Systematic method for initial rapid analysis of maxillofacial computed tomography scans for surgeons in training.

BACKGROUND: Reading maxillofacial computed tomography (CT) scans is a basic skill for any surgeon who operates on the face. A standard systematic approach for how to read a maxillofacial CT scan from a surgeon's perspective is lacking in literature, particularly for those in the early stages of training. We present a method of initial rapid analysis which our residents in training have responded to and report to be conceptually simple to apply. METHODS: The senior author reviewed over 250 consults for face trauma and performed a detailed composite of the most common elements of each consult a

Civil liability in oral & maxillofacial surgery: Α systematic review.

Maxillofacial surgery is a surgical specialty with anatomical, functional, and aesthetic requirements, which makes it a field at increased risk of medical negligence and subsequent legal claims. The review was conducted in accordance with the PRISMA guidelines and used international medical and legal databases. Studies that analyzed court decisions, insurance claims, or recorded compensation data related to maxillofacial surgery were included. The extracted data included, among others, the year and country of publication, the causes of action, and the amounts of financial compensation. Most la

Maxillofacial fractures related to work accidents.

Work-related maxillofacial fractures were studied retrospectively over a six-year period. There were 98 cases accounting for 4.5% of all facial bone fractures treated in our hospital between 1981 and 1986. Of the patients, 89.8% were male. The mean age of the injured was 36.4 years. The incidence of work-related maxillofacial fractures was 0.37 per 1000 workers. Most of the injuries (66%) occurred in factories and construction work. Such work was associated with an to 15 times higher risk of maxillofacial fracture than service and office work. Of the fractures, 20.4% were sustained on the way

[Facial perforation trauma: an unusual injury in a skiing accident].

Facial perforation injuries are very rare. We describe a case of a 48-year-old man who sustained a perforation trauma from an 11 cm long wooden tree branch in the middle of the face in a skiing accident. He suffered from additional injuries, such as fractures of the ribs and hand, but was neurologically without pathologic findings and was cardiopulmonary stable. The branch penetrated the head from the sinus maxillaris through the maxilla just missing the internal and external carotid arteries and ending just short of the cervical vertebra. The patient was transported to a center for oral and ma

Airway management in patients with facial trauma.

Management of the airway is a major concern in patients with maxillofacial trauma (gunshot wounds, facial fractures, cervical spine injuries, laryngotracheal injuries) because a compromised airway can lead to death. The method of intubation to use in these patients remains a controversial topic. Although there are many options available, each one has specific indications, and the choice will ultimately depend on the patient's situation and the expertise of the trauma team. In general, endotracheal intubation is usually not a viable option, but nasotracheal intubation and tracheotomies can be p

Mandibular fractures in India during the Second World War (1944 and 1945): analysis of the Snawdon series.

The records of Major J. W. E. Snawdon of the No. 2 Indian Maxillofacial Unit provide a rare and detailed insight into the treatment of mandibular fractures during the Second World War. Notable features were the high frequency of civilian-type injuries, the considerable delays between injury and definitive treatment, the lengthy periods of intermaxillary fixation required, the high incidence of infections and the common occurrence of delayed union. Despite these problems, only 12% of fractures resulted in non-union, usually when these were missile injuries with considerable destruction. Reporti

Fabrication of orbital prosthesis by two different methods in patients with post-COVID-19 rhino-orbital maxillary mucormycosis: A case series.

Neoplasms, congenital disorders, fungal infections, and traumatic injuries are the predominant causes of orbital defects. Various retentive mechanisms such as application of adhesive, utilization of mechanical undercuts, and implant-supported attachments are generally used in the maxillofacial prosthesis. In the orbital region, the result of magnet-retained attachments is favorable compared with other mechanisms. Different advantages of the magnet-retained prosthesis are less manual dexterity needed during insertion or removal and better maintenance of hygiene. The skin-implant interface and t

Adapting the University of Washington Quality of Life Questionnaire for Oral Cancer to trauma patients.

Assessing the health-related quality of life (HRQOL) in maxillofacial trauma patients is essential for understanding the holistic impact of injury. Whilst validated tools exist for oral cancer, trauma-specific HRQOL questionnaires are lacking. The University of Washington Quality of Life (UOW-QOL) questionnaire for oral cancer was adapted to evaluate maxillofacial patients as a pilot study. The aim was to capture functional and psychosocial challenges unique to trauma, thereby improving patient care. Thirty-five patients who sustained facial injuries then underwent surgery at the Royal London

Radiologic screening for midfacial fractures: a single 30-degree occipitomental view is enough.

BACKGROUND: The standard radiologic screening views for maxillofacial trauma in the accident and emergency department have been the Waters (30-degree occipitomental [OM]) view, the Caldwell (posteroanterior) view, and the lateral view. We wanted to see if a single 30-degree occipitomental view would be sufficient to screen for maxillofacial trauma (excluding nasal fractures). METHODS: Over a 1-year period, each of 730 patients had three radiologic views obtained for maxillofacial trauma at our emergency department. A consultant emergency physician read the single OM view for injury, and then

Mechanisms of trigeminal nerve injuries.

Injuries to the trigeminal nerve branches are a known and accepted risk in oral and maxillofacial surgery. It is prudent for the practitioner to explain the risks to patients as part of the informed consent process and to recognize and document the presence of nerve injury postoperatively. Patients should be referred to a surgeon experienced in microsurgical techniques in a timely fashion for evaluation and possible surgical intervention if an injury is not resolving.

The surgical treatment of reinforced steel bar injury penetrating the skull base and maxilla-mandibular area.

Penetrating injuries with reinforced screwed steel bar in the skull base represent a unique challenge for oral maxillofacial surgeons. Management of these injuries is complicated by associated injuries and the proximity to vital neurovascular structures. A 35-year-old man was admitted to our hospital because of injury due to a downward fall upon a reinforced steel rod. Radiologic studies of the skull base revealed that the steel bar traversed the temporomandibular space between the left cervical spine and the mastoid process to the space between the inner side of the left mandibular ramus and

The effects of nasomaxillary injury on future facial growth.

The appearance of results of injury to the columella, the nasal septum, and the nasal bones, in particular, has been well described. Anomalies of the maxilla and global facial balance secondary to nasomaxillary injury are less well known. Three cases involving children, aged 11, 14, and 17 years, who had suffered nasomaxillary injury at least 8 years earlier as a result of physical beating, were studied with the use of photographs and architectural craniofacial lateral cephalometric radiographic analysis. The architectural craniofacial analysis of Delaire produced a graphic representation of t
