Cytofluorimetric study of cerebrospinal fluid at staging of diffuse large B cell lymphoma.

Central nervous system (CNS) involvement is a devastating manifestation of diffuse large B-cell lymphoma (DLBCL), historically associated with poor outcomes. However, the criteria for accurately identifying patients at highest risk of leptomeningeal involvement at diagnosis or of future CNS relapse remain unclear. Currently, the Central Nervous System-International Prognostic Index (CNS-IPI) score is widely used at diagnosis to identify patients at increased risk of CNS relapse. This study aimed to evaluate the prognostic significance of cerebrospinal fluid (CSF) analysis at diagnosis in relat

Primary Diffuse Large B-Cell Lymphoma Mimicking a Dental Abscess: A Case Report.

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma, yet its presentation in the maxillofacial region can pose diagnostic challenges, particularly in patients with confounding medical histories. We report the case of a 40-year-old Iranian male with a history of ulcerative colitis on mesalazine therapy, who presented with progressive left facial swelling. Symptoms persisted for over a year, during which initial fine-needle aspiration (FNA) of a cervical lymph node showed only chronic lymphadenitis. This result prompted repeated empiric treatments with antibi

Primary breast diffuse large B-cell lymphoma mimicking breast carcinoma: a case report.

BACKGROUND: Primary breast lymphoma (PBL) is an exceedingly rare extranodal malignancy that frequently mimics breast carcinoma on clinical and radiological examination, posing a significant diagnostic challenge. CASE PRESENTATION: We report a case of a 39-year-old Chinese woman who presented with two palpable right breast masses. Imaging revealed suspicious lesions classified as Breast Imaging-Reporting and Data System 4A, prompting surgical excision and biopsy. Histopathological examination and immunohistochemical (IHC) analysis confirmed a diagnosis of diffuse large B-cell lymphoma, germina

Outcomes of primary gastrointestinal diffuse large B cell lymphoma in the rituximab era.

BACKGROUND: Primary gastrointestinal diffuse large B-cell lymphoma (PGI-DLBCL) constitutes 30%-40% of primary extranodal lymphomas. While rituximab has significantly improved survival in nodal DLBCL, its impact on the prognosis of PGI-DLBCL remains inadequately defined. This study aimed to compare the survival outcomes of PGI-DLBCL patients between the pre-rituximab and rituximab eras. METHODS: We identified all PGI-DLBCL cases from the Surveillance, Epidemiology, and End Results (SEER) registry (1975-2015). Patients were categorized into pre-rituximab (diagnosed before 2006) and rituximab-er

Diffuse Large B-Cell Lymphoma Presenting as a Thigh Mass: A Case Report.

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma and typically occurs later in life. Classic symptoms include lymphadenopathy, fevers, night sweats, and weight loss. Extranodal presentations are uncommon and may mimic other malignant processes. A 76-year-old female presented with right thigh swelling and pain. Imaging demonstrated a soft-tissue mass with radiologic features concerning for sarcoma. A biopsy was performed, and histopathology confirmed DLBCL. Primary involvement of the thigh by B-cell lymphoma is exceedingly rare and may be confused with sa

Diffuse Large B-Cell Lymphoma Causing Melena, Gastric Ulcer, Liver Mass, and Hypercalcemia.

This is a case of a woman in her 70s who demonstrated an uncommon presentation and consequence of diffuse large B-cell lymphoma (DLBCL). She presented with a month of generalized weakness and 1-2 weeks of melena. She was found to be anemic and had 1,25-dihydroxyvitamin D (calcitriol)-mediated hypercalcemia. Computed tomography (CT) of the abdomen and pelvis revealed a liver mass. Esophagogastroduodenoscopy (EGD) showed a gastric ulcer, biopsied without evidence of active bleeding. Liver mass was biopsied. Positron emission tomography scan demonstrated an F-18 fluorodeoxyglucose (FDG)-avid hepa

A case of spontaneous regression of primary hepatic diffuse large B-cell lymphoma.

