[Magnetic resonance tomography (MRT) of the adrenals].

Thirty-eight MRT examinations were carried out in 35 patients who, between them, had 19 abnormalities in the adrenal glands. These included ten adenomas, three metastases, two carcinomas, two phaeochromocytomas, and two primary retroperitoneal tumours. MRT provides images of normal and abnormal adrenal glands which are equivalent to those of CT, with the exception that it cannot demonstrate calcification. Using a ratio of signal intensity from the adrenal tumours and the liver on T2-weighted images, it is usually possible to differentiate adenomas from metastases, carcinomas or phaeochromocyto

[Magnetic resonance imaging in dissection of the aorta].

In order to evaluate the usefulness of this new exploratory method in dissection of the aorta, 13 patients (10 men and 3 women, mean age 58 +/- 14 years) were examined by means of an 0.5 Tesla Magniscan 5000 nuclear magnetic resonance machine. Fifteen examinations were performed: 5 in patients with actual aortic dissection (type A 4 including one acute, type B1), 5 in patients previously operated upon for type A dissection and 5 for suspected acute aortic dissection unconfirmed by other examinations. Magnetic resonance imaging (MRI), synchronized with electrocardiography, was carried out using

Tuberous sclerosis: magnetic imaging of the brain.

The results of the magnetic resonance imaging of three patients with proven tuberous sclerosis will be discussed. Cortical tubers and subcortical areas of junctional dysmyelination can be demonstrated clearly by MRI, whereas small calcifications may be missed because of low contrast if the chosen slice thickness should be too large. It is suggested that MRI should be applied to those patients with suspected tuberous sclerosis, in which CT has failed to demonstrate typical subependymal calcifications and to those in which doubts about the definite diagnosis remain. This should be done in order

Magnetic resonance imaging in genital anomalies.

Magnetic resonance imaging, a new radiographic modality that is not dependent on ionizing radiation or intravenous contrast agents, was used to identify undescended testes in 11 patients. Of 14 undescended testes 13 were identified and the only testis that was not seen was in a patient with surgically proved unilateral anorchism. We also used magnetic resonance imaging in 9 patients with various types of intersex. The anatomy of the corpora cavernosa, vagina, uterus and gonads was well seen in each instance.

Magnetic resonance imaging in primary amyloidosis.

Twelve patients with primary amyloidosis (AL) were investigated with magnetic resonance imaging (MRI). In 9 patients an abnormal thickening of the heart walls was present and in 2 macroglossia was found at MRI. T1 was significantly increased in liver (p less than 0.05) and subcutaneous fat (p less than 0.01) while it was decreased in the spleen (p less than 0.05). T2 was significantly decreased (p less than 0.01) in the spleen in patients with amyloidosis, while it was not significantly altered in the liver or subcutaneous fat. After therapy T1 of the liver was reduced towards normal values in

Magnetic resonance imaging: lumbosacral lipoma.

To evaluate the clinical efficacy of magnetic resonance imaging (MRI) of lumbosacral lipomas, the magnetic resonance images of nine patients were reviewed. T1- and T2-weighted spin echo sequences were used with a 0.5-T magnetic resonance system. The tethered or low-positioned conus medullaris, the lipoma itself, the lipoma--cord interface, the subarachnoid space, and hydromyelia were clearly disclosed. The nerve rootlets were not as clear. These results indicate the possible discontinuance of myelography and metrizamide computed tomography (CT) cisternography for such imaging. The diagnostic m

[Magnetic resonance tomography in acoustic neurinomas].

The possibilities of MR and CT were compared in respect of neurinomas of the acoustic nerve (n. acusticus). The comparison showed that MR detection of tumours in the posterior of the cranial cavity is the more sensitive method, especially in respect of neurinomas of the acoustic nerve, although CT enables the use of the small-level technique with or without the addition of a contrast substance, as well as the application of pneumocisternography. The sensitivity of MR can be increased further by adding a contrast medium. Whereas in diagnosis of extrameatal neurinomas of the n. acusticus, CT and

Magnetic resonance imaging in the evaluation of the retroperitoneum.

