Half a century of change: demographic trends and their clinical impact in acetylcholine receptor antibody-positive myasthenia gravis.

BACKGROUND: The impact of myasthenia gravis (MG) changing epidemiology on disease management remains unclear. We investigated how MG subtypes changing over the last 50 years have affected disease prognosis. METHODS: Adult patients with MG onset between 1970 and 2020, followed up at our center for ≥ 2 years, were included in this retrospective study. Demographics, MG subtypes, and antibody (Ab) status over the last 50 years were assessed. Treatment response and adverse events (AEs) were evaluated in acetylcholine receptor (AChR)-MG patients with onset between 2000 and 2020. Treatment goal was …

Assessing respiratory status in myasthenia gravis: limited value of the MG-ADL as a standalone tool compared with spirometry in a Danish cohort.

BACKGROUND: Patients with generalized Myasthenia Gravis (gMG) may develop respiratory involvement, which can be life-threatening. In clinical practice, the Myasthenia Gravis Activities of Daily Living scale (MG-ADL) is often used to assess patient-reported respiratory impairment, as spirometry can be time-consuming and require specialized equipment and expertise. However, it is unclear whether the MG-ADL is a sufficient screening tool for identifying respiratory insufficiency. METHODS: In this cross-sectional study with a longitudinal subgroup, patients completed the MG-ADL, the Myasthenia Gr…

Late-Onset Ocular Myasthenia Gravis-Like Symptoms During Erenumab Therapy for Chronic Migraine: A Case Report.

Chronic migraine (CM) is a disabling neurological disorder for which calcitonin gene-related peptide (CGRP)-targeted monoclonal antibodies, such as erenumab, provide effective prophylaxis. Although generally well tolerated, rare neuromuscular complications resembling ocular myasthenia gravis (MG) have been reported. A 46-year-old woman with CM achieved marked improvement with erenumab for over three years before developing diplopia and intermittent ptosis. Laboratory and imaging studies were unremarkable, but clinical suspicion for MG led to pyridostigmine therapy, resulting in rapid improveme…

Concurrent Nivolumab-Induced Myocarditis and Myasthenia Gravis: A Case Report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) such as nivolumab have improved 10-year overall survival rates up to 43% in advanced melanoma. They carry a risk of severe immune-related adverse events (irAEs) up to 9%-33%, including cardiotoxicity and neuromuscular complications. Acral melanoma is a rare subtype that is often diagnosed late and requires aggressive therapy with adjunctive immunotherapy. Here, we report a rare case of an elderly male who developed myocarditis and myasthenia gravis 3 weeks after receiving the first dose of nivolumab for stage IIB acral melanoma. CASE PRESENTATIO…

Engineered Bi-Specific AChR CAAR T Cells for Selective Elimination of Myasthenia Gravis B Cells.

Myasthenia gravis (MG) is an autoimmune neuromuscular disorder caused by autoantibodies targeting the nicotinic acetylcholine receptor (nAChR), which can lead to severe disability and life-threatening crises. Current therapies rely on broad immunosuppression and fail to achieve sustained remission in the majority of patients. Here, we report the development of AChR chimeric autoantibody receptor (CAAR) T cells engineered to selectively eliminate autoreactive B cells producing anti-AChR autoantibodies. T cells were co-transduced with CAARs expressing extracellular domains of the AChR α1 or β1 s…

Clinical characteristics and peripheral immune profile analysis of thymoma-associated myasthenia gravis with anti-titin antibodies: a multicenter, retrospective study.

BACKGROUND AND PURPOSE: Thymoma-associated myasthenia gravis (TAMG) patients with anti-titin antibodies exhibit uncertain clinical and immunological relevance. METHODS: We analyzed 91 TAMG patients (40 Titin + group, 51 Titin- group) treated between 2019 and 2025. Clinical features, severity scores, and peripheral immune markers were compared, and finally tested the correlation between the latter and severity. RESULTS: Titin + group had higher rates of MGFA ≥ III (55.00% vs. 25.49%, p = 0.004) and elevated QMG bulbar scores [3.00 (0.75-3.00) vs. 0.00 (0.00-0.00), p = 0.002]. They also showed…

Selective Depletion of Autoreactive Plasma Cells as a Novel Strategy to Treat Acetylcholine Receptor Antibody-Positive Myasthenia Gravis.

