To compare metal concentrations in tibia tissues with various metallic implants, SUS316L stainless steel, Co-Cr-Mo casting alloy, and Ti-6Al-4V and V-free Ti-15Zr-4Nb-4Ta alloys were implanted into the rat tibia for up to 48 weeks. Thirty healthy cashiers continuously exposed to nickel in coins were tested in vivo and in vitro for the presence of metal contact allergy. A traditional epicutaneous test and lymphocyte transformation test were used. We tested for nickel, cobalt and chromium sensitivity. Seven of the 30 cashiers were patch test positive and 3 were in vitro positive to nickel sulphate. Two were in vivo positive to cobalt and only one in vitro positive. None was chromium allergic. BACKGROUND: Metal-on-metal implants have been used extensively for hip resurfacing arthroplasty (HRA). When failure occurs from reaction to metal wear debris, implant loosening or instability, conversion to total hip replacement (THR) has been the most common treatment. BACKGROUND: Metal-on-metal (MOM) hip prostheses have a higher failure rate than conventional prostheses and leaching of cobalt and chromium has been linked to cardiomyopathy. We screened MOM subjects to evaluate if cobalt and chromium are related to subclinical cardiac dysfunction.
METHODS: A single centre, non-randomised, observational study using echocardiography in 95 patients who had undergone MOM hip prostheses, and 15 age matched controls with non-MOM hip replacement. Serial plasma cobalt and chromium levels were recorded, and data compared by tertiles of cobalt exposure.

Metal on metal bearings were reintroduced into the United States for use in total hip arthroplasty approximately a decade ago. We evaluated a consecutive series cohort of 148 patients with 169 hips who underwent THR using a metal on metal bearing surface with a modular acetabular shell that had fixation augmentation with dome screws. At 3 to 8 year follow-up, average 4.7 years, only one component was revised (for femoral loosening). 3 hips demonstrated proximal femoral osteolysis and 3 hips demonstrated pelvic osteolysis.