Magnetic resonance imaging techniques have made it possible to delineate a variety of pathologic processes in the retroperitoneum, including lymphadenopathy, atherosclerotic and aneurysmal disease of the aorta, congenital venous anomalies, and primary and secondary retroperitoneal neoplasms. The availability of multiplanar image display, the excellent soft tissue contrast resolution, and the fact that exogenous contrast materials are not needed are valuable assets of this imaging modality.

Magnetic resonance imaging of musculoskeletal infections.

In summary, MRI has a high sensitivity for the detection of inflammatory disease involving the musculoskeletal system. Because of the usual anatomic pattern of involvement in the vertebral column, there is also a high specificity. In the appendicular skeleton and pelvis, it is unclear whether MRI can add to the sensitivity and specificity of radionuclide studies in uncomplicated cases of acute infection. However, it can be performed more rapidly and provides greater anatomic detail and delineation of the extent of marrow involvement than radionuclide studies. MRI can be used in both acute and

[Magnetic resonance imaging in moyamoya disease].

Ten patients, seven children and three adults, with "moyamoya" disease were examined by MR-CT. MRI findings in these patients were compared with X-ray CT findings to study the features of the ischemic lesions in "moyamoya" disease and the following results were obtained. Old cerebral infarct, cortical atrophy and intracerebral hematoma in "moyamoya" disease were better delineated by image of the spin echo method. It is possible to differentiate the ischemic lesions such as ischemic edema, or acute or chronic cerebral infarct by calculating the T1 and T2 value from the T1 and T2 image. Ischemic

Magnetic resonance imaging of the hila and mediastinum.

In the diagnosis of chest diseases, MR imaging has been shown to be most helpful in the evaluation of hilar and mediastinal lesions. The detection of intrathoracic abnormalities is technique-dependent, and imaging with both short and long repetition and echo times is necessary. ECG gating and sagittal or coronal images supplement transaxial scans in some patients. MR allows the diagnosis of mediastinal vascular lesions and the differentiation of mediastinal mass and vessel without contrast agents. It provides diagnostic information quite similar to that of CT in patients with mediastinal mass,

[Magnetic resonance tomography of spinal masses].

We examined 22 intramedullary, 17 intradural extramedullary and 20 extradural lesions, with direct involvement of the spinal cord by magnetic resonance tomography, using surface coils and 5 mm., or narrower continuous sections. Spinecho pulse sequences are suitable for multi-slice examinations. T.1-weighted examinations are suitable for demonstrating the syrinx in syringomyelia, in all other circumstances T1 and T2 sequences are essential. Gd-DTPA is unnecessary. The results of these recommendations are illustrated.

Magnetic resonance imaging of the genitourinary tract.

MRI is in its infancy as a clinical imaging tool. It is undergoing intensive investigation in various areas of the body. Evaluation of the brain and spine is superb, and in some areas of the brain, like the posterior fossa, it is thought to be superior to CT. Evolving indications for body scanning include staging of pelvic malignancies, evaluation of liver malignancy, evaluation and staging of musculoskeletal problems, and, to a lesser degree, staging of renal malignancies and evaluation of vascular disease. The main problem in body imaging stems from image degradation because of respiratory m

Magnetic resonance imaging in marrow diseases.

Magnetic resonance imaging is an exciting and promising diagnostic tool. It employs a powerful magnet and radio frequency waves to produce details of the human body with no known risk to patients or staff. It produces superior soft-tissue contrast between muscle, fat, ligaments and tendons, nerves, and blood vessels. Magnetic resonance imaging has potential diagnostic value in avascular necroses of bone caused by trauma, Legg-Perthes diseases, sickle cell syndrome, and other conditions. It is useful in determining the extent of marrow lesions, including tumors and metabolic disturbances, muscl
