[Clinical study of transcrestal maxillary sinus floor elevation with the disk-up sinus reamer].

OBJECTIVE: To evaluate the clinical effect of the disk-up sinus reamer (DSR) applied to transcrestal maxillary sinus floor elevation with simultaneous placement of implants. METHODS: Thirty-seven patients underwent transcrestal maxillary sinus floor elevation with fifty-one implants placed simultaneously using the DSR. The residual bone height (RBH) was 3 to 8 mm, (5.61 ± 1.61) mm on average. The safety of this technique and the pain index during the operation was evaluated. The final prostheses were restored in 3-6 months postoperatively. The follow-up period was 3 to 24 months. The stabilit…

Maxillary sinus nonodontogenic myxoma extending into the sphenoid sinus and pterygopalatine fossa: case report.

Myxomas are benign mesenchymal tumors of uncertain etiology. They can arise from odontogenic tissue, as well as from the sinonasal tract and from the facial and temporal bones, apparently from nonodontogenic mesenchyme. Although histologically benign, myxomas are locally invasive. Histopathology demonstrates a characteristically hypocellular lesion with loose stellate cells. We describe the case of a 48-year-old woman with a nonodontogenic maxillary sinus myxoma that invaded the sphenoid sinus and pterygopalatine fossa. We removed the mass via a combined endoscopic and gingivobuccal approach. …

Evaluation of maxillary sinus anatomy by cone-beam CT prior to sinus floor elevation.

PURPOSE: Surgical complications during sinus floor elevation are frequently caused by septa in the maxillary sinus. In this study, the prevalence of septa was retrospectively determined via three-dimensional imaging to assess the necessity for appropriate imaging prior to sinus floor elevation. MATERIALS AND METHODS: The cone-beam computed tomography scans of 1,029 consecutive patients were evaluated to assess the prevalence of septa, taking into account patient age and sex as well as the number and orientation of septa and their locations. If septa were present, the height was measured and t…

Incidence of sinus membrane perforation in transcrestal graftless maxillary sinus augmentation: a meta-analysis.

OBJECTIVES: This meta-analysis aimed to evaluate Schneiderian membrane perforation incidence and complications in graftless transcrestal sinus floor elevation. MATERIALS AND METHODS: Several databases (PubMed, Embase, Cochrane Library, Web of Science and Scopus) were searched for studies reporting perforation rates, early implant failure (≤12 months) and postoperative complications. Two reviewers independently selected studies, extracted data and assessed bias. Random-effects meta-analysis was performed. Evidence certainty was graded using the GRADE system. RESULTS: Among 14 studies comprisi

Analysis of Sinus Inflammation Associated With Maxillary Sinus Fungal Ball Based on CT Imaging.

Background: Paranasal sinus fungal balls usually occur unilaterally, and the maxillary sinus is most commonly involved. However, other sinuses could be concomitantly inflammed, and this phenomenon is rarely discussed. Aims/Objectives: To evaluate the severity of maxillary sinus fungal ball (MSFB) and the occurrence of sinus inflammation in paranasal sinuses according to the image findings and analyze the potential correlations. Material and Methods: A total of 1226 cases of MSFB were divided into 2 groups according to ostiomeatal complex obstruction on computed tomography. The potential correl…

New classification of maxillary sinus contours and its relation to sinus floor elevation surgery.

BACKGROUND: It is complicated to select an appropriate sinus floor elevation and the procedure for sinus floor elevation lacks of consensus. Sinus contour plays an important role in choosing a surgery approach. But there are still no published articles revealing the influence of sinus contours to sinus floor elevation surgery. PURPOSE: We propose a new classification depending on sinus contours from cone-beam computed tomography (CBCT), analyze clinical characters of different types, and investigate the relationship between sinus contours and sinus floor elevation. MATERIALS AND METHODS: We

Variation in bucco-palatal maxillary sinus width does not permit a meaningful sinus classification.

