A new approach for the concomitant surgical treatment of trapeziometacarpal joint osteoarthritis and carpal tunnel syndrome through the same incision is described. The technique was used in 25 patients (20 women, five men; mean age, 56 years). At a mean follow-up of 27 months, there was complete disappearance of the symptoms of carpal tunnel syndrome in 20 of the 25 patients, incomplete but substantial relief in four patients, and no improvement in one patient. We report a case of reconstruction of the first metacarpal by using an extended vascularized bone graft taken from the distal part of the second metacarpal. Following a gunshot injury, a 57 year-old man presented with a thumb amputated at the interphalangeal joint level and an index finger amputated at the metacarpophalangeal joint level. Aneurysmal bone cysts of the hand skeleton are rare (only 3 to 4 % of all ABC; Tillmann and v. Torklus 1966 ) and require a differentiated treatment. In the presented case, the fourth metacarpal bone of the right hand was affected (Enneking stage 3). On account of its rapid growth, radical excision was performed and the resected metacarpal bone was replaced by a bicortical iliac crest graft. The bone graft healed in without complications, the functional result is satisfactory. On twelve-month follow-up there were no signs of relapse.

Foreign bodies within soft tissues are common in children. They may cause a chronic inflammatory reaction that can result in abnormal findings on radiographs, including lytic or blastic osseous changes. These radiographic findings can mimic both benign and malignant processes. In cases where the history is uncertain and the foreign body is not recognized, magnetic resonance (MR) imaging can make a specific diagnosis and direct appropriate therapy.

The case records, radiographs, and nuclear bone scans of 58 horses with stress reactions or stress fractures of the proximal palmar aspect of the third metacarpal bone (MC3) were reviewed. Fifty-six fractures of the metacarpal occurring in 53 athletes were studied from September 1985 to December 1986, regarding mechanism of injury, type of fracture, type of treatment, and time lost from sport. Age range of the patients was 8 to 28 years with greater than 77% being in the 14 to 18 year age range, the high school athlete. The trapeziometacarpal (TM) joint is often the site of painful arthrosis, particularly in women at the onset of the fourth decade. It is generally associated with capsuloligamentous laxity of the joint or with work requiring the repetitive use of the thumb and index finger and thus work which is typically done by women. It may also be of traumatic origin following joint fracture, but this is not discussed in the present paper. The treatment of this syndrome by stabilisation of the joint is described.

Fracture-dislocation of the ulnar carpometacarpal (CMC) joints may result in malunion or arthropathy when it is missed or inadequately treated. Partial resection arthroplasty of the ulnar CMC joints was done in 16 patients to treat malunion subsequent to dorsal fracture-dislocation. This procedure is recommended as a reliable means of treatment that preserves the important motion on the ulnar side of the hand.

Pre- and post-operative radiological appearances of various isolated congenital malformations of the hand are described, as observed in a personal series of 52 cases. A cheiro-lumbar dysostosis is described, an association of constitutional stenosis of the lumbar canal due to the shortness and hypertrophy of the posterior arch of the vertebrae and of a dysostosis of the hand also due to shortness and hypertrophy of metacarpals (brachymetacarpy) and of phalanges (brachyphalangy). This cheiro-lumbar dysostosis is frequent and described in 6 patients suffering from lumbar back pain and sciatic.

Since 1969, a slightly modified silicone-rubber neurosurgical burr-hole cover has been used for a type of interposition arthroplasty of the carpometacarpal joint of the thumb.
