Evaluation of patient satisfaction and maximum biting force of three differently constructed bars on two implants retaining mandibular overdenture - one year follow-up (a randomized controlled clinical trial).

BACKGROUND: Different bar construction techniques will affect the bar passive fitness, which may induce stresses or strain on the implant and/or tightening screw and sequentially may affect the biting force and patient satisfaction. AIM OF THE STUDY: This clinical investigation assessed patient satisfaction and maximum biting force (MBF) using three differently constructed (conventional casting, milling, and 3D printing CAD/CAM techniques) cobalt-chromium (Co-Cr) bar-retained implants mandibular overdentures over a one-year period of follow-up. MATERIALS AND METHODS: Thirty edentulous patien

Vertical ridge augmentation of atrophic posterior mandible using an inlay technique with a xenograft without miniscrews and miniplates: case series.

BACKGROUND: Rehabilitation of partially or totally edentulous posterior mandible with implant-supported prosthesis has become a common practice in the last few decades, with reliable long-term results. The use of miniscrews and miniplates have been reported to increase the risk of fracture of the osteotomy segments. The purpose of this case series was to use an inlay technique, without the use of miniscrews and miniplates for stabilization of the transported bone fragments. MATERIALS AND METHODS: Nine consecutive patients (six men and three women) aged between 26 and 51 years (mean 44 years)

Biomechanical analysis of a temporomandibular joint prosthesis for lateral pterygoid muscle reattachment.

OBJECTIVE: To analyze the biomechanical properties of a novel temporomandibular joint (TMJ) prosthesis with an attachment area for the lateral pterygoid muscle (LPM). STUDY DESIGN: Three prosthesis models were created and compared using finite element analysis for the displacement, stress, and strain when simulating the maximum bite force loading. A verification experiment and a compression test were conducted. RESULTS: The displacement, stress, and strain of the novel TMJ prosthesis were larger than the solid condylar neck prosthesis and similar to the slotted condylar neck prosthesis, but

Pericranium graft in reconstructive surgery of atrophied maxillary bones.

In preprosthetic surgery the autologous bone is universally considered the gold standard. Calvaria is, among many options, one of the preferred for its unique characteristics of hardness, easy of harvest and very low morbidity at donor site. Moreover, it gives the possibility of harvesting the pericranium. This technique, recently introduced in common practice in Milan, allows to harvest a large quantity of periosteum to cover bone grafts perioperativly. Periosteal tissue is used to cover bone grafts for two reasons. First, it would provide a layer of tissue that, thanks to its osteogenic pote

Prototype design of porous Ti6Al4V intraosseous implant for use in mandibular reconstruction.

The design of patient-specific implants often requires computer simulations for the characterization of mechanical properties before manufacturing. We previously developed numerical models to predict the mechanical properties of porous Ti6Al4V constructs built using laser powder bed fusion (LPBF). Here, we developed a patient-specific porous intraosseous mandibular implant based on the models and techniques described in our previous research. The implant model used a simple cubic porous design with an average unit cell size of 1 mm and strut thicknesses between 350 and 450 μm. Finite element a

In-House Advanced 3D Printing Enhances TMJ Surgical Planning: The 10-Year Evolution of the Mayo Clinic Experience.

The temporomandibular joint (TMJ) is an anatomically rich area, and TMJ surgery may be considered both anatomically and technically challenging, particularly when the pathology is complex or poorly visualized on conventional imaging. This case series highlights the role of advanced in-house 3D printing in TMJ surgical planning. The authors present 7 diverse TMJ surgical cases treated over the past decade, where 3D models were instrumental in preoperative planning. Models were fabricated from CT and/or MRI data using different materials for the print and model design, and customized to highligh

Influence of different implant designs on stress distributions in all-on-four concept: A finite element analysis.

The study aims to investigate the effect of different implant designs on stress distribution on the implant and mandibular bone in the All-on-four treatment concept using three-dimensional (3D) finite element analysis (FEA). A 3D FEA model was constructed based on the edentulous mandible's mean value. Eight different implant designs, consisting of two main groups with conical and cylindrical body geometry and four subgroups with four different thread designs, v-shaped, square, buttress, and reversible buttress in each group, were experimentally created in a computer environment. Implants 13 an

Mandibular reconstruction system reliability analysis using probabilistic finite element method.

The aim of this study was to design for mandibular reconstruction of large lateral defect with minimum target reliability with designated confidence interval under bite force range of 300 ± 102 N. The performance of the models has been evaluated by numerical analysis considering the uncertainty of input parameters. Computer-Aided design was used to develop the models of three designs according to the patient's anatomy and to achieve to near symmetry of the mandible. Stress-strength modeling was utilized for the probabilistic physics of failure analysis under assumption of a quasi-static load.

Therapeutical attitudes in tooth supported overdentures with ball attachements. Case report.

Tooth supported overdenture with ball attachments has a number of advantages in prosthetics, but presents some difficulties as well, which sometimes make impossible the use of these anchoring systems; these difficulties should be well known. In this regard we present a suggestive case. It is the case of a patient, aged 57, who came for treatment, suffering from subtotal maxillary and mandibular edentulism (present 11 and 21, respectively 33 and 43), previously having an overdenture prosthesis on natural teeth, with special systems - ball attachment type, dissatisfied with the treatment (due to
