Eosinophilic gastroenteritis is a rare entity, and although usually idiopathic, it may arise from gastrointestinal infections. We report a case of a 33-year-old woman from Vietnam presenting with acute abdominal pain, new-onset eosinophilic ascites, peripheral eosinophilia, and a positive Toxocara antibody. The patient had no recent international travel, known animal host contact, or other toxocariasis risk factors. She was treated with ivermectin and mebendazole with complete resolution of symptoms. This case emphasizes the consideration of a broad differential for eosinophilic ascites, inclu…

Ocular thelaziosis by Thelazia callipaeda is an emerging disease that affects primarily dogs, but also cats, foxes and other wild carnivores, as well as humans. Three clinical cases of unilateral conjunctivitis caused by Thelazia nematodes were detected in Border Collie, a dog breed intolerant to the macrocyclic lactones. Animals came from southwestern Spain, on the border with Portugal. Eight worms were collected and identified molecularly as T. callipaeda by amplification and sequencing of partial cytochrome c oxidase subunit 1 gene. Oral treatment with mebendazole 20mg/kg (Telmin(®)) was ef…

While intestinal parasites are infrequent causes of acute diarrhea, investigations in travellers, children, homosexuals and immunocompromised persons who have had diarrhea for more than 2-4 weeks should include stool analyses for intestinal parasites. Intestinal helminthiases are treated with pyrantel pamoate, mebendazole, thiabendazole, albendazole, praziquantel or niclosamide. For treatment of intestinal protozoa, ornidazole (or other benzimidazoles), diloxanide furoate and co-trimoxazole are drugs of first choice; at present spiramycin is recommended for treatment of cryptosporidiosis in AI…

We report a case of left-sided extended pelvic infestation of hydatid disease. The destruction of the hip joint, os pubis, larger parts of os ileum and the femoral head made pelvic resection and prosthetic replacement necessary. Recurrence of a hydatid cyst 5 years later in the left groin originating in the femur was treated with a second femoral resection and total hip replacement. Because of instability, the pelvic replacement also had to be changed to a custom-made CAD replacement. This surgical procedure was combined with chemotherapy by mebendazole. The need for radical resection in ossea…

It is reported on a 63-year-old female patient who fell ill 20 years after an echinococcus cyst of the liver with a multiple parasite-caused cyst-formation in the lung which was detected by chance and was already calcified at that time. On account of inoperability a conservative treatment with mebendazole (2,000 md/die, for 4 months). During a one-year follow-up period a drastic involution of the lung cysts with improvement of the general condition of the patient appeared. The affection of the liver, however, remained uninfluenced. The ELISA on echinococcus was unchanged strongly positive also…

OBJECTIVES: To summarise age- and intensity-stratified associations between human hookworm infection and anaemia and to quantify the impact of treatment with the benzimidazoles, albendazole and mebendazole, on haemoglobin and anaemia in non-pregnant populations.
METHODS: Electronic databases (MEDLINE, EMBASE, PubMed) were searched for relevant studies published between 1980 and 2009, regardless of language, and researchers contacted about potential data. Haemoglobin concentration (Hb) was compared between uninfected individuals and individuals harbouring hookworm infections of different inten…

A 13-year-old boy was diagnosed as having intestinal capillariasis by gastroduodenoscopy. He presented with a 10-month history of chronic abdominal pain and diarrhea. The boy had stayed in central Thailand and had eaten uncooked fish and raw shellfish. Gastroduodenoscopy showed normal jejunal mucosa although histology revealed flattened villi, crypt proliferation, acute inflammation, and eosinophilic granulomata. An egg of Capillaria philippinensis was also seen. The child was treated with mebendazole for 30 days. He had gained six kilograms by the time of his last follow-up.

Intestinal capillariasis was diagnosed in a 30-year-old fisherman from Malihan Village, Khoozestan Province, Iran. He reported a more than two month history of diarrhea, borborygmus, muscular atrophy, and debility. Eggs, larvae, and adults of Capillaria philippinensis were found in his feces. Treatment with mebendazole 400 mg per day for 25 days resulted in clinical and parasitological cure. The patient had reported eating a few small viable fish almost two months before clinical symptoms appeared. This is the first report of this infection from the Middle East.

The authors report the evaluation of therapeutic efficiency of imidazole derivatives (flubendazole and mebendazole) against human larval cestoda. 31 patients were treated: 28 had hydatid cysts, 2 had alveolar echinococcosis, and 1 had cerebral cysticercosis. Results after several months follow-up (up to 6 years) show an arrest in development of lesions and absence of development of new cyst after leakage of hydatid fluid. Although interpretation of such results is difficult, imidazole derivatives seem efficient in high dosages (100 mg/kg), given over 1 or 2 years, without side effects.

A 37-year-old Italian man presented with acute fever, headache, confusion and right arm clumsiness. Computerised axial tomography of the head showed right cerebellar, left parietal and frontal lobe haemorrhagic infarcts. An X-ray film of the chest showed a circular mass adjacent of the heart. On aspiration of the mass lesion during bronchoscopy, we found hydatid hooklets. A left ventriculogram showed this cyst to be attached to the pericardium. Subsequently, we performed left thoracotomy and the hydatid cyst was excised (though incompletely). The patient was treated with mebendazole and made a…

Parasite infestation is still associated with significant morbidity and mortality. Hookworm infestation is a very rare cause of pancreatitis. This parasitic infestation might be asymptomatic. Acute pancreatitis as a result of the hookworms migrating into the ampulla of Vater with chronic inflammation was a very rare complication. Were port a case of hookworm infestation that was associated with significant complication of recurrent pancreatitis. The patient was treated with mebendazole. He was asymptomatic and had gained weight at the 3-month follow-up. Our case demonstrates that pancreatitis …

Abnormal eggs of Trichuris trichiura were found in the stools of one of the patients during a study on the prevalence of intestinal parasitoses among an institutionalized population. The abnormalities observed included great variation in shape, size, and color. Similar atypical whipworm eggs have been reported in patients after treatment with mebendazole, thiabendazole, tetracloroethylene, and dithiazanine. Apparently some anthelminthics have an effect on the reproductive system of female T. trichiura, resulting in production of abnormal eggs, which could lead to misdiagnosis of the infection,…

A 31 year old Turkish woman patient was hospitalized with a febrile obstructive jaundice. Despite extensive diagnostics (clinical laboratory values, sonography, computed tomography and endoscopic retrograde cholangiography), emergency laparotomy had to be carried out without definitive clarification of the cause of the obstruction. Intraoperatively, bile duct infiltration by Echinococcus cysticus was established. In the further course under pharmacotherapy with mebendazole without detection of cysts in the liver and in regression of the serological parameters, there was a recurrence after half…

Two cases of cerebral echinococcosis in children emphasize the need to consider the Echinococcus in the differential diagnosis of children with CNS signs and symptoms. One patient died because of delay in seeking medical attention. The other is now a 22-year-old college student with no evidence of recurrence more than five years after removal of Echinococcus cysts from both brain and liver. The newer modalities available for diagnosis are discussed. Therapy with mebendazole is reviewed; this relatively new, highly effective anthelmintic is well tolerated and has been apparently highly effectiv…
