Green Walkability and Physical Activity in UK Biobank: A Cross-Sectional Analysis of Adults in Greater London.

Urban greenspace provides opportunities for outdoor exercise and may increase physical activity, with accompanying health benefits. Areas suitable for walking (walkability) are also associated with increased physical activity, but interactions with greenspace are poorly understood. We investigated associations of walkability and green walkability with physical activity in an urban adult cohort. We used cross-sectional data from Greater London UK Biobank participants (n = 57,726) and assessed walkability along roads and footpaths within 1000 m of their residential addresses. Additionally, we as…

School neighbourhood and compliance with WHO-recommended annual NO2 guideline: A case study of Greater London.

Despite several national and local policies towards cleaner air in England, many schools in London breach the WHO-recommended concentrations of air pollutants such as NO2 and PM2.5. This is while, previous studies highlight significant adverse health effects of air pollutants on children's health. In this paper we adopted a Bayesian spatial hierarchical model to investigate factors that affect the odds of schools exceeding the WHO-recommended concentration of NO2 (i.e., 40 μg/m3 annual mean) in Greater London (UK). We considered a host of variables including schools' characteristics as well as…

Hospital Mortality and Resource Implications of Hospitalisation with COVID-19 in London, UK: A Prospective Cohort Study.

BACKGROUND: Coronavirus disease 2019 (COVID-19) had a significant impact on the National Health Service in the United Kingdom (UK), with over 35 000 cases reported in London by July 30, 2020. Detailed hospital-level information on patient characteristics, outcomes, and capacity strain is currently scarce but would guide clinical decision-making and inform prioritisation and planning. METHODS: We aimed to determine factors associated with hospital mortality and describe hospital and ICU strain by conducting a prospective cohort study at a tertiary academic centre in London, UK. We included adu…

[THE VACCINATION CONTROVERSY IN THE JEWISH COMMUNITY IN LONDON IN THE LATE 19TH CENTURY: "NOTHING IS NEW UNDER THE SUN"].

In the last decade of the 19th century, some hundred years after the introduction of the smallpox vaccine, a heated controversy broke out in London regarding the compulsory vaccination of children against the disease. Amongst those affected by the legislation, mandating the immunization of children against smallpox was a Jewish father who was eventually imprisoned for refusing to have his son vaccinated. His grounds for refusal were his religious beliefs and it is with his case and its aftermath that the present article is concerned. The two major protagonists in the controversy were Chief Rab…

The Three Glycotypes in the London Classification System of Sporadic Creutzfeldt-Jakob Disease Differ in Disease Duration.

Sporadic Creutzfeldt-Jakob disease (sCJD) is the most common form of CJD and is believed to be caused by the misfolding and aggregation of e … feldt-Jakob Disease Registry (ANCJDR) between 1993 and 2018 were analysed and classified as per the London classification system. The data presented here demonstrated that sCJD cases with 'type 1' and 'type 2' PrPSc as defined by the London classification system differ in their disease duration. No other differences in clinical phenotype or biological ch …

Exposure to air pollutants during commuting in London: Are there inequalities among different socio-economic groups?

People with low income often experience higher exposures to air pollutants. We compared the exposure to particulate matter (PM1, PM2.5 and PM10), Black Carbon (BC) and ultrafine particles (PNCs; 0.02-1μm) for typical commutes by car, bus and underground from 4 London areas with different levels of income deprivation (G1 to G4, from most to least deprived). The highest BC and PM concentrations were found in G1 while the highest PNC in G3. Lowest concentrations for all pollutants were observed in G2. We found no systematic relationship between income deprivation and pollutant concentrations, sug…

Diagnosis of cystic fibrosis in London and South East England before and after the introduction of newborn screening.

INTRODUCTION: Newborn screening (NBS) for cystic fibrosis (CF) was introduced to London and South East England in 2007. We wished to assess the details of missed cases, and to compare the age at diagnosis and other clinical parameters, prescreening and postscreening. METHODS: Retrospective and prospective case notes and database review of all newly diagnosed CF patients in our 7 CF centres, for 18 months before and 4 years after NBS started. RESULTS: 347 patients were diagnosed with CF. 126 patients were not screened (born before or abroad), and had a median age at diagnosis of 2.4 years, ex…

Hepatic fascioliasis at a London hospital--the importance of recognising typical radiological features to avoid a delay in diagnosis.

Hepatic fascioliasis is a zoonosis caused by the trematode Fasciola hepatica, which can cause hepatobiliary disease in humans via the ingestion of contaminated water or aquatic vegetables. Reported cases of human infection with F. hepatica arise in regions where cattle are raised, with particular abundance in South America and the Middle East. The typical radiological appearances associated with fascioliasis are well reported and are likely to be more easily recognisable by radiologists in these areas of high prevalence. However, in Western countries where the disease is seldom found, there ma

HIV prevalence and testing practices among tuberculosis cases in London: a missed opportunity for HIV diagnosis?

BACKGROUND: Universal testing for HIV in patients with tuberculosis (TB) has been advocated for over a decade. The aim of this study was to describe the prevalence and testing practices of HIV in TB centres in London. METHODS: A cohort study was undertaken of all patients with TB in Greater London in 2003-4 (n = 1941). Logistic regression was used to assess factors affecting being offered and accepting testing and having a positive HIV result. RESULTS: The overall known prevalence of HIV was 9.9% (193/1941). In those with a test result (including those diagnosed previously) it was 25.6%. Ove…

Palliative care provision for people with intellectual disabilities: interviews with specialist palliative care professionals in London.

Growing numbers of people with intellectual disabilities (ID) are in need of palliative care, but there is inequity of access to palliative care services for this group. This study investigates the issues and difficulties arising for palliative care staff in providing care for people with ID. Semi-structured interviews were conducted with 32 palliative care professionals in London. Factors affecting palliative care provision for people with ID included social issues (home situation and family issues), emotional and cognitive issues (fear, patient understanding, communication, cooperation and c…

Trends in the presentation of cutaneous malignant melanoma over three decades at King's College Hospital, London.

The aim of this study was to examine trends in the presentation of cutaneous malignant melanoma at King's College Hospital (KCH) over the last three decades (1970-2000). KCH was one of seven centres that participated in the 1987 Cancer Research Campaign (CRC) publicity campaign aimed at promoting earlier self-recognition of melanoma. Data included patient age at presentation, sex, tumour site, Breslow thickness and histological subtype. The late 1980s saw a threefold increase in the annual number of melanomas and an eightfold increase in thin melanomas compared to the 1970s. The increase occur

Mortality related to cold and air pollution in London after allowance for effects of associated weather patterns.

We looked for atypical weather patterns that could confound, and explain large inconsistencies in, conventional estimates of mortality due to SO(2), CO, and smoke. Using Greater London data for 1976-1995 in the linear temperature/mortality range 0-15 degrees C we determined weather patterns associated with pollutants (all deseasonalized) by single regressions of daily temperature, wind, rain, humidity, and sunshine at successive days advance and delay. Polluted days were colder (P<0.01 for SO(2), CO, and smoke) and less windy and rainy than usual, and this cold weather was more prolonged than …
