Mechanisms for purple discoloration of the plastic urine bag in purple urine bag syndrome (PUBS) were investigated. Activities of bacterial indoxyl sulfatase catalyzing the conversion of indoxyl sulfate to indigo (or indirubin) were detected in strong alkaline liquid media but not in normal ones. These enzyme activities were particularly high in simple and combined cultures of Proteus mirabilis and/or Klebsiella pneumoniae. A model of the catheterised bladder was used to test the ability of urease-producing urinary tract pathogens to encrust urethral catheters. Encrustation was assessed by determining the amounts of calcium and magnesium deposited on the catheters and visualised by scanning electron microscopy. Records of the Sickle Cell Clinics of the South Carolina Children's Rehabilitative Services from 1977 to 1990 were analyzed for cases of osteomyelitis. Cases included in the study demonstrated either positive bone cultures or blood cultures with correlating radiographs. Seven hundred seventeen charts were reviewed, and 16 cases of osteomyelitis were identified in 15 patients. Thirteen cases were due to Salmonella sp., one was due to Proteus mirabilis, one to Escherichia coli, and one to Staphylococcus aureus. The annual incidence of osteomyelitis in our series was 0.36%.