We report a rare case of spontaneous regression of primary hepatic diffuse large B-cell lymphoma (DLBCL) in an 89-year-old man. A 12 cm solitary hepatic mass was found in the anterior and medial segments. Contrast-enhanced CT showed a hypovascular solid tumor. Serologic markers including hepatitis B virus, hepatitis C virus, human immunodeficiency virus, alpha-fetoprotein, protein induced by vitamin K absence or antagonist-II, carbohydrate antigen 19-9, and carcinoembryonic antigen were negative, but soluble interleukin-2 receptor was elevated. Ultrasound-guided liver biopsy confirmed DLBCL, a

Case Report: Collision Tumor Involving Diffuse Large B-Cell Lymphoma (DLBCL) and Meningioma.

We report on a 73-year-old man, initially diagnosed with testicular stage IE DLBCL and an incidental finding of an extra-axial right frontal mass with MRI features of a convexity meningioma. The DLCBL relapsed <2 years after treatment, with multiple lymphoma localizations and increased uptake in the grown extra-axial meningeal mass on 18 F-FDG PET-CT. CD19-CAR-T therapy induced a complete metabolic response with the exception of the frontal mass. Surgical resection revealed a collision tumor involving DLBCL and meningothelial meningioma. Collision tumors-2 histologically different neoplasms th

Bispecific Antibodies-A New Hope for Patients with Diffuse Large B-Cell Lymphoma.

T-cell-engaging antibodies are a promising new type of treatment for patients with refractory or relapsed (R/R) diffuse large B-cell lymphoma, which has changed the prognosis and evolution of these patients in clinical trials. Bispecific antibodies (BsAbs) bind to two different targets (B and T lymphocytes) at the same time and in this way mimic the action of CAR (chimeric antigen receptor) T-cells. They are the T-cell-engaging antibodies most used in practice and are a solution for patients who do not respond to second- or later-line therapies, including chemoimmunotherapy, followed by salvag

Upper Gastrointestinal Bleeding Secondary to Gastric Invasion by Diffuse Large B-cell Lymphoma.

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma subtype. We present a case of an 80-year-old male with upper gastrointestinal bleeding (UGIB), ultimately diagnosed with DLBCL involving the liver, gallbladder, inferior vena cava, and stomach, leading to a large gastric ulcer. Despite initial suspicion of alcohol-related UGIB, imaging revealed multiple masses with extranodal lymphoma involvement. Endoscopic hemostasis was attempted but was complicated by tumor-induced neovascularization. This case highlights a rare cause of UGIB and underscores the challenges in dia

Primary renal diffuse large B-cell lymphoma presenting as new-onset kidney failure.

BACKGROUND: Primary renal lymphoma is a rare form of kidney tumor, accounting for less than 1% of all kidney masses, and with only a few pediatric cases reported in the literature. Bilateral forms can present with kidney failure, constituting a therapeutic challenge for both hematologists and nephrologists, with the need to combine complex chemotherapy regimens and kidney replacement therapy. METHODS: We present a rare case of primary bilateral renal diffuse large B-cell lymphoma in a teenage girl presenting with kidney failure (eGFR of 7 ml/min/1.73 m2), proposing a practical approach to opt

Diffuse FDG uptake of bilateral renal cortex in intravascular large B-cell lymphoma.

Intravascular large B-cell lymphoma (IVLBCL) is a rare type of large B-cell lymphoma characterized by selective growth in the lumina of small vessels in systemic organs. Renal involvement in IVLBCL is rarely reported. Fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) is a powerful tool for the diagnosis of lymphoma, but physiologic FDG excretion in the kidneys makes the interpretation of the tracer uptake in this organ difficult. We treated a 71-year-old Japanese woman with infarct-like lesions whose lactate dehydrogenase was elevated; IVLBCL was thus suspected.