Myasthenia gravis (MG) is a chronic autoimmune disease mediated by autoantibodies targeting the neuromuscular junction and leading to muscle weakness. Although there are autoantibodies of different specificities, most MG patients have autoantibodies directed against the nicotinic acetylcholine receptor (AChR), in particular against the extracellular domain of the α1 subunit (αECD), containing the main immunogenic region (MIR). Here, we demonstrate an original approach to selectively deplete plasma cells secreting autoantibodies targeting αECD. An antibody-mediated cytotoxicity-engager (ACE) co…

Longitudinal omics reveals immune repertoire remodeling in myasthenia gravis patients post-tacrolimus therapy.

Myasthenia gravis (MG) is a T/B cell-driven autoimmune disease. The immunomodulatory mechanisms of the common immunosuppressant tacrolimus (TAC) on the immune repertoire are unclear. This study investigated TAC's immunomodulatory effects via high-throughput sequencing of peripheral blood mononuclear cells from four MG patients pre- and post-four months of TAC monotherapy, revealing dynamic T-cell receptor (TCR) and B-cell receptor (BCR) repertoire remodeling. The immune repertoire of MG patients was characterized by a skewed usage of TRBV gene families compared to healthy controls, indicating …

Opposing effects of efgartigimod and rozanolixizumab on serum albumin levels in patients with generalized myasthenia gravis.

Although efgartigimod and rozanolixizumab are both neonatal Fc receptor (FcRn) inhibitors for generalized myasthenia gravis (MG), they possess distinct molecular structures: efgartigimod is an engineered immunoglobulin G1 (IgG1) Fc fragment, whereas rozanolixizumab is an IgG4 monoclonal antibody. This study aimed to investigate whether these distinct molecular structures induce differential effects on serum albumin in a real-world setting. We conducted a single-center observational study in patients with generalized MG who newly initiated efgartigimod or rozanolixizumab, measuring serum IgG an…

A population-based legacy study of myasthenia gravis in Iceland: insights from a small Arctic nation.

Myasthenia gravis (MG) is a rare autoimmune neuromuscular disorder for which population-based estimates remain scarce, particularly in small, homogeneous settings. Leveraging Iceland's centralized healthcare, we reassessed MG epidemiology and clinical features in a nationwide legacy cohort. Cases diagnosed from 1981 to 2002 were identified through hospital databases, private neurologists, immunology laboratory records and the national MG association. Inclusion required characteristic symptoms plus at least one confirmatory test (response to acetylcholinesterase inhibitors, abnormal electrophys…

The SF3b complex in cancer: structural basis, molecular mechanisms, and therapeutic opportunities.

BACKGROUND: Acetylcholine receptor antibodies (AChR-Ab) are established biomarkers in myasthenia gravis (MG). In ocular MG (OMG), their presence has been linked to disease severity, treatment response, and progression to generalized MG (GMG); however, the relevance of AChR-Ab titers or seropositivity to ocular manifestations remains uncertain. OBJECTIVE AND METHOD: We retrospectively reviewed OMG patients treated at Rajavithi Hospital (2009-2024). AChR-Ab titers were measured using enzyme-linked immunosorbent assay (ELISA). Clinical and demographic factors associated with AChR-Ab seropositivi…

Women and Myasthenia Gravis.

Myasthenia gravis (MG) is a prototypical antibody-mediated autoimmune disorder of the neuromuscular junction, characterized by fluctuating skeletal muscle weakness and substantial morbidity. Although therapeutic advances have markedly improved survival and long-term outcomes, MG is not a gender-homogeneous condition. Women are disproportionately affected, exhibit a distinct bimodal age distribution, and experience the disease within unique biological and psychosocial contexts that shape presentation, disease course, quality of life, and treatment response. Accumulating evidence highlights sex-…

Thromboembolic risk in subcutaneous versus intravenous immunoglobulin therapy: a systematic review and narrative meta-synthesis.

Intravenous immunoglobulin (IVIg) therapy is associated with thromboembolic complications, including deep vein thrombosis (DVT), pulmonary embolism (PE), ischaemic stroke, and myocardial infarction (MI). Subcutaneous immunoglobulin (SCIg) achieves lower peak serum IgG concentrations and may confer a reduced thromboembolic risk. We systematically reviewed the comparative thromboembolic safety of SCIg versus IVIg across neurological and immunological indications. We searched PubMed, Embase, Cochrane Library, Web of Science, Scopus, and ClinicalTrials.gov from inception to December 2024. Studies
