The effects of the Le Fort I osteotomy on the periodontium.

Two age-matched populations of equal size (n = 40), one having orthodontic therapy and the other combined orthodontic therapy and orthognathic surgery, were evaluated for their periodontal status 1 to 10 years posttherapy. The parameters investigated were plaque index, gingival index, tooth mobility, width of keratinized tissue, probing depth, gingival recession, and attachment level. No significant differences were found (P less than .05). Within the surgery group, patients with maxillary osteotomies segmentalized between the central incisors (n = 11) and between the canines and second premol

Bimaxillary osteodistraction for the treatment of facial asymmetry in adults.

Nine patients with hemifacial microsomia or facial asymmetry were treated between April 1998 and November 1999. The ages of the patients ranged from 21 years to 45 years (mean: 24.6 years); six were female and three were male. The follow-up period ranged from 6 months to 24 months (mean: 15.3 months). The operative procedure was based on Ortiz Monasterio's simultaneous mandibular and maxillary distraction technique. This technique frees only the pterygo-maxillary junction of the affected side, thereby leaving the nasal septum and the pterygo-maxillary junction of the unaffected side intact. It

[Maxillofacial fractures. Surgical and prosthetic procedures].

A patient presented complex fractures of the face and skull with leg trauma after a traffic accident. Maxillofacial repair used sophisticated procedures: bone grafts, muscle flaps and multiple osteotomies. After reconstructing the mid face and the mandibular repair, bone implants were used to stabilize the maxillary prosthesis. The psychological and functions problems raised in this case are discussed considering the occupational and social outcome.

Location of the nasolacrimal canal in relation to the high Le Fort I osteotomy.

The positional relationship between the high-level Le Fort I osteotomy and the nasolacrimal canal was studied in standard posteroanterior and lateral cephalograms made of 100 adult dry skulls after orienting the Frankfort horizontal of each skull to the natural horizontal plane. The simulated high-level Le Fort I osteotomy in all of the 100 skulls was under the level of the inferior orifice of the nasolacrimal canal by a mean distance of 5.2 mm (range, 0.5 to 11.5 mm). The average distance between the canal orifice and a line which extended from the lacrimal fossa to the anterior attachment of

Summers' technique modification for sinus floor elevation using a connective tissue graft. A case report.

The sinus floor elevation technique with Summers' osteotome is simpler and less invasive than access through the lateral wall of the sinus; however, it is susceptible to a higher level of sinusal membrane rupture. The objective of the present study was to display a modification of the osteotomy technique by connective tissue interposition in order to weaken the impact of sinus cortical fracture, and therefore prevent perforations in the sinusal membrane. A case is reported in which a patient presented with an absent tooth #4 for osseointegrated implant rehabilitation. Maxillary sinus elevation

Orthodontic-surgical retreatment of facial asymmetry with occlusal cant and severe root resorption: A 3-year follow-up.

Our objective was to report the orthodontic and surgical retreatment of a patient who had undergone a prolonged orthodontic treatment with extractions, but who had unsatisfactory results and persistent side effects. The man, aged 25 years 3 months, sought treatment with major complaints of facial and smile asymmetries. The clinical examination showed a mandibular deviation to the right and a maxillary occlusal cant. A Class II Division 1 subdivision right was observed. Radiographic examination showed extensive root resorptions in the maxillary second premolars and absence of the 4 first premol

Bone transport and bone graft using auto-tooth bone for alveolar cleft repair.

We herein report the application of a combination of maxillary bone transport and auto-tooth bone grafting for alveolar cleft repair using autogenous extracted teeth developed in Korea.A 9-year-old female patient suffering from unilateral cleft lip and palate was treated with this method. After sagittal interdental right-sided maxillary osteotomy was performed completely between #11 and #12 to the nasal floor, alveolar maxillary bone (#11, 21) was transported in the planned direction and the alveolar cleft was closed. At the end of the transporter activation period, soft tissue in the cleft wa…

Premaxillary Repositioning in the Severe Form of Bilateral Cleft Lip and Palate.

Severe forms of bilateral cleft lip and palate remain a challenging issue. Although nasoalveolar molding dramatically improves overall treatment success, the position of the premaxilla often remains dislocated. The authors attempted to relocate the malpositioned premaxilla into the correct position to obtain the correct three-dimensional (3D) maxillary arch structure and growth. Eight patients with severe bilateral cleft lip and palate were treated with premaxillary osteotomy for premaxilla repositioning. The position of the premaxilla was measured directly using cephalometry. Two raters inclu…

Maintenance of Segmental Maxillary Expansion: The Use of Custom, Virtually Designed, and Manufactured Palatal Appliances Without the Use of an Occlusal Splint.