Primary gingival diffuse large B‑cell lymphoma with muscle invasion: A case report.

Primary gingival diffuse large B-cell lymphoma (DLBCL) with muscle invasion is rare and accounts for ~0.5% of all reported cases of extranodal lymphoma. The present study describes the case of a 49-year-old man that initially presented at Yingshan County People's Hospital (Nanchong, China) in August 2017 with a chief complaint of tenderness and swelling of the jaw. Computed tomography revealed a mass in the gingiva, and enlarged lymph nodes in the cervical, mediastinal and gastro-hepatic ligaments. Histological examination indicated the gingival mass was a DLBCL, which was positive for CD20, B

Rare Case of Spontaneous Regression in Primary Gastric Diffuse Large B Cell Lymphoma.

The spontaneous regression (SR) of primary gastric diffuse large B-cell lymphoma (PG-DLBCL) is extremely rare. Although the mechanism of SR is unclear, the host immune response, ischemia, or changes in the tumor microenvironment may play a role. On the other hand, SR does not indicate a complete cure and residual lymphoma may remain dormant, leading to relapse. This paper presents a rare case of an 82-year-old woman with PG-DLBCL who experienced SR without treatment, followed by a short-term relapse. In the present case, the most likely mechanism was biopsy-induced ischemia and increased apopt

Immune signature and pathways investigation of adrenal gland diffuse large B-cell lymphoma.

Diffuse large B-cell lymphoma (DLBCL) accounts for approximately 30-40% of all non-Hodgkin lymphoma cases. Organs located DLBCL such as lymph node, stomach, gastrointestinal tract, or skin were reported. However, the adrenal gland DLBCL was not been well explored. We performed RNA sequencing of ten DLBCL samples from adrenal gland, integrated analyzed DLBCL RNA data from multiple organs, defined the new subtypes of adrenal gland DLBCL and explored their molecular signatures. The special expression pattern and microenvironment immunology scores of adrenal glands DLBCL were observed when compare

Cathepsins effect on diffuse large B cell lymphoma risk: A Mendelian randomization study.

Recently, the relationship between cathepsins and a variety of diseases has gained increasing attention, especially various solid tumors and hematologic malignancies. However, there is no consensus on the causality between cathepsins and malignant lymphomas. The cathepsin genome-wide association studies (GWAS) dataset for this study was derived from THE INTERVAL STUDY and was freely downloaded via the OPENGWAS website. GWAS summary statistics for lymphomas were obtained from the FinnGen database. Single nucleotide polymorphisms satisfying the genome-wide association significance (P < 5 × 10-6)

Prebiopsy Steroids and Diagnostic Yield in Patients With Diffuse Large B-Cell Lymphoma.

IMPORTANCE: Corticosteroids are frequently used to manage symptoms in patients with diffuse large B-cell lymphoma (DLBCL), but longstanding clinical caution suggests that prebiopsy administration may reduce diagnostic accuracy. The association between steroids and biopsy outcomes in patients with systemic DLBCL remains unexamined. OBJECTIVE: To determine whether corticosteroid use before biopsy is associated with diagnostic yield in patients with newly diagnosed DLBCL. DESIGN, SETTING, AND PARTICIPANTS: Retrospective case series of adult patients newly diagnosed with DLBCL at a single instit

Isolated Palatal Perforation Secondary to Diffuse Large B-Cell Lymphoma: A Diagnostic Challenge.

Palatal perforation, a diagnostically complex condition, arises from diverse etiologies including neoplasms, infections, or inflammatory disorders. Patients often experience dysphagia, dysarthria, and social distress due to nasal regurgitation. We report a 22 year-old cachectic woman with isolated palatal perforation secondary to diffuse large B-cell lymphoma (DLBCL). She presented with a 2-month history of a painless, progressive midline ulcer unresponsive to antibiotics, culminating in a 2 cm perforation. Biopsies revealed necrotic tissue, fungal elements, and atypical lymphocytes; immunohis
