Disease modifying anti-rheumatic drug treatment cycles and rates of flare in juvenile idiopathic arthritis-a tale of Sisyphean endeavour?

OBJECTIVES: Withdrawal of DMARD therapy to determine need for ongoing medication is common in JIA management. Little is known about how often patients undergo trials of medication discontinuation or the outcomes of successive trials. This study examined DMARD treatment cycles in a JIA cohort to determine how frequently medication withdrawal trials occur over follow-up and if relapse rates change over consecutive cycles. METHODS: This was a retrospective review of longitudinal cohort data of children with DMARD-treated JIA diagnosed between 2010 and 2014. Extracted data included demographics,

Hepatic transplantation with perioperative and long term anticoagulation as treatment for Budd-Chiari syndrome.

Conventional medical and surgical management of Budd-Chiari syndrome is often unsuccessful. In this communication, we report the results of 19 hepatic transplants done for 17 patients suffering from Budd-Chiari syndrome. The first patient who had a transplant did not receive anticoagulant therapy during the postoperative period, and recurrent thrombosis of the hepatic veins in the newly transplanted liver rapidly developed. Sixteen patients who subsequently underwent transplantation were managed using a philosophy of early postoperative anticoagulant treatment when feasible. Using this approac

Do comprehensive medication review completion rates predict medication use and management quality?

OBJECTIVES: Little is known about how Medicare Part D Comprehensive Medication Review (CMR) affects quality of medication use and management. In this study, changes of plan level CMR completion rates over time were examined and their associations with medication use and management related (MUMR) quality measures. METHODS: Using Medicare Part D plan Star Rating and contract information data, a longitudinal data set was developed with CMR completion rate and 17 MUMR measures. T-tests and one-way Analysis of Variance were used to examine the variation of CMR rates between contracts and over time

Use of Prescription Sialagogues for Management of Xerostomia in Chronic Graft-versus-Host-Disease.

The aim of this study is to analyze utilization patterns of prescription sialagogues for management of xerostomia in patients with chronic graft-versus-host disease (cGVHD) after allogeneic hematopoietic cell transplantation (alloHSCT). There have been several small reports describing the clinical use of sialagogue therapy in the management of patients with cGVHD. While these reports suggest that sialagogue therapy is safe and effective in this unique patient population, the numbers of patients reported, and overall evidence base, remain limited. The objective of this study was to characterize

Medication-overuse headache: can anti-CGRP monoclonal antibodies replace withdrawal?

INTRODUCTION: Medication-overuse headache (MOH) is a secondary headache disorder arising from the excessive use of acute headache medications, most commonly triptans and nonsteroidal anti-inflammatory drugs (NSAIDs). Traditional management relies on education strategies and drug withdrawal followed by preventive therapy, but relapse rates remain high and detoxification is not always feasible. AREAS COVERED: This review summarizes current evidence on anti-calcitonin gene-related peptide (CGRP) monoclonal antibodies in migraine patients with medication overuse or MOH. A structured literature se

Corticosteroids for the treatment of Duchenne muscular dystrophy: a safety review.

INTRODUCTION: Duchenne muscular dystrophy (DMD) is an X-linked genetic disorder characterized by progressive muscle degeneration and weakness, caused by mutations in the dystrophin gene. DMD has effects in early age with significantly shortened lifespan and deteriorated quality of life in the second decade, creating an urgent need to develop better therapeutic options. Corticosteroid medication therapy is an integral tool for the management of DMD and several therapeutic options have been recently approved for use. AREAS COVERED: A comprehensive literature search was completed to examine effi

An examination of the socio-demographic correlates of patient adherence to self-management behaviors and the mediating roles of health attitudes and self-efficacy among patients with coexisting type 2 diabetes and hypertension.

BACKGROUND: Patients with coexisting type 2 diabetes and hypertension generally exhibit poor adherence to self-management, which adversely affects their disease control. Therefore, identification of the factors related to patient adherence is warranted. In this study, we aimed to examine (i) the socio-demographic correlates of patient adherence to a set of self-management behaviors relevant to type 2 diabetes and hypertension, namely, medication therapy, diet therapy, exercise, tobacco and alcohol avoidance, stress reduction, and self-monitoring/self-care, and (ii) whether health attitudes and

Association of osteonecrosis of the jaws and bisphosphonate pharmacotherapy: dental implications.