Expansion of the maxilla using a segmental Le Fort I osteotomy is a routine and commonly used procedure in the treatment of dentofacial deformities. Although orthognathic surgery has seen tremendous advancement in technology, including virtual surgical planning and customized hardware, some room for advancement exists in the maintenance of surgical maxillary expansion. Traditionally, this has been accomplished with retention of a maxillary occlusal splint 4 to 8 weeks postoperatively. This article introduces some novel techniques to help maintain this expansion, including the intraoperative tr

Radiological examination of the relationship between the pterygomaxillary junction and fracture pattern.

BACKGROUND: Le Fort 1 (LF1) osteotomies are widely used to correct midface deformities. To move the maxilla freely, the ptery-gomaxillary junction (PMJ) must be separated. When performing this osteotomy, the pterygoid plate must remain intact. The objective of this study was to evaluate relationship between the anatomical features of the PMJ and fracture patterns in LF1 osteotomy. METHODS: Pre-operative and post-operative cone-beam computed tomography images of 41 patients (82 samples) who have undergone LF1 osteotomy surgery were radiologically evaluated. Morphologic measurements of the pter

CAD/CAM Engineered Patient-Specific Impants as a Reposition Device in Le Fort I and Modified Subcondylar Osteotomies: Case Report of Facial Deformity Correction in Acromegaly.

Acromegaly is a medical condition where elevated growth hormone or insulin-like growth factor I levels cause several changes in the craniofacial soft and hard features. We report the correction of facial deformity and posterior open bite with Le Fort I and modified subcondylar osteotomies in a patient affected by acromegaly. Computer-aided design and manufacturing generated saw and drill guides were used to perform osteotomies and segment removal. The placement of the patient-specific implants (PSIs) was guided by predesigned drill holes ensuring the required and planned movement of the jaws a

Measurement of Lacrimal Sac Fossa Using Orbital Computed Tomography.

PURPOSE: To evaluate the clinical usefulness of measuring the lacrimal sac fossa length using orbital computed tomography in normal Koreans. METHODS: The authors retrospectively evaluated 140 patients (70 males and 70 females) who underwent orbital computed tomography at Guro Hospital and who had no history of orbital disease or orbital trauma. Computed tomography scans of the right orbit, including the proportion of the lacrimal bone and maxillary bone that comprise the lacrimal sac fossa, were evaluated at 3 different axial planes (lower, middle, and upper levels). Additionally, the mid-poi

Two-stage mandibular setback employing modified Dingman's mandibular body osteotomy with step for large antero-posterior dentofacial deformity correction: a report of two cases.

Mandibular prognathism is a well-known skeletal discrepancy with varying prevalence rates demographically. Patients with severe prognathism may experience difficulties speaking and chewing food as well as problems with psychosocial adjustment, indicating the need for surgical correction. In cases of single-stage larger mandibular setbacks, complications such as reduction in the pharyngeal airway and instability can be encountered. Therefore, two-stage orthognathic surgery could be recommended. Two patients (17-year-old female and 16-year-old female) reported with large mandibular antero-poster

When the vomeronasal organ is removed or damaged during a maxillary osteotomy, the inhibitory function is lost, the patient loses the involuntary ability to exclude inappropriate mates, may become less committed to an existing mate, or even become promiscuous.

Postoperative stability of conventional bimaxillary surgery compared with maxillary impaction surgery with mandibular autorotation for patients with skeletal class II retrognathia.

We aimed to compare the postoperative stability of conventional bimaxillary surgery (with bilateral sagittal split osteotomy) with that of maxillary impaction surgery (with mandibular autorotation without bilateral sagittal split osteotomy) in patients with skeletal class II retrognathia. Patients were assigned to have conventional bimaxillary surgery (conventional group, n=6) or mandibular autorotation (experimental group, n=7). Measurements were made using serial lateral cephalometric radiographs taken immediately preoperatively (T0), immediately postoperatively (T1), and one year later (T2)

Wassmund osteotomy for excessive gingival display: a case report with three-dimensional facial evaluation.

AIMS: This case report describes the management of an adult patient presenting with a skeletal Class II malocclusion with congenitally missing lower second premolars, maxillary dentoalveolar protrusion, and an excessive gingival display in full smile. METHODS: The treatment plan consisted of a combination of a segmental osteotomy to reposition the anterior part of the maxilla after upper premolar extractions and a bilateral sagittal split osteotomy (BSSO) to advance the mandible. Because the upper left permanent second molar required extraction, the upper left third molar was moved mesially d

Sclerostin-Neutralizing Antibody Enhances Bone Regeneration Around Oral Implants.

BACKGROUND: Dental implants are an important option for replacement of missing teeth. A major clinical challenge is how best to accelerate bone regeneration and reduce the healing time for functional restoration after implant placement. A sclerostin-neutralizing antibody (Scl-Ab) has been shown to enhance alveolar bone formation and fracture repair. The aim of this study was to investigate the effects of systemic administration of Scl-Ab on dental implant osseointegration and bone regeneration in an experimental alveolar ridge tooth extraction model. MATERIALS AND METHODS: To investigate the
