Actinomycotic mastoiditis complicated by sigmoid sinus thrombosis and labyrinthine fistula.

Actinomyces is a rare pathogen that can be the cause of infections in the digestive and urinary tracts, skin, genitalia, and lungs, which generally have an indolent clinical course. However, in some cases these can be locally destructive and become generalized infections. Actinomyces has been previously implicated in infections of the middle ear, nasopharynx, and sinuses, occasionally causing complications such as chronic mastoiditis. Here we describe the case of a 10-year-old-male presenting with nausea, vomiting, and headache who developed intracranial complications of actinomycotic mastoidi…

The management of acute mastoiditis in children with cochlear implants: saving the device.

OBJECTIVE: Early treatment of profound bilateral sensorineural hearing loss with cochlear implantation has become routine, resulting in an increased proportion of children implanted at younger ages. These children are at a relatively high risk for acute otitis media (AOM), and are more likely to develop mastoiditis in the implanted ear. Despite the significant risks associated with mastoiditis, including compromise of the implant, there are no specific guidelines on the management of this population. We propose a treatment algorithm emphasizing early but conservative operative intervention. M…

What effect has pneumococcal vaccination had on acute mastoiditis?

BACKGROUND: Streptococcus pneumoniae is a frequent cause of acute mastoiditis. Despite the recent (2005) introduction of pneumococcal vaccination, mastoiditis incidence and severity may be increasing. This study aimed to assess the incidence, severity and microbiology of acute mastoiditis over an 11-year period. METHODS: Retrospective review of paediatric acute mastoiditis cases seen at our institution (2000-2010), comparing patients seen prior to vaccination introduction (period one, 2000-2004), around the time of vaccine introduction (period two, 2005-2007) and post-vaccination (period thre…

Fusobacterium necrophorum mastoiditis in children - emerging pathogen in an old disease.

BACKGROUND: Anaerobic bacteria are uncommon etiologic agents of acute mastoiditis in children. However, recent studies suggest an increase in the incidence of Fusobacterium necrophorum mastoid infections in the last two decades. METHODS: A surveillance study performed over 3.5 years in a tertiary pediatric medical center identified 7 children with acute F. necrophorum mastoiditis. Clinical, laboratory, and treatment data were collected by file review. RESULTS: Five of the 7 children presented in the last year of the study. All 7 children were less than 26 months old on admission, and none ha…

Pediatric acute mastoiditis in the post-pneumococcal conjugate vaccine era.

OBJECTIVES/HYPOTHESIS: To determine whether the characteristics of acute mastoiditis in children have changed in the post-heptavalent pneumococcal conjugate vaccine (PCV7) era. STUDY DESIGN: Case series. METHODS: Clinical data for all patients admitted for acute mastoiditis at an urban tertiary children's hospital from July 1, 1996, to June 30, 2009, were reviewed. Patients with a known immune deficiency or cholesteatoma were excluded. Patients were divided into pre-PCV7 (July 1996 to December 2002; n = 38) and post-PCV7 (January 2003 to June 2009; n = 58) groups. Statistical analyses were u…

Decisions regarding intracranial complications from acute mastoiditis in children.

PURPOSE OF REVIEW: To review the clinical findings and treatment algorithms for intracranial complications of acute mastoiditis, such as sigmoid sinus thrombosis, otitic hydrocephalus, intracranial abscess, and otitic meningitis. We also briefly discuss the clinical sequelae of these complications. RECENT FINDINGS: Recent changes in the microbiology and treatment paradigms of otitis media have the potential to influence the rates of intracranial complications of mastoiditis; however, evidence supporting a resultant increase in the rates of these complications is lacking. SUMMARY: Antibiotic …

Mastoiditis secondary to mycobacterium abscessus imaged with gallium-67 scintigraphy.

Atypical mycobacterium is rarely seen as a cause of chronic mastoiditis but has been increasingly recognized over the past few years. Mycobacterium abscessus is the most pathogenic and chemotherapy-resistant, rapid-growing mycobacterium of all the four groups. This paper presents a case of a 57-year-old woman who had chronic mastoiditis with recurrent exacerbations. The initial computed tomography (CT) findings showed the presence of an inflammatory process and she was treated with the appropriate antibiotics. The patient subsequently underwent a tissue biopsy when she presented with another e…

Silent mastoiditis-tuberculous aetiology presenting as facial nerve palsy.

We report an unusual case of sudden onset lower motor neuron facial palsy in a 30-year-old male. It was subsequently diagnosed to be caused by silent mastoiditis of tuberculous aetiology. The diagnosis was based on the histology of granulations found during facial nerve decompression. The facial palsy resolved after initiating anti-tubercular therapy and surgical decompression. Our case is an uncommon case of acute onset infranuclear facial palsy due to tuberculosis, in the absence of any ear findings, resembling Bell's palsy.

