Changes in mandibular and articular dynamics associated with surgical versus nonsurgical treatment of mandibular condylar fractures: a systematic review with meta-analysis.

OBJECTIVE: The aim of this study was to evaluate the mandibular and articular dynamics and the presence of complications associated with surgical or nonsurgical treatments of condylar fractures. STUDY DESIGN: Clinical trials that compared open reduction internal fixation and maxillomandibular fixation in patients with condylar fractures were included. We performed an electronic search of PubMed, Scopus, Cochrane Library, Web of Science, and LILACS (Latin American and Caribbean Health Sciences) databases starting from February 2017 and updated in January 2019 and found 467 articles. We evaluat

The impact of fracture fragment displacement on sensory function recovery of the inferior alveolar nerve in mandibular angle fractures.

OBJECTIVE: To investigate the correlation between the initial, three-dimensionally quantified displacement of mandibular angle fractures and the prognosis of inferior alveolar nerve (IAN) sensory function recovery following open reduction and internal fixation (ORIF). METHODS: This retrospective cohort study included patients with unilateral mandibular angle fractures treated with ORIF between March 2022 and August 2025. Preoperative cone-beam computed tomography (CBCT) was used to measure the maximum 3-dimensional (3D) displacement of the fracture fragments. Patients were stratified into thr

Treatment of mandibular condyle fractures with a rhombic 3D condylar fracture plate - Does the surgical approach matter?

Fractures involving the mandibular condyle are a significant subset of mandibular fractures, demanding specialized implant design considerations for effective surgical treatment. The core objective of this study was to assess the clinical and radiographic results in patients treated with a single rhombic-shaped implant according to the surgical approach used. This retrospective cohort investigation, spanning a ten-year recruitment window, included individuals with unilateral fractures who underwent surgical treatment with a rhombic-shaped implant. These patients were subject to follow-up exami…

Biomechanical Evaluation of Mandibular Condyle Fracture Osteosynthesis Using the Rhombic Three-Dimensional Condylar Fracture Plate.

PURPOSE: The aim of the present study was to evaluate the biomechanical stability of osteosynthesis in mandibular condyle fractures using a newly designed rhombic 3-dimensional (3D) condylar fracture plate and compare it with that using standard two 4-hole miniplates and with that in nonfractured condyles. MATERIALS AND METHODS: Using 200 porcine mandibles, 3 different monocortical plating techniques were evaluated. The condyles were fractured along a defined line tangentially through the sigmoid notch and perpendicular to the posterior border. After anatomic reduction, osteosynthesis was per

A Case Report of Oral Bisphosphonate Treatment for Osteoporosis Leading to Atypical Femoral Fracture and Pathologic Mandibular Fracture.

BACKGROUND Bisphosphonates inhibit bone resorption in patients with postmenopausal osteoporosis and reduce osteoporotic fracture incidence. Medication-related osteonecrosis of the jaws (MRONJ) and atypical femoral fractures (AFF) are both rare but serious adverse effects of anti-resorptive drugs (ARD) such as bisphosphonates. The most advanced form of MRONJ is termed stage 3 and can lead to severe local sequelae like pathologic mandibular fractures (PMF). This study reports a case of MRONJ-related PMF and AFF with osteomyelitis secondary to bisphosphonate treatment for osteoporosis. CASE REPOR…

Mandibular Alveolar Ridge Fracture Repair With Circummandibular and Interdental Vicryl Sutures.

Pediatric alveolar ridge fractures can be managed surgically with semi-rigid splinting, plates, or wires. However, these surgical methods necessitate a second operation for hardware removal. This case highlights an alternative approach that does not require reoperation. A 2-year-old girl presented to the emergency department with a mandibular alveolar ridge fracture and dental displacement. Our team performed interdental and circummandibular fixation of the displaced segment using absorbable 4-0 Vicryl sutures. Postoperative follow-up demonstrated stable alignment, proper healing, and no compl

Endoscope-Assisted or Skin-Approach Osteosynthesis of Mandibular Condylar Fracture-A Comparison.

Open reduction and internal fixation (ORIF) for mandibular condyle fractures remains a controversial and challenging issue, with the exception of basal and low-neck fractures. Currently, there is a consensus that fractures causing irreparable malocclusion or dislocation, when the fracture line runs through the base or lower neck of the condyle, require ORIF. Due to the different characteristics of fractures, various surgical approaches and their modifications are available. The use of a minimally invasive intraoral approach during endoscope-assisted procedures is considered safer for the facia

Injury to Anatomic Structures Due to Fixation Device in Management of Mandibular Fracture.

INTRODUCTION: ORIF is the gold standard for the management of mandibular fractures. The chances of postoperative complications are due to infection, damage of the inferior alveolar nerve canal, or tooth root during the fixation of the fracture fragments. Fixation hardware placed in the vicinity of the vital structures may encroach on the anatomic structures, leading to postoperative complications. AIM: The aim of this study is to evaluate the injury to the anatomic structures in the proximity of the fracture line due to the fixation device. MATERIAL AND METHODS: This retrospective study was …

Analysis of Surgical Complications in Mandibular Fractures in the Center of Italy: A Retrospective Study.

Mandibular fractures are the second most common maxillofacial fractures. The prevalent treatment for this kind of fractures is either a surgical procedure such as maxillomandibular fixation or open reduction and internal fixation or a combination of both. Sometimes the patients might develop locoregional postoperative complications. The most frequently reported complications are trigeminal V3 injury and surgical site infection. A 4-year retrospective study was performed at the Department of Oral and Maxillofacial Surgery, Policlinico Umberto I of Rome, Italy, and the postoperative complication…
