Pharmacological Management of Psoriasis: Current Landscape and Future Perspectives.

Psoriasis is a relapsing, chronic, and inflammatory disease of the skin. However, its impact goes beyond just pathophysiology and takes a toll on the physical and psychological aspects of the health of the afflicted, lowering the quality of life significantly. It is also a mechanistically complex disease with a substantial immune component. Therefore, ongoing treatment strategies focus on targeting at least one immune component associated with the disease development and progression by employing biological agents like IL-1 inhibitors, IL-23 inhibitors, IL-36 inhibitors, and TNF-α inhibitors. Targeting chemokine signaling networks for therapeutics in skeletal disorders.

Chemokine signaling networks have emerged as pivotal regulators of skeletal homeostasis, integrating inflammation, angiogenesis, and immune activation with the processes of bone remodeling and regeneration. Recent evidence demonstrates that dysregulated chemokine-receptor interactions, including CCL2/CCR2, CCL5/CCR5, and CX3CL1/CX3CR1, disrupt the equilibrium between osteogenesis and osteoclastogenesis, thereby contributing to the pathogenesis of osteoporosis, osteoarthritis, multiple myeloma, and bone metastasis. This review synthesizes current insights into how chemokine-mediated signaling c

Autophagy in myositis, a dysregulated pathway, and a target for therapy.

Corticosteroids and immunosuppressants are the mainstay of therapy for idiopathic inflammatory myopathies (IIMs). However, a significant therapeutic challenge extends beyond mitigating inflammation with these agents in achieving meaningful improvements in muscle strength and physical function, a goal that remains largely unmet. IIMs encompass a heterogeneous group of autoimmune disorders, including dermatomyositis, polymyositis, necrotizing autoimmune myopathy, inclusion body myositis, and others, characterized by chronic muscle inflammation, progressive weakness, and fatigue. The etiology of

Molecularly directed therapy in cancers of unknown primary: A systematic review and meta-analysis.

BACKGROUND: Cancers of unknown primary (CUP) are associated with a high mortality rate, with limited therapeutic options available and platinum-based chemotherapy recommended as standard of care. Over the past decade, molecularly guided approaches aiming to adapt treatment strategies in patients with CUP based on predicted site of origin (site-specific approach) or genomic characteristics (tissue-agnostic approach) have been explored in clinical studies, with heterogenous findings identified. METHODS: PubMed/MEDLINE, Scopus, Web of Science, Embase, the Cochrane Library, and conference abstrac

Podocyte Metabolic Reprogramming and Targeted Therapy.

Podocytes, highly specialized glomerular epithelial cells, are essential for maintaining the filtration barrier integrity, yet they are particularly susceptible to metabolic stress. Recent advances have identified metabolic reprogramming as a central driver of podocyte injury in diverse glomerular diseases, including diabetic kidney disease and FSGS. Pathologic stimuli, such as hyperglycemia, lipotoxicity, oxidative stress, and inflammatory cytokines, lead to profound alterations in podocyte metabolism, encompassing dysregulation of lipid, glucose, amino acid, and ion handling and activation o

Optimizing outcomes and personalizing care with targeted agents in advanced cholangiocarcinoma.

Traditional chemotherapy and immunotherapy-based systemic treatments for locally advanced or metastatic cholangiocarcinoma have been associated with poor clinical outcomes driven partly by molecular heterogeneity promoting early treatment resistance and a higher toxicity profile associated with these regimens. Few patients are eligible for upfront surgical resection and clinical studies have been traditionally difficult to conduct due to the orphan nature of this disease. However, increasing use of genomic profiling in clinical practice have led to active investigations of aberrant albeit prom

Targeting autophagy in urological system cancers: From underlying mechanisms to therapeutic implications.