Bisphosphonates are drugs of choice in the management of a variety of bone disorders including osteoporosis, Paget's disease and bone cancer. Recently there have been increasing reports of a possible relationship between bisphosphonate therapy and osteonecrosis of the jaws. Osteonecrosis may occur following extractions or dental surgery and, in some cases, may appear spontaneously. Because of the potentially serious nature of these complications and the failure of exposed bone to heal, dentists must be aware of recommended precautions for the management of patients taking bisphosphonate medica

Cluster headaches. How to recognize and treat them.

Cluster headaches must be distinguished from migraine, trigeminal neuralgia, and sinusitis, since these conditions require different therapy. Cluster headaches are so unique, however, that the history alone should be adequate for diagnosis. Prophylactic use of medication is the best approach to management, although determination of the best regimen may take time and require patience of both physician and patient.

Cardiac Rehabilitation to Optimize Medication Regimens in Heart Failure.

Cardiac rehabilitation (CR) is an inherently patient-centered program that provides holistic care to adults with cardiovascular conditions to promote lifelong health and fitness, facilitate self-care and self-efficacy, and improve clinical outcomes. CR offers an excellent platform for patient-centered optimization of medication regimens for older adults with heart failure through its potential to address several aspects of care that have historically served as major challenges to clinicians-diuretic management, the use of guideline-directed medical therapy, review and reconciliation of noncard

Patient-Centered Treatment of Chronic Migraine With Medication Overuse: A Prospective, Randomized, Pragmatic Clinical Trial.

BACKGROUND AND OBJECTIVES: Overuse of symptomatic (i.e., acute) medications is common among those with chronic migraine. It is associated with developing frequent headaches, medication side effects, and reduced quality of life. The optimal treatment strategy for patients who have chronic migraine with medication overuse (CMMO) has long been debated. The study objective was to determine whether migraine preventive therapy without switching or limiting the frequency of the overused medication was noninferior to migraine preventive therapy with switching from the overused medication to an alterna

Development of a Practice Tool for Primary Care Providers: Medication Management of Posttraumatic Stress Disorder in Veterans with Mild Traumatic Brain Injury.

Posttraumatic stress disorder (PTSD) and comorbid mild traumatic brain injury (mTBI) are highly prevalent in veterans who served in Iraq [Operation Iraqi Freedom/Operation New Dawn] and Afghanistan [Operation Enduring Freedom]. Complicated psychotropic medications are used for treatment of PTSD and comorbid mTBI symptoms lead to polypharmacy related complications. Primary care providers (PCPs) working in Community Based Outpatient Clinics (CBOCs) are usually burdened with the responsibility of managing this complicated medication regimen or relevant side effects. The PCPs do not feel equipped

Management of postoperative nausea and vomiting in the context of an Enhanced Recovery after Surgery program.

The concept of Enhanced Recovery after Surgery (ERAS) emerged at the turn of the millennium and quickly gained footing worldwide leading to the establishment of institutional ERAS protocols and subspecialty guidelines. While the use of postoperative nausea and vomiting (PONV) prophylaxis predates ERAS by a significant extent, the emergence of ERAS amplified the importance of antiemetic prophylaxis in perioperative care and drew attention to the truly multifactorial nature of postoperative gastrointestinal dysfunction. The following discussion will review key paradigms behind PONV prophylaxis a

Endothelial Function Is Impaired in Patients Receiving Antihypertensive Drug Treatment Regardless of Blood Pressure Level: FMD-J Study (Flow-Mediated Dilation Japan).

Hypertension is associated with endothelial dysfunction. Blood pressure significantly correlates with endothelial function in antihypertensive drug-naive subjects. The purpose of this study was to determine whether treatment status affects the relationship between blood pressure and endothelial function. We measured flow-mediated vasodilation (FMD) in 2297 subjects, including 1822 antihypertensive drug-naive subjects and 475 treated hypertensive patients. FMD significantly decreased in relation to increase in systolic blood pressure (8.2±3.1% in subjects with systolic blood pressure of <120 mm

Hypertension Canada's 2017 Guidelines for Diagnosis, Risk Assessment, Prevention, and Treatment of Hypertension in Adults.

Hypertension Canada provides annually updated, evidence-based guidelines for the diagnosis, assessment, prevention, and treatment of hypertension. This year, we introduce 10 new guidelines. Three previous guidelines have been revised and 5 have been removed. Previous age and frailty distinctions have been removed as considerations for when to initiate antihypertensive therapy. In the presence of macrovascular target organ damage, or in those with independent cardiovascular risk factors, antihypertensive therapy should be considered for all individuals with elevated average systolic nonautomate
