The clinical, roentgenographic, phonocardiographic, ECG, and echocardiographic data were evaluated in 40 consecutive middle-aged and elderly male patients with echocardiographically detectable systolic prolapse of mitral valve leaflets. Prolapse was present during more than half of systole in 31 patients and was holosystolic in six patients. In most instances, both leaflets prolapsed during systole. The closing velocity and excursion of the anterior leaflet were frequently increased particularly in association with evidence of mitral insufficiency. A majority of the patients had cardiac sympto

Brain Age Gap has been associated with dementia in old age. Less is known relating brain age gap to dementia risk-factors or cognitive performance in middle-age. Cognitively healthy, middle-aged subjects from PREVENT-Dementia had comprehensive neuropsychological, neuroimaging and genetic assessments. Brain Ages were predicted from T1-weighted 3T MRI scans. Cognition was assessed using the COGNITO computerized test battery. 552 middle-aged participants (median [interquartile range] age 52.8 [8.7] years, 60.0% female) had baseline data, of whom 95 had amyloid PET data. Brain age gap in middle-ag

BACKGROUND: Understanding the hierarchy of functional impairment in older adults has helped illuminate mechanisms of impairment and inform interventions, but little is known about whether hierarchies vary by age. We compared the pattern of new-onset impairments in activities of daily living (ADLs) and instrumental ADLs (IADLs) from middle age through older age.
METHODS: We conducted a cohort study using nationally representative data from 32 486 individuals enrolled in the Health and Retirement Study. The outcomes were new-onset impairment in each ADL and IADL, defined as self-reported diffic

While the concept of 'stress' in the modern sense is a twentieth-century innovation, many of the symptoms we associate with the modern condition appear in historical materials going back many centuries. But how did premodern people understand and experience these symptoms and their relation to sleep? This study focuses on the rich materials from the central middle ages in Western Europe, a period during which understandings of the body, mind, emotions and sleep were radically different from the present. It analyses two examples, nightmares and insomnia, disease categories which illustrate medi

BACKGROUND: One-third of older adults fall each year. Orthostatic hypotension (OH) has been hypothesized as an important risk factor for falls, but findings from prior studies have been inconsistent.
METHODS: We conducted a prospective study of the association between baseline OH (1987-1989) and risk of falls in the Atherosclerosis Risk in Communities (ARIC) Study. Falls were ascertained during follow-up via ICD-9 hospital discharge codes or Centers for Medicare & Medicaid Services claims data. OH was defined as a drop in systolic blood pressure (SBP) ≥20mm Hg or diastolic blood pressure (DBP

Otitis media is a common disease in childhood. In adults, the disease is relatively rare, but more frequently associated with complications. Possible reasons for this discrepancy are age-related differences in pathogen exposure, anatomy of the Eustachian tube and immune system. The objective of this study was to analyze the relationship between age and the mucosal immune system in the middle ear. It is hypothesized that genes involved in the middle ear immune system will change with age. A comprehensive assessment of these genetic differences using the techniques of complementary DNA has not b

Proceeding from a comprehensive literature survey the urgency of acute abdomen conditions in elderly and senile patients is thoroughly discussed. Not infrequently, geriatric surgeons face serious difficulties of diagnostic and therapeutic character relating to the severity of clinical picture and elevated postoperative lethality, explaining in turn the great surge of interest in the problem. Recent achievements along this line, surgery in particular, provide for adequate conditions for performing early surgical interventions with reduced intraoperative risk in elderly and old age patients pres

Effects of 10, 20 and 30 mg midazolam on sleep and on performance the next day were studied in six healthy adult males aged between 47 and 53 years. The study was double-blind, placebo-controlled and included another rapidly eliminated benzodiazepine (brotizolam, 0.25 mg) as an active control. With 10 mg midazolam, sleep-onset latency was quicker, and the duration and percentage of stage 2 sleep was increased over the first 6 h. Effects of 20 and 30 mg midazolam were similar to each other. Sleep onsets were earlier, total sleep times and stage 2 sleep were increased, and the sleep efficiency i

IMPORTANCE: Elevated blood pressure (BP) in adolescence has been linked to higher risk of cardiovascular disease mortality, as well as surrogate markers of atherosclerosis, such as carotid intima-media thickness and coronary artery calcification. However, these markers do not fully capture the complex spectrum of subclinical atherosclerotic cardiovascular disease.
OBJECTIVE: To examine the association between systolic and diastolic BP in adolescence and atherosclerosis in middle age, measured by coronary computed tomography angiography (CCTA).

BACKGROUND: Mobility decline often begins in midlife, and early identification of individuals at risk of accelerated deterioration can enable timely prevention. However, there is no validated self-report instrument that specifically assesses mobility in the middle-aged population.
METHODS: The Mobility in Middle-Age Questionnaire (MMQ) was developed through a 7-step Delphi process, consisting of 10 experts, involving item selection and content validation in both English and Hebrew, comprising 2 factors: (1) Current Mobility Ability and (2) 1-Year Mobility Change. Psychometric properties were

We present the unusual appearance on brain magnetic resonance imaging of a glioblastoma with an uncanny shape of a rabbit. By invoking fearsome "killer" rabbits depicted in the art and literature of the Middle Ages, this image is an eerie reminder of the current lethality of this disease. There is a pressing need for more effective treatments for glioblastoma.