The urological system, including kidneys, ureters, bladder, urethra and prostate is known to be vital for blood filtration, waste elimination and electrolyte balance. Notably, urological system cancers represent a significant portion of global cancer diagnoses and mortalities. The current therapeutic strategies for early-stage cancer primarily involve resection surgery, which significantly affects the quality of life of patients, whereas advanced-stage cancer often relies on less effective chemo- or radiotherapy. Recently, accumulating evidence has revealed that autophagy, a crucial process in

SOHO State of the Art Updates and Next Questions | From Biology to Therapy: Progress in Hodgkin Lymphoma.

Classic Hodgkin lymphoma (HL) is a unique lymphoid malignancy where the malignant cells comprise only 1% to 2% of the total tumor cellularity. Over the past 2 decades, the treatment of HL has evolved drastically based on the advent of novel targeted therapies. Novel agents including programmed death-1 (PD-1) inhibitors, antibody-drug conjugates such as brentuximab vedotin, bispecific antibodies, and chimeric antigen receptor (CAR) T cell therapies have served to shape the management of HL in the frontline as well as the relapsed and refractory (R/R) setting. Some of these agents have been inco

Navigating the Current Landscape of Non-Clear Cell Renal Cell Carcinoma: A Review of the Literature.

Non-clear cell renal cell carcinoma (nccRCC) is an entity comprised of a heterogeneous constellation of RCC subtypes. Genomic profiling has broadened our understanding of molecular pathogenic mechanisms unique to individual nccRCC subtypes. To date, clinical trials evaluating the use of immunotherapies and targeted therapies have predominantly been conducted in patients with clear cell histology. A comprehensive review of the literature has been undertaken in order to describe molecular pathogenic mechanisms pertaining to each nccRCC subtype, and concisely summarise findings from therapeutic t

The α9α10 nicotinic acetylcholine receptor: a compelling drug target for hearing loss?

INTRODUCTION: Hearing loss is a major health problem, impacting education, communication, interpersonal relationships, and mental health. Drugs that prevent or restore hearing are lacking and hence novel drug targets are sought. There is the possibility of targeting the α9α10 nicotinic acetylcholine receptor (nAChR) in the prevention of noise-induced, hidden hearing loss and presbycusis. This receptor mediates synaptic transmission between medial olivocochlear efferent fibers and cochlear outer hair cells. This target is key since enhanced olivocochlear activity prevents noise-induced hearing

The Role of Noncoding RNA in the Pathophysiology and Treatment of Premature Ovarian Insufficiency.

Premature ovarian insufficiency (POI) is defined as a loss of ovarian function before the age of 40 years, with a prevalence rate estimated at approximately 1%. It causes infertility and is related to serious long-term health consequences, including reduced life expectancy, increased cardiovascular risk, decreased bone mineral density and neurological disorders. There is currently no effective therapy for POI that is widely available in clinical practice; therefore, the treatment of patients with POI is based on hormone replacement therapy. One of the recent advances in the understanding of th

[Endocrine Organ Dysfunction with Immune-Checkpoint Inhibitors].

Cytotoxic T-lymphocyte associated antigen 4(CTLA-4), programmed death 1(PD-1), and programmed death-ligand 1 (PD-L1)are referred to as immune-checkpoints. CTLA-4 is located on the surface of activated T-cells, therefore inhibiting binding of CD28 to B7 molecule on antigen presenting cells. The CTLA-4 pathway predominantly acts in lymph nodes. PD-1 is majorly expressed on T-cells. The PD-1 pathway is involved with tumor microenvironment. Immune-checkpoint inhibitors (ICIs)are monoclonal antibodies to these molecules and are promising novel agents for malignant tumor treatment. ICIs promote T-ce

The future of bypassing agents for hemophilia with inhibitors in the era of novel agents.

Bypassing agents are presently the standard of care for the treatment of bleeding episodes in patients with hemophilia and high-titer inhibitors and are also used for bleed prevention. Only two bypassing agents are available to patients, and these products trace their lineage to the 1970s (activated prothrombin complex concentrates) and the 1980s (recombinant factor VIIa). Given the limited repertoire of available products, clinicians have relied on experience, empirical observation, registry data and individualized care to improve clinical outcomes on a case-by-case basis. Research over the p