OBJECTIVES: The bucco-palatal sinus width (SW) appears as relevant factor for graft consolidation after maxillary sinus (MS) floor augmentation. The present study aimed to assess (a) SW at different height levels of posterior teeth, (b) possible factors influencing SW, and (c) whether a simple/meaningful sinus classification based on SW is possible. METHODS: The following parameters were recorded on computed tomographies of 76 edentulous and 86 partially edentulous maxillary quadrants displaying 383 tooth sites in total: (a) alveolar ridge height, (b) -area, (c) -width 2 mm apical to the alve…

Modified In-Office Maxillary Balloon Sinus Dilation for Post-Procedure Sinus Monitoring and Access.

Introduction  As experience grows with in-office balloon sinus dilation (BSD) procedures, technique modifications will evolve to meet specific needs. The major disadvantage with the maxillary BSD procedure is that the intact uncinate process prevents visualization of and instrument access into the maxillary sinus both intraoperatively and postoperatively. Combining a partial or complete uncinectomy procedure with maxillary BSD could be beneficial in selected patients with certain maxillary sinus problems. Objective  The primary objective of this study was to see if, in selected patients, combi…

Spontaneous maxillary sinus rupture in the absence of pre-existing sinus disease: case report.

INTRODUCTION: Spontaneous fracture of the maxillary sinus is usually associated with enophthalmos and pre-existing sinus disease. CASE REPORT: We present a case of spontaneous maxillary sinus fracture without enophthalmos and with no preceding history of trauma or evidence of sinusitis. DISCUSSION: The closest condition to that presented is silent sinus syndrome. The differences between our case and this syndrome are reviewed. There are no previously reported cases of lateral wall maxillary fracture and associated facial surgical emphysema following nose-blowing.

Radiological findings of the post-sinus lift maxillary sinus: a computerized tomography follow-up.

The purpose of this paper is to present radiological findings of a short-term (8 to 10 months) computerized tomography (CT) follow-up study on 1-stage maxillary sinus lift cases. Pre- and postoperative dental CT scans of 21 patients (24 sinuses) after sinus lift procedures were compared. CT scans were used to assess newly formed bone and its interface with the implants, condition of the sinus membrane, evidence of buccal window cortication, and presence of any sinus pathology. Of the 57 implants placed, 28 had bone fully covering the implant on all sides which did not extend above the apical p…

Radiologic assessment of diseased mucosa of the maxillary sinus after functional endoscopic sinus surgery.

We attempted to evaluate postoperative mucosal changes and symptomatic improvement in 99 patients who underwent functional endoscopic sinus surgery from September 1991 through August 1992. The patients were divided into 2-, 4-, 6-, and 12-month postoperative groups. Thickness of the maxillary sinus mucosa measured at the midpoint of the lateral sinus wall on a follow-up ostiomeatal unit computed tomogram (OMU CT) was compared with that of preoperative OMU CT. Postoperative endoscopic findings of the maxillary sinus and changes in presenting symptoms such as rhinorrhea, nasal obstruction, facia…

Evaluation of maxillary sinus width on cone-beam computed tomography for sinus augmentation and new sinus classification based on sinus width.

PURPOSE: The degree of difficulty in performing lateral window sinus augmentation may depend on the morphology of the maxillary sinus. The aim of this was to measure the distances between the medial and lateral sinus wall (sinus width [SW]) at different levels and apply those SW values to formulate a new sinus classification. MATERIALS AND METHODS: Edentulous sites adjacent to maxillary sinuses with inadequate ridge height (RH; <10 mm) were included from cone-beam computed tomography database in the University of Michigan. SW was measured at the heights of 5, 7, 10, 13, and 15 mm from alveola…

Sinus Reformation for Management of Maxillary Posterior Bone Atrophy.

INTRODUCTION: Implant rehabilitation in the posterior maxilla with critical residual bone height (≤6 mm) requires maxillary sinus floor augmentation techniques, although controversy exists regarding the necessity of heterologous bone grafts. Although some protocols consider grafting essential for osteogenesis, recent evidence suggests that modern implants, through a "tent-pole" effect, may eliminate this need, thereby avoiding its drawbacks (increased cost, surgical time, and risk of complications such as sinusitis). This retrospective study compares implant survival and complications between …
