as the ampulla of Vater. Despite preoperative endoscopic retrograde cannulation of the biliary tree (ERCP) in patients with suspected stones, routine intraoperative cholangiography reveals a further 6% with unsuspected common bile duct stones. Both the preoperative suspected stone and the stone found on intraoperative cholangiography can be adequately managed by the laparoscopic method. The approach to the common bil

Xanthogranulomatous pyelonephritis infected with the Providencia stuartii: a case report and literature review.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a rare and severe chronic inflammatory disease of the renal parenchyma, which is most commonly associated with super-infections by bacteria such as E. coli, Proteus mirabilis, and occasionally Pseudomonas species. CASE PRESENTATION: Herein, we present a rare case of a patient with XGP infected with Providencia stuartii. Initially, the patient refused nephrectomy and underwent holmium laser lithotripsy and right ureteral stenting, followed by meropenem treatment of 7 days. Relapse occurred in the third month after discharge from the hospi

Laparoscopic-assisted mini percutaneous nephrolithotomy in the ectopic pelvic kidney: Outcomes with the laser dusting technique.

INTRODUCTION: The treatment of renal lithiasis has undergone a sea change with the advent of extracorporeal shock wave lithotripsy (ESWL) and endourological procedures such as percutaneous nephrolithotomy (PCNL), ureterorenoscopy and retrograde intrarenal surgery (RIRS). The presence of anatomical anomalies, such as ectopic pelvic kidney, imposes limitations to such therapeutic procedures. This study is aimed to find a simple and effective way to treat the stones in ectopic kidney. MATERIALS AND METHODS: From 2010 to 2014, nine patients underwent laparoscopic-assisted mini PCNL with Laser dus

Management of ureteric stones.

The contemporary management of ureteric stones is reviewed and evidence based recommendations about treatment are made. Stones measuring less than 4mm in diameter have a high chance of spontaneous passage and the main debate for optimum treatment of larger stones centres around the choice of shock wave lithotripsy or endoscopic management combined with laser fragmentation. Treatment recommendations should be based on patient preference, published evidence, local audit (surgeon expertise and availability of equipment) and cost. Artificial Neural Networks could become a useful tool for predictio

[Micropercutaneous laser nephrolithotripsy].

INTRODUCTION: The least invasive technique of PCNL is micropercutaneous nephrolithotripsy (micro-PCNL). A possibility of kidney puncture under direct endoscopic control with the creation of a working channel sized of 8-4.85 F is a characteristic feature of this system. AIM: To study the possibilities of micro-PCNL and to determine its role in the treatment of kidney stones. MATERIALS AND METHODS: A total of 74 patients aged 49.8+/-16.3 years were included in the study. In majority cases an isolated kidney stone was diagnosed (86.4%). The most common stone localization was pelvis (51.5%), fol

Leveraging access to technology and enhanced surgical technique (LATEST) in laparoscopic bile duct exploration (LBDE).

UNLABELLED: Single-stage management of choledocholithiasis with concomitant gallstones consists of performing either laparoscopic bile duct exploration (LBDE) or intra-operative endoscopic retrograde cholangiopancreatography at the same time as laparoscopic cholecystectomy. Transductal LBDE is associated with significantly higher post-operative morbidity, longer operative times and longer hospital stay when compared to transcystic LBDE. The aim of this study was to report the transcystic exploration rate and post-operative outcomes from LBDE before and after implementation of the LATEST (Lever

Can shock wave lithotripsy lead to impaction of ureteral stones?

BACKGROUND: One of the underlying reasons for shock wave lithotripsy (SWL) resistance is the ureteral stone impaction. This study aimed to investigate the accuracy of the hypothesis, suggesting that SWL per se can be the reason for ureteral stone impaction. METHODS: One hundred and seventy-six patients were enrolled in the study. Data of the patients (N.=50) treated with SWL and subsequent semirigid ureteroscopic laser lithotripsy (SULL) in our center between January 2014 and January 2020 were retrospectively reviewed compared with the data of the patients (N.=126) who underwent SULL without

Flexible and navigable suction access sheaths: what size stone particles can be cleared?

OBJECTIVES: To perform benchtop experiments on flexible and navigable suction (FANS) ureteric access sheaths (UASs) to determine the clearance capabilities for various stone sizes when paired with different-sized ureteroscopes. METHODS: Quartz stones were used as a surrogate for renal stones. Stone samples were created to simulate the range of particle sizes produced by 'dusting' a 1-cm calcium oxalate monohydrate stone. The stone mixture was introduced into an open vial at a 30° angle in aliquots every 3 min over the course of 30 min, mimicking their gradual production by lasertripsy. This b

Ureterolithotripsy for a Ureteral Calculus at the Ureteroureterostomy of a Renal-transplant Recipient.

We describe a 40-year-old living-donor renal-transplant recipient who underwent successful ureterolithotripsy. He had been on hemodialysis for >15 years pre-transplant and underwent ureteroureterostomy along with the surgery. One year post-transplant, ultrasound examination demonstrated hydronephrosis, and CT showed a 6-mm ureteral calculus at the ureteroureterostomy site. No pain and no elevated serum creatinine were present. As the ureter was easily accessed, we performed a ureterolithotripsy, which would confirm whether a suture caused the calculus. Despite ureteral tortuosity, laser stone

Percutaneous cystolithotomy in the pediatric augmented bladder.

PURPOSE: Bladder stones are a common problem following augmentation cystoplasty. With the addition of Mitrofanoff channels for intermittent catheterization and more aggressive bladder neck tightening procedures, the incidence of stones has increased and endoscopic access to the bladder has become more complicated. We present our results of percutaneous endoscopic cystolithotomy in children with bladder augmentation. MATERIALS AND METHODS: We performed percutaneous extraction of bladder calculi in 13 children with bladder augmentation. All patients performed catheterization either via a Mitrof

[Urethral diverticulum in a patient with two stones after combined treatment of prostate cancer with metastasis to the left cavernous body of the penis].

Urethral diverticula are sac-like dilatations of the urethra that communicate with its lumen. They may be congenital or acquired. In males, urethral diverticula are rare and classified as congenital (true) and acquired, which are associated with trauma, abscess, strictures, surgical procedures, in particular in patients with hypospadias. A clinical observation of urethral diverticulum with two stones in a man after complex treatment of prostate cancer with metastasis to the left cavernous body of the penis is presented in the article. At the first stage, the patient underwent endoscopic lithot

Calcinosis cutis in autoimmune connective tissue diseases.

Calcinosis cutis is a chronic condition involving insoluble calcified deposits of the skin and subcutaneous tissue. It is commonly associated with autoimmune connective tissue diseases and can be a source of pain and functional disability. The likelihood of developing calcinosis varies among the autoimmune connective tissue diseases, with systemic sclerosis and dermatomyositis being the most commonly associated. Identification of therapy for this challenging disorder has been hampered by a paucity of large controlled trials. Although there is no uniformly effective treatment for calcinosis cut

Assessment of a novel bendable-tip ureteral access sheath with integrated suction: an in vitro study.

Surgical management of urinary stones has significantly evolved with the advent of extracorporeal shockwave lithotripsy, flexible ureterorenoscopy (fURS), and percutaneous nephrolithotomy. For lower pole stones larger than 1 cm, the choice between fURS and miniaturized percutaneous nephrolithotomy remains debated. fURS with ureteric access sheaths (UAS) is a widely utilized technique, but fragment clearance is challenging in unfavorable anatomical conditions and/or lower pole stones. This study compares a novel flexible suction UAS (FANS) with a standard UAS in an in vitro model of lower pole