Genetically-induced deep venous thrombosis presenting as acute mastoiditis.

Sigmoid sinus thrombosis (SST) usually follows acute as well as chronic otitis media with coalescent mastoiditis. A singular case of noncoalescent mastoiditis complicated with thrombosis of deep cerebral sinuses occurring in a young child is presented. A genetic thrombophilic disorder (prothrombin G20210A allele mutation) was identified as the predisposing factor for this unusual complication. Particular emphasis is placed on the course of the disease, which showed regression only after surgical exploration and additional anticoagulant therapy. We conclude that a thorough and early assessment …

[Acute mastoiditis in children: clinical and bacteriological study of 17 cases].

OBJECTIVE: Acute mastoiditis (AM) represents the most frequent complication of acute otitis media (AOM) in children. In the literature, its incidence is stable but with an impressive increase in pneumococci with reduced sensitivity to penicillin (PRSP). The aim of this study was to assess the incidence of AM in the area of Tours and the prevalence of PRSP. METHOD: This was a retrospective study of children admitted to the regional paediatric centre in the Tours area, between January 1994 and May 2001. The coded AM files were analysed, excluding all the sub-acute forms or those complicating a …

Acute mastoiditis in a Norwegian population: a 20 year retrospective study.

We have retrospectively examined the nature of acute mastioditis (in western Norway) during a 20 year period (1980-2000). Sixty-one cases of AM were identified in 57 patients with a mean age of 3.6 years. We found no significant change in the incidence of AM during the last 20 years. Seven patients were treated solely with intravenous antibiotics and myringotomies. Fifty patients also underwent cortical mastoidectomy, four cases with bilateral surgery. Antibiotic treatment was given to 31 of the patients before admission to hospital and this group had a significant longer duration of symptoms …

Acute mastoiditis in children: a seventeen-year experience in Dallas, Texas.

BACKGROUND: In the preantibiotic era acute mastoiditis was the most common complication of acute otitis media, often resulting in substantial morbidity and mortality. Since 1989 several investigators have documented an increased frequency of acute mastoiditis in children. METHODS: The medical records of all children with a discharge diagnosis of acute mastoiditis, managed at Children's Medical Center, Dallas, TX, from 1983 through 1999 were reviewed. RESULTS: There were 57 cases of acute mastoiditis during the 17-year period of 1983 through 1999 compared with 57 cases in a 25-year period of …

[Morphological and morphometric study of the tympanic and mastoid portions of the fallopian aqueduct].

In surgery of the temporal bone the best way to avoid injuries to the facial nerve is to be acquainted with its location in all the surgical steps of the procedure scheduled. That is to say: an exact knowledge of its normal intratemporal run, having always in mind some characteristic landmarks, is absolutely essential. This work has been done through drilling 92 human temporal bones, measuring the length of both the tympanic and mastoidal segments and also considering the bending of the 2nd knee of the nerve. In the issue the mastoidal portion resulted the most straight (by half of the cases) …

Chronic mastoiditis mimicking recurrence of nasopharyngeal carcinoma: report of a case.

A nasopharyngeal mass with cranial neuropathies usually indicates an advanced neoplastic process. We present a case of nasopharyngeal carcinoma diagnosed and treated 38 years ago. A nasopharyngeal mass recurred with cranial neuropathies and concurrent otologic infection in June, 1990. Repeated nasopharyngeal biopsies were negative with respect to tumors. Suspecting direct extension of the otologic infection, modified radical mastoidectomy was performed. The nasopharyngeal mass resolved after the surgical intervention. Spread of otologic infection from the temporal bone to the nasopharynx may b…

Intracranial and extracranial complications of acute mastoiditis: evaluation with computed tomography.

The incidence of acute mastoiditis has decreased significantly because we can effectively treat otitis media with oral antibiotics. Inadequate or delayed treatment of otitis predisposes to the development of mastoiditis and more serious, life-threatening complications. Extension of the infectious process beyond the mastoid system can lead to a variety of intracranial and extracranial complications including meningitis, epidural and intracerebral abscesses, vascular thrombosis, osteomyelitis, and abscesses deep within the neck. Signs of clinical deterioration in a patient with otitis media shou…

Obliteration of mastoid and middle ear for severe trauma to the temporal bone.

A review of four cases of severe trauma to the temporal bone involving fracture of the external auditory canal indicates that adipose obliteration of the mastoid and middle-ear cleft with closure of the external auditory canal is the optimal method of repair. In two of four patients, one-stage repair was successful using this technique, and in the other two patients, obliteration was required as a revision operation after cholesteatoma developed from previous procedures.
