Is phantom tooth pain a deafferentation (neuropathic) syndrome? Part I: Evidence derived from pathophysiology and treatment.

Phantom tooth pain is a syndrome of persistent pain or paresthesia in teeth and other oral tissues that may follow dental or surgical procedures such as pulp extirpation, apicoectomy, tooth extractions, or exenteration of the contents of the maxillary antrum. It can also occur when nerves are injured after trauma to the face or even after routine inferior alveolar nerve blocks if the needle pierces the nerve sheath. In the case of tooth extraction, the pain is found in the edentate area. After periodontal surgery, pain or paresthesia is located in the gingiva. The incidence of phantom tooth pa

Effects of methylmercury chloride on rat incisor odontoblasts and dentinogenesis.

Methylmercury chloride, a well-known nerve cell toxicant, was administered to 40 rats in 2 groups, in doses of 10 mg/kg and 20 mg/kg. Histomorphologic investigation of maxillary incisors after 2 and 4 days revealed granular and hydropic changes in parts of the odontoblastema. Deposition of interglobular dentin was seen after 7 and 11 days. The most vulnerable cells seemed to be the mantle dentin-producing odontoblasts, and the alterations found are probably due to interference with the rough endoplasmic reticulum.

[Localized invasive intracranial aspergillosis with multiple cranial nerve failure -- case report and review of the literature].

Contrary to the more frequent hematogenously spread cerebral aspergillosis, localized invasive intracranial aspergillosis is a fungal infection that can also occur in patients who are not severely immunosuppressed. This illness can be effectively treated in some of these patients by early and rigorous therapy. Localized invasion of the fungus, generally from one of the nasal sinuses, causes intracranial growth mainly along the base of the skull and larger vessels, where fibrous, granulomatous tissue develops. This generally leads to damage of the cranial nerves (primarily I-VI) as well as local

The extradural approach to lesions involving the cavernous sinus.

We report our experience with five lesions exclusively involving the entire cavernous sinus in which an essentially extradural surgical approach was used. There were two cases of cavernous haemangioma, two cases of meningioma and one case of fungal granuloma. The dural cover of the superior orbital fissure, and mandibular and maxillary divisions of the fifth nerve was dissected along with the dura of the lateral wall of the cavernous sinus. The presence of a relatively large intracavernous bulge due to the tumour assisted in this dissection. The contents of the cavernous sinus were exposed fro

Experimental trigeminal glycerol injection. Electrophysiologic and morphologic effects.

The mechanism by which glycerol abolishes the pain of tic douloureux with minimal trigeminal deafferentation remains unknown. To study the action of glycerol, ten cats underwent unilateral retrogasserian injection of anhydrous glycerol. The contralateral (control) side was injected with saline. Glycerol injection increased the average latencies and reduced the average amplitudes of trigeminal brain-stem evoked potentials. Histopathologic examination disclosed focal demyelination, axonal swelling, endoneurial fibrosis, and neuronal loss. Evoked potentials were severely altered or abolished in c

Feline viral rhinotracheitis virus: explant and cocultivation studies on tissues collected from persistently infected cats.

Tissues from 16 feline viral rhinotracheitis (FVR) carrier cats and 15 controls were examined by the techniques of homogenisation, cocultivation and explant cultivation. The tissues examined included trigeminal ganglion and maxillary nerve, olfactory lobe and nerve endings, tonsils, submandibular lymph node, spleen and parotid salivary gland. Most of the cats were shedding FVR virus in the oropharynx at the time the tissues were collected. No evidence of a persistent FVR virus infection was found in any of the tissues. Trigeminal ganglion explant cultures from six FVR virus carrier cats were s…

Occipital lobe infarction: a rare presentation of bilateral giant cavernous carotid aneurysms: a case report.

BACKGROUND: Cavernous carotid aneurysm (CCA) represents 2-9% of all intracranial aneurysms and 15% of internal carotid artery (ICA) aneurysms; additionally, giant aneurysms are those aneurysms that are > 25 mm in size. Bilateral CCAs account for 11-29% of patients and are commonly associated with structural weaknesses in the ICA wall, secondary to systemic hypertension. CCAs are considered benign lesions, given the low risk for developing major neurologic morbidities (i.e., subarachnoid hemorrhage, cerebral infarction, or carotid cavernous fistula). Moreover, concurrent presentation with poste

Perineural Spread in COVID-Associated Rhino-Orbito-Cerebral Mucormycosis: A Single Tertiary Care Center Study.

BACKGROUND: Infection with rhino-orbito-cerebral mucormycosis (ROCM) has increased in the coronavirus disease (COVID) era, especially in diabetic patients. Rapid extension from the nose and paranasal sinuses to the orbit and brain is noted by osseous erosion, vascular invasion, and infrequently documented perineural spread. OBJECTIVE: To determine the incidence of perineural spread in ROCM cases and evaluate the common neural pathways involved, which help in prognosticating and triaging patients. METHODS AND MATERIALS: We evaluated demographic, clinical, and radiological findings of 270 pati

Craniofacial gunshot injury resulting in pseudoaneurysm of the left internal maxillary artery and Collet-Sicard syndrome.

Facial gunshot is a challenging medico-surgical emergency because of the complex neurovascular structure in the area. We present a patient who sustained a facial gunshot injury, with massive nasal and oral bleeding that resulted in shock. We review the management of gunshot injury to the internal maxillary artery, both in acute and chronic stages. In addition, we describe the clinical course of the rarely occurring Collet-Sicard syndrome, which involves injury to the 9th, 10th, 11th, and 12th cranial nerves associated with gunshot injury.

Contrast-free visualization of distal trigeminal nerve segments using MR neurography.

BACKGROUND AND PURPOSE: The 3-dimensional cranial nerve imaging (CRANI) sequence may assist visualization of anatomical details of extraforaminal cranial nerves and aid in clinical diagnosis and preoperative planning. In this study, we investigated the feasibility of using a combined CRANI and magnetization-prepared rapid-acquisition gradient-echo (MPRAGE) imaging protocol to comprehensively identify trigeminal nerve projections. METHOD: We evaluated the detection of distal regions of three branches of the ophthalmic nerve (V1), three branches of the maxillary nerve (V2), and five branches of…

Three-dimensional topography of rat trigeminal ganglion neurons using a combination of retrograde labeling and tissue-clearing techniques.

The trigeminal nerve is the sensory afferent of the orofacial regions and divided into three major branches. Cell bodies of the trigeminal nerve lie in the trigeminal ganglion and are surrounded by satellite cells. There is a close interaction between ganglion cells via satellite cells, but the function is not fully understood. In the present study, we clarified the ganglion cells' three-dimensional (3D) localization, which is essential to understand the functions of cell-cell interactions in the trigeminal ganglion. Fast blue was injected into 12 sites of the rat orofacial regions, and gangli

Infratemporal fossa infection with inferior alveolar nerve involvement.

Abscess of the infratemporal fossa is a rare complication which can be difficult to diagnose. It occurs mostly due to dental infection, tooth extraction, fractures and/or infections involving the maxillary sinus. This condition can be life threatening if not dealt with immediately. The maxillofacial surgeon must be aware of the symptoms and clinical findings of the infection of the infratemporal fossa in order to initiate the treatment as soon as possible. In this case report, a patient with an infratemporal fossa infection presenting with an unusual symptom of inferior alveolar nerve involvem

Surgically assisted rapid maxillary expansion in two cases of osteopathia striata with cranial sclerosis.

Osteopathia striata with cranial sclerosis (OS-CS) is a rare skeletal dysplasia characterized by linear striations of the long bones, osteosclerosis of the cranium, and extraskeletal anomalies. Osteosclerosis of the cranial and facial bones can lead to disfigurement and to disability due to the pressure on the cranial nerves. We report two cases of OS-CS where surgically assisted rapid maxillary expansion was performed for widening the extremely narrow maxilla. One should be aware of the disease-related problems and the possible complications that might occur with this type of patient.

Classification and treatment of zygomatic fractures: a review of 1,025 cases.

The treatment of zygomatic fractures varies among surgeons, and the cosmetic and functional results are frequently less than optimal. A treatment guideline based on a simple classification of zygomatic fractures is presented. The emphasis is placed on the indications for closed and open reduction, consistent methods of three-dimensional alignment and fixation, and the management of concomitant infraorbital rim and orbital floor fractures. Postoperative results with regard to infraorbital nerve and maxillary sinus dysfunction, malar asymmetry, and orbital complications in the treatment of 1,025

The thoracic muscular system and its innervation in third instar Calliphora vicina Larvae. II. Projection patterns of the nerves associated with the pro- and mesothorax and the pharyngeal complex.

We describe the anatomy of the nerves that project from the central nervous system (CNS) to the pro- and mesothoracic segments and the cepha … ontal nerve (FN) and innervate the cibarial dilator muscles (CDM) which mediate food ingestion. The maxillary nerve (MN) innervates the sensory terminal organ (TO), ventral organ (VO), and labial organ (LO) and comprises the motor pathways to the mouth hook (MH) elevator, MH depressor, and the labial retractor (LR) which opens the mouth cavity. An anastomosis of …

Peripheral nerve carbonic anhydrase activity and chronic acetazolamide treatment of rats.

Examination of cranial nerves shows that the sensory infraorbital branch of the trigeminal nerve contains many carbonic anhydrase-reactive axons whereas axons of the motor facial nerve are non-reactive. This motor/sensory axon staining difference holds for both cranial and spinal nerves. Chronic treatment with acetazolamide produced no apparent changes in carbonic anhydrase histochemical activity or the structure of peripheral nerve fibers.

Radiofrequency thermoablation of the infraorbital nerve for posttraumatic trigeminal neuropathic pain: illustrative case.

BACKGROUND: Trigeminal neuropathic pain (TNP), particularly posttraumatic TNP (PTTN), is often refractory to medical therapy and difficult to manage surgically. Although radiofrequency thermoablation (RFA) of the gasserian ganglion is established, peripheral nerve RFA remains underutilized, particularly in anatomically complex cases. OBSERVATIONS: A 62-year-old man developed severe left maxillary (V2) facial pain following sinus surgery, refractory to multiple pharmacological therapies and prior gasserian balloon rhizotomy. A diagnostic infraorbital nerve block produced significant temporary
